Real-world experience with rituximab for multiple sclerosis treatment in a Brazilian tertiary center.

BACKGROUND: Rituximab, an anti-cluster of differentiation 20 (anti-CD20) monoclonal antibody, has been widely used off-label for multiple sclerosis (MS) treatment, demonstrating high effectiveness and a favorable safety profile. However, real-world data from Brazil, particularly within the public healthcare system, remain scarce. OBJECTIVE: To assess the real-world effectiveness and safety of rituximab for MS treatment in a Brazilian tertiary center. METHODS: We conducted a single-center, observational study at Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo (HCFM…

Psychological correlates of suicidal ideation in people with multiple sclerosis: a cross-sectional study.

BACKGROUND: People with multiple sclerosis (PwMS) are vulnerable to suicidal ideation (SI), yet the relative contribution of mood symptoms and disease-related factors remains unclear. This study aimed to determine the frequency of SI and its independent correlates among PwMS. METHODS: This cross-sectional study was conducted on 366 PwMS (312 relapsing-remitting MS and 54 progressive MS) in Isfahan, Iran, from September 2023 to April 2024. Baseline demographic and clinical data were collected, and SI, anxiety, and depression were evaluated. Linear and logistic regression analyses were employed…

Role of Caveolin-1 in Inflammation: Genetic Predisposition and Potential Implication for Multiple Sclerosis.

Multiple Sclerosis (MS) is a chronic, immune-mediated disorder of the central nervous system characterized by leukocyte infiltration, inflammation, demyelination, and progressive neurodegeneration. Susceptibility to MS is influenced by genetic factors, including variants within the human leukocyte antigen (HLA) region, (notably HLA-DR15), and multiple single nucleotide polymorphisms that modulate T cell function and immune regulation. Clinically, early manifestations such as visual disturbances, sensory deficits, fatigue, and impaired coordination often precede more advanced features, includin…

Sphingosine-1-Phosphate Is a Key Signaling Molecule in Normal Conditions and in Multiple Sclerosis.

Sphingosine-1-phosphate (S1P) is one of the most extensively studied bioactive signaling molecules of sphingolipid metabolism, which plays a … neurodegeneration, and neuroprotection. In pathogenesis of the demyelinative diseases, particularly multiple sclerosis, disruption of the "sphingolipid rheostat" is observed - a shift toward predominance of pro-apoptotic ceramides and relative decrease in the S1P levels, which promotes prevalence of the neuroinflammatory and neurodegenerative processes over remyelin …

Comorbidities and lifestyle changes as predictors of the multiple sclerosis prodrome: A prospective cohort study.

BACKGROUND: Multiple sclerosis (MS) is increasingly recognized as having a prodromal phase, characterized by non-specific symptoms preceding demyelinating events, yet prospective, longitudinal assessments of relevant signs before the first MS symptom are lacking. OBJECTIVES: To investigate whether a wide range of signs and symptoms occurred more frequently in individuals who later developed MS than in the underlying cohort. METHODS: We followed 113,876 women from the Nurses' Health Study II with prospectively collected data on lifestyle, health indicators, and diet. We examined the trajector…

Upper Cervical Cord Area as a Biomarker of Conversion to Secondary Progressive Multiple Sclerosis.

OBJECTIVE: This study assessed whether upper cervical cord area (UCCA) measured on routine brain MRI can serve as a biomarker of conversion to SPMS. METHODS: This is a single-center retrospective cohort study of RRMS patients with cross-sectional and longitudinal analyses of clinical and MRI data. Future SPMS converters were matched by age, sex, and disease duration with non-converters. UCCA at C1-C3 was measured at a pre-conversion visit in converters and at the first available visit in non-converters. Analysis was performed using Pearson's correlation and multivariable logistic regression.

Exploring attitudes and acceptance of artificial intelligence in multiple sclerosis from the patient perspective.

Artificial intelligence (AI) is increasingly being integrated into healthcare, particularly in data-intensive chronic diseases that rely on longitudinal monitoring and shared decision-making. Multiple sclerosis is a prototypical example of such care, but real-world benefit will depend on whether people accept AI support in different clinical roles. We conducted a cross-sectional, web-based survey among 241 people with MS (pwMS) to assess comfort with AI across eight clinical domains and to identify predictors of acceptance. We derived an artificial-intelligence attitudes composite with high in…

Assessing the relation between fatigue and depression in multiples sclerosis: the impact of measurement choice.

BACKGROUND: Fatigue and depression are among the most prevalent and disabling symptoms in multiple sclerosis (MS) and are frequently reported to be strongly associated. This association is often interpreted as reflecting shared pathophysiology. However, widely used depression scales such as the Beck Depression Inventory-II (BDI-II) include somatic items that overlap with core features of MS and MS-related fatigue, potentially inflating observed relationships. OBJECTIVES: To examine whether the association between depression and fatigue in MS depends on the choice of depression measure, we com…
