The trapezo-metacarpal joint has a complex structure with specific anatomical, physiological, radiological and clinical characteristics. Several anatomical descriptions of this joint are available, and we believe one should know it well in order to understand its radiological problems. For instance a wrong projection may lead to overlook a fracture, especially in the trapezium. Several mechanisms have been suggested to explain the lesions of the base of the first metacarpal and of the trapezium. The therapeutic consequences of these considerations are not very important. Regardless of the lesi

In De Quervains dislocation fracture pseudarthroses of the scaphoid bone following closed reduction are very common, 59% being reported in a former publication. Exact adaptation of the fragments after reduction of the perilunar dislocation is essential for healing of the fracture. In cases where this is not possible by conservative means, open reduction and screwing of the scaphoid fracture is necessary for better result with a rate of pseudarthroses of only 16%.

The frequency fo secondary displacement in fractures of the distal end of the radius is very high and varies from 40 to 60% in various series. This results from the void in the epiphysis after reduction due to crushing of the cancellous bone. Secondary displacement can be easily avoided by the bipolar fixation method. This method is quite simple. Two strong Kirschner wires are driven into the radius and into the first metacarpal, and then incorporated in the cast after the reduction of the fracture. In this study we have followed 102 out of 250 patients treated by this method. The number of se

OBJECTIVE: To evaluate the clinical outcomes of primary anterior focus debridement, bone autograft, and internal fixation via transperitoneal approach in treating tuberculosis of the lumbosacral junction.
METHODS: From February 2002 to April 2007, 16 patients with tuberculosis of the lumbosacral junction underwent anterior radical debridement, autologous iliac bone graft (two pieces of iliac bone, 5 cm x 3 cm in size), and internal fixation via transperitoneal approach, including 4 males and 12 females aged 27-63 years old (average 38 years old). The course of the disease ranged from 6 to 18

PURPOSE: Three different methods of fixation used in acute disruption of the acromio-clavicular (AC) joint-namely, the coraco-clavicular Bosworth screw (CC Screw), a coraco-clavicular sling of Mersilene #5 tape (CC Sling), and a Hook Plate-were compared to baseline to see which could most closely replicate the stiffness of healthy cadaveric AC specimens (Intact).
HYPOTHESIS: It is hypothesized that the Hook Plate method, as compared with the other reconstructions tested, will be most similar mechanically to the intact AC joint with respect to present outcome measures.
METHODS: Five matched p

INTRODUCTION: The aim of this study was to improve the management of cervical tumor osteolysis. A new modular rod-screw implant system for the posterior instrumentation of the occipito-cervical, cervical and cervico-thoracic spine (neon occipito cervical system, Ulrich, Germany) is available since 2000. K-wire guided pedicle screws are used, CT-guided instrumentation is possible. Previous studies have demonstrated increased biomechanical stability compared to established posterior cervical systems.
METHODS: The cervical and cervico-thoracic spine of 8 patients (6 males, 2 females, mean age 62

STUDY DESIGN: A three-group design with consistent pullout strength measures.
OBJECTIVES: To determine pullout strength of three fixation types (unicortical screws, bicortical screws, wires) and to investigate their correlation with respect to occipital morphology.
SUMMARY OF BACKGROUND DATA: A secured, multidirectional occipitocervical fusion requires internal fixation. Devices secured at occipital protuberance were suggested to offer the greatest pullout strength because of this region's thickness.
METHODS: Twelve fresh human cadaveric occiputs were sketched with a grid delineating 21 fix

Maxillofacial trauma in the pediatric population is a relatively infrequent occurrence. Studies have demonstrated consistently that 5% of all facial fractures occur in children. The low percentage of facial fractures in this age group has been attributed, in part, to the lack of full pneumatization of the sinuses until later in childhood. Review of the literature indicates that boys are more commonly affected than girls and that the majority of pediatric facial fractures occur in children between 6 and 12 years of age. Motor vehicle accidents, falls, and blunt trauma are responsible for the la

The aim of this study was to evaluate functional and radiological results, tumor control, and complications of the surgical treatment of aneurysmal bone cysts (ABCs) in children by extended curettage using a mechanical burr and cauterization, grafting, and internal fixation in specific locations. Sixty-four children [38 males, 26 females, median age=10 years (range, 5-18 years)] with active or aggressive ABCs of long and flat bones were subjected to a median follow-up of 66 months (range, 28-130 months) following surgical treatment. The pathological fracture rate was 72%. Surgical procedures i

The aim of this multicenter study of 138 patients with scaphoid nonunions was to assess the prognostic factors of bone healing or failure after curative surgical treatment options: isolated bone grafting (30%), internal fixation (23%), or combined bone grafting and internal fixation (47%). Bone healing occurred in 75% of cases. Persistent nonunion was evident in 20% of cases; it was possible in 6%. The clinical and radiologic results were worse in the group of failures. Stepwise multiple logistic regression analysis was conducted to identify the factors of prognosis toward bone healing or fail

A left elbow fracture-dislocation in a 51-year-old female patient following a backward fall at home presented with a comminuted radial head fracture, coronoid fracture, and ruptured collateral ligaments. The patient underwent open reduction and internal fixation (ORIF) with a long-arm splint. Post surgery, aggressive rehabilitation was initiated to restore the range of motion (ROM). Compared to previous reports, this case demonstrates successful management of a severe elbow injury with excellent short-term outcomes. This case highlights the occurrence of the terrible triad in a patient without

Hip fracture is a common injury in elderly patients. In Japan, the number of super-old patients-age >90 years-with hip fractures has increased drastically over time. Available strategies for anaesthetic management for hip fracture surgery include general anaesthesia, neuraxial anaesthesia and peripheral nerve block. However, general and neuraxial anaesthesia are often avoided for various reasons, particularly in elderly patients. In recent years, peripheral nerve block has proven effective in various surgical procedures. Additionally, dexmedetomidine exhibits neuroprotective effects and has be

CASE: A 21-year-old woman presented with a chronic fracture dislocation of her left shoulder after striking a submerged tree during a watersport activity. She was initially treated at an outside hospital, where she underwent open reduction and internal fixation of her posterior scapula. She presented 8 weeks after index procedure with pain and severely decreased range of motion. Her comminuted and displaced fracture comprising 70% of the anterior superior glenoid was surgically reconstructed using a distal tibia allograft (DTA), resulting in a functional shoulder.
CONCLUSION: A DTA is a feasi

To investigate the occurrence of carpal ligamentous injuries in patients with fractures of the distal radius, the results of 122 wrist arthroscopies were analysed within a retrospective study. Indications for arthroscopy included suspected carpal instability according to radiographic findings and/or the necessity for internal fixation of the radius fracture. Arthroscopy revealed acute scapholunate ligament tears in 84 patients. Scapholunate separation was found to be of prognostic value for ligament tears. There was no association between ligamentous lesions and carpal angles or fracture dislo

The treatment of fractures of the posterior pilon is a timely issue. Restoration of the integrity of the incisura fibularis and subsequent anatomic reduction of the fibula are essential for reconstruction of the ankle mortise after trauma, and syndesmotic stability. Inappropriate treatment ultimately will lead to a poor functional outcome and quality of life. Open reduction and direct internal fixation through a posterolateral or posteromedial approach are increasingly preferred over indirect reduction and anteroposterior screw fixation. The posteromedial approach, although elegant, straightfo
