Minimally invasive surgical techniques have evolved to reduce soft-tissue injury associated with open surgical techniques. The use of endoscopic visualization allows the exposure of deep structures and provides a mechanism to perform all the components of an open surgical procedure through small portals, thus satisfying a basic requirement of minimally invasive surgical procedures. Surgeons in the field of skull-base and spine surgery are now taking advantage of the benefits of such endoscopes. The pneumatically powered EndoArm endoscopic holder has been used extensively in both cranial and sp

An adenocarcinoma of the distal esophagus may also be designated as Barrett's carcinoma as it evolves from Barrett's esophagus. Barrett's esophagus currently is defined as a columnar metaplasia of the distal esophagus, as identified by endoscopy, that, upon histopathology, is confirmed to contain intestinal metaplasia. A different histological entity of columnar metaplasia of the distal esophagus is cardia-type mucosa which probably precedes intestinal metaplasia, but lacks goblet cells typical for the latter. The conversion rate from Barrett's esophagus to Barrett's carcinoma amounts to 0.5 t

Apocrine adenocarcinoma is a rare malignancy with invasive potential. Its clinical behavior and histologic appearance are highly variable, making accurate diagnosis difficult. We report on a case of apocrine carcinoma presenting with lymph node infiltration and extensive extramammary Paget's disease. Wide local excision with lymph node dissection is the mainstay of treatment. The role of adjuvant therapy has yet to be established.

The purpose of this study was to analyse multi-detector row CT (MDCT) signs of peripancreatic arterial and venous invasion in pancreatic carcinoma. Among 101 patients with pancreatic carcinoma examined by MDCT, 54 candidates for surgery were pre-operatively evaluated for vascular invasion based on MDCT signs. The peripancreatic major vessels (including superior mesenteric artery, coeliac artery, common hepatic artery, superior mesenteric vein and portal vein) were examined carefully by surgeons during the operation. At surgical exploration, 78 of 224 vessels were invaded by tumour. The invaded

Extracranial spread of meningiomas to involve the middle ear is very rare. We present the case of a 43-year-old woman with a known cerebellopontine angle meningioma who subsequently presented with left-sided otalgia and a middle-ear mass extruding through the tympanic membrane due to local invasion of the meningioma. The tumour was excised surgically. A discussion of the relevant literature is also presented.

We explored the immunohistochemical expression of fatty acid synthase (FAS) in Paget's disease of the vulva (PDV) and its association with clinico-pathological features. FAS is a recently discovered molecule involved in energy supply of normal cells. FAS is also overexpressed in neoplastic tissues because of their increased necessity of energy. Specimens from 20 patients with PDV were immunohistochemically evaluated; increased FAS expression was observed in 7 of 8 patients with invasive PDV (87%), in 3 of 4 patients with microinvasive PDV (75%), and in 1 of 8 patients with noninvasive PDV (12%

Surgical resection provides potential cure for patients with hepatocellular carcinoma. Unfortunately, resection is suitable in only about 10-37% of patients because of the limited hepatic functional reserve from the underlying chronic liver disease in the majority of patients. Survival of patients with unresectable diseases, especially those with portal vein tumor invasion, remains very poor. Radiofrequency ablation (RFA) is a form of locoregional therapy that allows a selected group of previously inoperable patients to be treated. However, problems with RFA leading to induced portal vein thro

AIM: The aim of this study was to report the incidence of occult cancer in reduction mammaplasty (RM) specimens.
METHODS: A retrospective study of 314 women who underwent RM from February 1996 to August 2001.
RESULTS: Occult carcinomas were detected in three patients, in two of which the lesions were invasive. Atypical ductal or lobular hyperplasia was found in five cases and other carcinoma risk lesions in four patients. Other benign changes were identified in totally 133 patients.
CONCLUSION: Incidental carcinomas will occasionally be found in RM procedures.

The predisposition of patients with lobular carcinoma in situ (LCIS) to develop invasive breast cancer (IBC) is well known. However, relatively little is known about the biologic characteristics, which may be involved in the development and progression of LCIS. This study evaluated 59 cases of LCIS (29 pure, 30 with synchronous IBC) for five biomarkers known to be important in IBC (ER, PgR, c-erbB-2, p53 and Ki-67 proliferation rate) by immunohistochemistry. A comprehensive analysis of loss-of-heterozygosity (LOH) was performed in 12 cases (10 pure, 2 with synchronous IBC) at 15 genetic loci o

Malignant meningioma is a relatively rare subtype of meningioma demonstrating an aggressive growth pattern, distinct histologic features, and a propensity for recurrence following surgical resection. There has been little consensus regarding imaging features of malignant meningioma, and typically the degree of tumor aggressiveness is determined by pathology. Here we describe a case of malignant meningioma determined to be benign at initial biopsy. Computed tomography (CT) and magnetic resonance (MR) imaging are reviewed with discussion of the aggressive features seen at presentation.

One of the most significant developments in recent decades in the management of malignant diseases is the recognition that surgery, radiation, and chemotherapy have limited curative potential. Along with this recognition has come the realization that although some patients can be cured with one of these modalities alone, the judicious combination of these modalities can result in better outcome. Carcinoma of the vulva, a disease that presents challenges with its management because of its anatomic location, affinity for spread to the lymph nodes, and incidence in elderly patients, lends itself

The incidence of gynaecologic cancers in women from Kingston and St Andrew for the period 1973-1997 were reviewed by analyzing data previously published by the Jamaica Cancer Registry. Gynaecologic cancer-related mortality statistics for the entire island for 1999 were compiled from data obtained from the Registrar General's Department (RGD) and the Statistical Institute of Jamaica (STATIN). Data were compared to gynaecologic cancer statistics for women from the United States of America for 1973-1997. A total of 2862 gynaecologic cancers were registered for the years 1973-1997, which represent

OBJECTIVE: Appropriate clinical management of cases of FIGO Grade I and II endometrial carcinoma relies heavily on the determination of myometrial invasion (MI). There are no reports addressing expression of the cell adhesion molecule CD44 in the subset of Grade I and II endometrioid carcinoma (EC) as it relates to prognosis, including MI.
METHODS: Immunohistochemical staining for CD44s and CD44v6 was evaluated in 40 hysterectomy specimens with Grade I and II EC, including 11 noninvasive ECs, 14 with MI <50% of myometrial thickness, and 15 with deep invasion (MI >50%). Staining characteristic

PURPOSE: To assess the MR imaging (MRI) in initial decision making of endometrial carcinoma.
PATIENTS AND METHODS: From january 1997 to march 2002, 86 patients with clinical stage I endometrial carcinoma were studied by pelvic (22) or abdomino-pelvic (64) MRI. The size and signal of lesions and junctional zone, the enhancement after contrast administration, the associated benign lesions and the pelvic and lumbo-aortic lymph nodes were assessed without knowledge of surgical results in viewing to identify pitfalls in MRI staging.
RESULTS: MRI allows correct staging in 81.4% (70/86) of cases. N

We report the MR features in a previously healthy patient with mucosa-associated lymphoid tissue lymphoma of the pituitary gland. The MR images showed a homogeneously enhancing solid mass in the sellar region and both parasellar regions; this mass extended through the bilateral foramina ovale along the third branches of the trigeminal nerves. The mass completely encircled the cavernous segments of both internal carotid arteries without narrowing the lumen. Gadopentetate dimeglumine-enhanced T1-weighted MR images showed homogeneous, strong enhancement of the mass.

Von Hippel-Lindau disease (VHLD) is an autosomal dominant disorder predisposing to the development of different histological types of tumours with predominance of CNS haemangioblastomas in the majority of cases. We present a case of a 35-year-old man with obstructive hydrocephalus due to multiple posterior fossa tumours. Familial history revealed two relatives operated on previously because of a posterior fossa tumour (in one case haemangioblastoma was diagnosed). During surgery two cerebellar tumours were removed radically via the suboccipital approach. On a histopathological examination the
