Correlation of segmental carpal tunnel pressures with changes in hand and wrist positions in patients with carpal tunnel syndrome and controls.

We investigated pressures at 1 cm intervals along the carpal tunnel in 39 patients with carpal tunnel syndrome (CTS) and 12 controls. Pressures were measured for relaxed and gripping hand positions in combination with neutral, extended, and flexed wrist positions. Patient pressures exceeded control pressures, were below the previously reported 30 mmHg threshold for four of five locations in the relaxed neutral position and were typically greater in extension than in flexion. In the neutral position, both patient and control pressures were slightly above threshold levels just distal to the tunn

Cervical myeloradiculopathy with entrapment neuropathy: a study based on the double-crush concept.

In an attempt to formulate a standardised approach to the diagnosis and management of patients with the double-crush syndrome, we reviewed 65 surgical cases (39 men and 26 women) with cervical myeloradiculopathy associated with entrapment neuropathy in the arm by examining the clinical features, results of nerve conduction velocity studies and distribution of the vertebral levels of compression. Fifty-three patients (average age, 43 years) presented with a carpal tunnel syndrome at the wrist and cervical lesions (41 with spondylosis and 12 with ossified posterior longitudinal ligament), while

Reproducibility of normal facial motor nerve conduction studies and their relevance in the electrophysiological assessment of peripheral facial paralysis.

To determine the intra-examiner intertrial reproducibility of normal facial motor nerve conduction studies (FNCS) and their relevance in electrophysiological assessments of peripheral facial paralysis, 52 patients with acute unilateral Bell's palsy were examined on two separate occasions 1 months apart. Three electroneurographic methods were assessed. On the unaffected side of the face, FNCS are reliable when performed by a single examiner over time. Nevertheless, compound muscle action potential (CMAP) baseline-to-peak and peak-to-peak amplitude showed a rather high intertrial variability. Re

An epidemic of optic neuropathy in Tanzania: characterization of the visual disorder and associated peripheral neuropathy.

An epidemic of subacute bilateral visual failure has affected large numbers of teenagers and young adult Africans in coastal Tanzania since 1988. Previous work had indicated that many patients had sensory symptoms, but the nature of the neurological involvement was uncertain. The primary objective of this study was to characterize the accompanying neurological disorder. Furthermore, the nature of the visual loss was uncertain from previous reports as both retinopathy and optic neuropathy had been suggested. Full ophthalmic and neurological examinations were carried out at the Muhimbili Univers

Neurological multisystem manifestation in multiple symmetric lipomatosis: a clinical and electrophysiological study.

Multiple symmetric lipomatosis (MSL) is characterized by a typical neck and shoulder distribution of subcutaneous lipomata and is often associated with polyneuropathy. Occasionally, the central nervous system (CNS) can be involved. Twelve of 14 patients in this retrospective study had clinical or electrophysiological evidence of a predominantly axonal polyneuropathy. Among those were 10 with alcohol abuse, but 2 patients without alcohol abuse also showed clinical or electrophysiological polyneuropathy. Clinical CNS involvement was present in 4 patients. CNS dysfunction was documented by evoked

Effect of treatment with an aldose-reductase inhibitor on symptomatic carpal tunnel syndrome in type 2 diabetes.

The aim of this study was to test the efficacy of the aldose-reductase inhibitor Tolrestat in the treatment of carpal tunnel syndrome in Type 2 diabetic patients. Seventeen patients were treated with Tolrestat (200 mg daily for 12 months) clinical and neurophysiological evaluations were performed at baseline, 6 and 12 months; symptoms and blood glucose control were assessed at baseline, 2, 6, and 12 months. Thirteen Type 2 diabetic patients suffering from symptomatic carpal tunnel syndrome served as controls. Neurophysiological studies showed improvement in the sensory conduction velocity of t

The use of patterned neuromuscular stimulation to improve hand function following surgery for ulnar neuropathy.

A 60-year-old man with wasting and weakness of the right hand following ulnar nerve entrapment at the elbow was referred for electrotherapy. An ulnar nerve transposition had been performed 2 years previously. This had produced some improvement in nerve conduction without significantly improving hand muscle function. The right first dorsal interosseous muscle (FDI) was stimulated for 4 hours per day over a 6-week period with a stimulus pattern replicating the discharge of a single motor unit from a healthy, fatigued FDI (patterned neuromuscular stimulation or PNMS). The response was assessed us

Lower limb arterio-venous shunts, autonomic neuropathy and diabetic foot.

We have quantitatively assessed the percentage of lower limb arterio-venous (a-v) shunting using a radioisotopic technique and correlated it with autonomic neuropathy evaluated by cardiovascular tests. We have studied three groups of diabetic patients: Group A, 12 non-neuropathic subjects without foot lesions; Group B, 12 neuropathic subjects without foot lesions; Group C, 12 neuropathic subjects with recurrent foot ulcers. Shunting was higher in Group C (10.4 +/- 2.7%) than in Group B (6.8 +/- 2.3%, P less than 0.01) and Group A (3.8 +/- 1.2%, P less than 0.001). Shunts in Group B were higher

Saccadic latency as a measure of afferent visual conduction.

Latency to initiate a saccadic eye movement to a visual target, and visual evoked potential, were measured in seven patients with resolved unilateral optic neuritis. Saccades were delayed when the target was presented to the clinically involved eye, but were normal when the contralateral eye was tested. With binocular target presentation, saccades were symmetric between eyes and normal in latency. In two patients with pituitary adenoma and low-grade bitemporal field defects, saccades were delayed when targets were presented in the temporal field, but were within normal limits when presented in

Hereditary motor sensory neuropathy type II with neurofilament accumulation: new finding or new disorder?

Peroneal muscular atrophy is now known to be heterogeneous and to be due to various underlying genetic mechanisms. Exploring this heterogeneity further, we report on a German kinship with the clinical, genetic, and nerve conduction features of hereditary motor and sensory neuropathy type II (HMSN type II) but whose sural nerves on biopsy were found to show infrequent axonal swellings with neurofilament accumulations not previously described. The dominant inheritance and absence of kinky hair set this disorder apart from giant axonal neuropathy. There was no history of toxic exposure to industr

Evoked potentials in the investigation of traumatic lesions of the peripheral nerve and the brachial plexus.

Nerve conduction studies using nerve action potential (NAP), sensory nerve action potential (SNAP), evoked muscle action potential (M-response), retrograde conduction in the motor axon (F-response), and cortical and subcortical somatosensory evoked potential (SEP) are useful tools for evaluation of the peripheral nervous system. SEP recording has the advantages of being applicable to severely damaged nerves because of its amplification mechanism and of disclosing proximal root lesions that would not be disclosed by other methods. When SEP recording is used in an operating theater, the amplific

[Reinnervation in obstetrical brachial plexus paralysis].

Post-paralytic associated movements ("co-contractions") in 16 cases with obstetrical brachial plexus palsy (OPP) were studied by a neurographic method. This consisted of stimulating a nerve and searching for heterogeneous motor axon reflexes (HAR) in the muscles of the upper limb and shoulder. HAR can be elicited when branches of a single axon reach two different muscles, even very distant from each other. The anatomy of the plexus and the fact that regenerating axons maintain direction determine the frequency with which two muscles are connected. Most HAR are recorded in muscles innervated by

[Neuromuscular disturbances in endocrine disorders].

In order to study the character of lesions in the neuro-muscular system in endocrinological diseases the authors examined 156 patients with diffuse toxic struma, hypothyroidism and diabetus with the aid of clinico-EMG methods. A comparison of the investigation results of clinical, bioptic material and EMG data testified to an involvement into the process of all regions of the segmental neuromuscular apparatus. The results of EMG studies may serve as control data in relation to the effectiveness of the used therapy.

The spinal evoked response in infants and children.

Summated responses to peroneal nerve stimulation were recorded from surface electrodes placed over the spine of 60 infants and children. These potentials generally were greater in amplitude in infants than in older children. Over the cauda equina and rostral cord, initially positive triphasic potentials were recorded. Over the caudal cord, complex potentials were recorded in children less than three years of age. The conduction velocity of the response from midlumbar to lower cervical recording sites was less in infants than in older children and progressively increased with age, reaching adul

The response of laryngeal afferent fibres to mechanical and chemical stimuli.

1. We have recorded afferent activity from ;single fibres' dissected from the superior laryngeal nerve of anaesthetized cats.2. Units which responded to gentle mechanical stimulation of the larynx epithelium were chosen for study.3. Receptors with myelinated fibres were grouped according to their spontaneous activity. Group 1 fibres had little or no spontaneous activity: group 2 fibres had constant and continuous spontaneous activity.4. Group 1 fibres had a wide range of adaptation rates. Their conduction velocities lay between 3.0 and 30 m/sec. The receptors were generally stimulated by ammon

Electrophysiological findings in entrapment of the median nerve at wrist and elbow.

In 117 consecutive patients with carpal tunnel syndrome and 11 patients with a compression syndrome of the median nerve at elbow, motor and sensory conduction along the median and ulnar nerves and quantitative electromyography were compared with findings in 190 normal controls of the same age. In 25% of patients with carpal tunnel syndrome in whom motor conduction and EMG were normal, the lesion was located from abnormalities in sensory conduction. The fact that conduction along the same fibres was moderately slowed from digit to palm, severely slowed across the flexor retinaculum, and normal

Improving Data Quality, Model Transparency and Performance in Lung Histopathology with Explainable AI.

Convolutional neural networks (CNNs) have shown strong capabilities for image analysis. However, deploying these models in medical settings is complicated by their limited transparency. Over recent years, many approaches have been developed to overcome the so-called "black box" problem of deep neural networks. Here, we show how such explainable AI (XAI) approaches can be applied to not only improve transparency but also training data quality and model performance, with classification of lung damage in histopathology images as use case. First, we conducted a thorough exploratory data analysis a
