Clinical outcomes of using 4 + 5 extensor compartment pedicled vascularized bone graft in Kienböck's disease.

BACKGROUND: This study aimed to evaluate the mid-term clinical and radiological outcomes of a 4th-5th extensor compartment artery (4 + 5 ECA) pedicled vascularized distal radius bone graft in patients with Lichtman stage II and IIIa Kienböck's disease, with a particular focus on differences between disease stages and magnetic resonance imaging (MRI) evidence of lunate revascularization. METHODS: A retrospective review was conducted on 30 patients (31 wrists) treated with a 4 + 5 ECA pedicled vascularized bone graft between 2022 and 2024. Nineteen wrists were classified as Lichtman stage II an

A single antegrade intramedullary k-wire for fifth metacarpal neck fractures.

OBJECTIVES: Antegrade intramedullary nailing (AIMN) using 2k-wires seems to be superior to other modalities in treating displaced fractures of the fifth metacarpal neck (FFMN). Few reports demonstrated similar results retrospective with a single k-wire. The aim of the study is to describe our single k-wire technique and evaluate the related results. METHODS: This is a retrospective study of a continuous series of patients treated with a single intramedullary k-wire for FFMN. It includes 30 patients who met the criteria for surgery: a dorsal angulation of more than 30°, malrotation, or both. T

Association between bone mineral density, muscle strength, and vitamin D status in patients with myasthenia gravis: a cross-sectional study.

UNLABELLED: Myasthenia gravis (MG) patients had low proximal hip BMD, which could be explained by reduced muscle strength, elevated bone resorption markers, vitamin D deficiency, and increased PTH levels in those with MG compared to controls. INTRODUCTION: Muscle strength is closely correlated with bone mineral density (BMD) and vitamin D status. Here, we evaluated muscle strength, BMD, and vitamin D status in a large sample of Chinese patients with MG. METHODS: In this cross-sectional survey, 86 patients with MG without glucocorticoid treatment and 86 healthy controls were included. Serum l

[Amputation of the hand as last resort in severe complex regional pain syndrome].

In very rare cases, a complex regional pain syndrome type I (CRPS I) of the hand can take a serious, chronic, incurable course. We describe the case of a 36-year-old patient who after reconstruction of a scaphoid fracture developed such a condition. 9 years after the operation an amputation of the hand was performed at the request of the patient after various expert opinions had been obtained and legal action against the insurance provider was successfully concluded. Amputation of the hand can be discussed as a last resort for relief of suffering in cases of severe CRPS I.

Mini-screw fixation for the treatment of proximal interphalangeal joint dorsal fracture-dislocations.

PURPOSE: Surgical intervention may be necessary to treat unstable dorsal fracture-dislocations of the proximal interphalangeal (PIP) joint of the hand. One method of stabilization is open reduction and internal fixation (ORIF). The purpose of this study was to assess the outcomes of ORIF for unstable dorsal fracture-dislocations of the PIP joint using mini-screws via a volar approach. METHODS: A retrospective chart review with clinical follow-up evaluation was performed on 9 patients who had ORIF for unstable dorsal fracture-dislocations of the PIP joint. Low dose dehydroepiandrosterone affects behavior in hypopituitary androgen-deficient women: a placebo-controlled trial.

Thirty-eight women, aged 25-65 yr, with androgen deficiency due to hypopituitarism were treated with oral dehydroepiandrosterone (DHEA; 30 mg/d if <45 yr of age and 20 mg if > or =45 yr of age) for 6 months in a randomized, placebo-controlled, double blind study, followed by a 6-month open treatment period. The administration of DHEA raised the serum levels of DHEAS to normal age-related reference ranges and increased androstenedione and T to subnormal levels. Androgen effects on skin and/or pubic and/or axillary hair were observed in 84% (32 of 38) of the women after all received 6 months of

Camurati-Engelmann disease type II: progressive diaphyseal dysplasia with striations of the bones.

We recently found mutations of the transforming growth factor beta 1 (TGF-beta1) gene (TGFB1) in 9 families, in which progressive diaphyseal dysplasia (Camurati-Engelmann disease) is segregating [Kinoshita et al., 2000: Nat Genetics 26:19-20]. During the study, we encountered two unrelated girls, aged 17 and 11 years, who had clinical manifestations of the disorder, such as marfanoid habitus, waddling gait, muscular weakness, intense leg pain, flexion contracture of the hip and knee joints, delayed sexual development, increased serum alkaline phosphatase levels, and increased erythrocyte sedim

[Vascularized surgical rotation of a bi-articular trapezoid-trapeziometacarpal complex for the treatment of severe rhizarthrosis of the thumb].

UNLABELLED: A new surgical procedure has been conceived to avoid persistent pain and to restore the functionality and pinch strength of the basal joint of the thumb destroyed by degenerative osteoarthritis. Since January 1998 a double total joint transposition was performed on eight patients, switching the trapezoid-metacarpal joint and the trapezio-metacarpal joint. A 180 degrees rotation of the bi-articular complex (trapezoid-trapezio-metacarpal joints) in its entirety was achieved, while preserving the intrinsic ligaments and vascular supply insured by the dorsal branch of the radial artery

[Our experience with repair of the scapholunate ligament using the MITEK bone anchor].

PURPOSE OF THE STUDY: A complete rupture of the scapholunate (SL) interosseal ligament results in palmar flexion of the scaphoid and dorsiflexion of the lunate that leads to disintegration of the carpal circle and the development of scapholunate dissociation with dorsal intercalary segment instability. If the injury is not treated properly or early, the abnormal position of the scaphoid and lunate results in degenerative changes of the wrist shown by X-ray and referred to as scapholunate advanced collapse (SLAC). The aim of this study was to evaluate the results of our method used for reconstr

A comparison of bridging external fixation vs. combined external fixation for preventing radial height loss in older patients with distal radius fractures.

Distal radius fractures are common injuries in older adult patients. The present prospective study aimed to compare the efficacies of bridging external fixation (BEF) and combined external fixation (CEF) in preventing radial height loss in older patients with distal radius fractures. Between January 2018 and December 2022, a total of 118 patients with distal radius fractures, aged 55-70 years, were randomly subjected to either BEF or CEF. Standard radiographs were used to measure radiological parameters, including radial height, palmar tilt, radial inclination, ulnar variance, articular step-o

DHEA and its transformation into androgens and estrogens in peripheral target tissues: intracrinology.

A new understanding of the endocrinology of menopause is that women, at menopause, are not only lacking estrogens resulting from cessation of ovarian activity but have also been progressively deprived for a few years of androgens and some estrogens originating from adrenal DHEA and androstenedione (4-dione). In fact, serum DHEA decreases by about 60% between the maximal levels seen at 30 years of age to the age of menopause. This decreased secretion of DHEA and DHEA-S by the adrenals is responsible for a parallel decrease in androgen and estrogen formation in peripheral tissues by the steroido

Long-term Outcomes of 3-dimensional-printed Cutting Guides for Long Bone Sarcoma Resection and Intercalary Allograft Reconstruction: An Updated Case Series.

BACKGROUND: The use of 3-dimensional (3D)-printed cutting guides for resection of long bone sarcoma may offer advantages over traditional free-hand or navigational osteotomy, including improved margin control and reconstruction accuracy. We evaluated long-term surgical and oncologic outcomes of limb salvage procedures using 3D-printed cutting guides, with updated follow-up from our previously published case series and the addition of new cases focusing on margin status, bony union, and local recurrence. MATERIALS AND METHODS: We retrospectively reviewed 9 patients from our surgical database w

Augmented versus non-augmented locking-plate fixation in proximal humeral fractures.

AIMS: Proximal humeral fractures are the third most common fracture among the elderly. Complications associated with fixation include screw perforation, varus collapse, and avascular necrosis of the humeral head. To address these challenges, various augmentation techniques to increase medial column support have been developed. There are currently no recent studies that definitively establish the superiority of augmented fixation over non-augmented implants in the surgical treatment of proximal humeral fractures. The aim of this systematic review and meta-analysis was to compare the outcomes of
