Peak alpha frequency and spectral exponent in postoperative delirium: a high-density EEG cohort study.

BACKGROUND: Present concepts around the neurophysiology of delirium suggest impairments in excitatory/inhibitory balance, leading to EEG slowing, altered synaptic activity and integration of information. We aim to clarify the mechanisms involved, hypothesising that, in postoperative delirium, the EEG peak alpha frequency (PAF; decreased peak frequency indicating slowing in alpha frequency) would slow and the spectral exponent (SE; a higher slope indicating an increased neural inhibitory tone) would be steeper compared with a baseline collected before surgery. The primary aim was to determine t…

The opioid flexion phenotype: postural flexion and nodding in the fentanyl era.

The fentanyl era has produced recurrent community observations of people remaining upright or semi-upright in sustained forward flexion of the head and trunk with intermittent nodding, colloquially referred to as the "fentanyl nod" or "fentanyl fold." Yet the motor state producing this appearance has never been measured directly. We conducted a structured narrative review spanning axial postural-control and reticulospinal neurophysiology, mu-opioid pharmacology, fentanyl-associated rigidity, alpha-2 adulterants, and observations from supervised consumption settings, and integrated these litera…

Focal hypermetabolism in suspected non-convulsive status epilepticus.

OBJECTIVE: To evaluate ¹⁸F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET) as a biomarker in suspected non-convulsive status epilepticus (NCSE) by characterizing electroclinical-metabolic correlations and quantifying focal hypermetabolism. METHODS: We included adult patients with rhythmic and periodic patterns (RPPs) undergoing 18F-FDG PET for suspected NCSE. Two neurologists reviewed patient electroencephalography (EEG) recordings (0-30 min post-injection) using 2021 American Clinical Neurophysiology Society (ACNS) criteria. 18F-FDG PET images were blindly assessed by two nuc…

Biological Effects of Music Therapy in End-of-Life Care: A Narrative Review.

Background and Objectives: Music therapy has a long tradition in palliative care, and recent studies have investigated its Neuro-Psycho-Endo … f music therapy in end-of-life care and their practical implications for nursing care. Results: The neurophysiology of music perception in terminal patients is complex, involving a wide array of clinical and cultural factors. Key musical elements-such as rhythm, melody, harmony, tempo, and mode-can influence physiological and psycho-emotional responses. Music the …

α-linolenic acid-induced facilitation of GABAergic synaptic transmission is mediated via acid-sensing ion channel (ASIC1a) activity in the basolateral amygdala.

ABSTRACT: To enhance the reliability and stability of information acquisition, biological perception systems have gradually evolved from uni … rning approaches in both performance and mechanistic transparency. GRAPHICAL ABSTRACT: Inspired by neurophysiology, we construct a sequence memory neurodynamic model that integrates visual and auditory processing, and validate its reliability through both micro- and macro-level experiments. We also explore novel functionalities of the model, such as bidirectiona …

Altered auditory brainstem responses are post-acute sequela of SARS-CoV-2 (PASC).

The Post-acute Sequela of SARS-CoV-2 (PASC) syndrome, also known as Long-COVID, often presents with subjective symptoms such as brain fog an … hat PASC may alter the central auditory pathway and lead to slower conduction and elevated auditory neurophysiology responses at the midbrain, a pattern associated with the typical aging process. This study marks a significant stride toward establishing an objective measure of subjective cognitive fatigue through assessment of the central auditory system.

Amyotrophic lateral sclerosis in Saudi Arabia: a multicenter descriptive study.

INTRODUCTION: Amyotrophic lateral sclerosis (ALS) is a rare neurodegenerative disease characterized by the progressive loss of muscle contro … ry Saudi centers (2003-2022). The authors identified cases from neurology/neuromuscular clinics and neurophysiology laboratories; diagnoses followed revised El Escorial criteria with EMG confirmation where indicated. ALS variants and cases lacking sufficient longitudinal evidence were excluded. Clinical genetic testing was performed at the clinician's discretion; …

Uncovering the neural correlates of the urge-to-blink: A study utilising subjective urge ratings and paradigm free mapping.

Neuroimaging plays a significant role in understanding the neurophysiology of Tourette syndrome (TS), in particular the main symptom, tics, and the urges associated with them. Premonitory urge is thought to be a negative reinforcer of tic expression in TS. Tic expression during neuroimaging is most often required as an overt marker of increased urge-to-tic, which can lead to considerable head movement, and thus data loss. This study aims to identify the brain regions involved in urge in healthy subjects using multi-echo functional magnetic resonance imaging (fMRI) and a timing-free approach to…

A matter of personality and point of view: How the interplay of reinforcement sensitivity and general attitudes towards pain impacts the responsivity to acute pain.

BACKGROUND: The relatively stable individual differences reflected in Grey's revised reinforcement sensitivity theory (rRST), with foundations in neurophysiology and learning theory, appear particularly applicable to the study of pain. However, remarkably little research has been conducted in this area. In acute pain, activation of the behavioural approach system (BAS), the behavioural inhibition system (BIS) and the fight, flight, freezing system might depend on an individual's evaluation of pain. It was thus hypothesised that higher-order interactions of rRST traits and pain attitudes affect…

OBJECTIVE: The Salzburg criteria for nonconvulsive status epilepticus (NCSE) and the American Clinical Neurophysiology Society (ACNS) Standardized Critical Care EEG Terminology 2021 include a diagnostic trial with intravenous (IV) antiseizure medications (ASMs) to assess electroencephalographic (EEG) and clinical response as a diagnostic criterion for definite NCSE and possible NCSE. However, how to perform this diagnostic test and assessing the EEG and clinical responses have not been operationally defined. Vibro-Tactile Stimulation as a Non-Invasive Neuromodulation Therapy for Cervical Dystonia: A Case Study.

INTRODUCTION: Cervical dystonia (CD) is a type of focal dystonia that is characterized by involuntary neck postures. The underlying neurophysiology mechanism of CD is unknown, but there is increasing empirical evidence that motor deficits of CD are associated with somatosensory and proprioceptive deficits in the upper limb area. Vibro-tactile stimulation (VTS) is a non-invasive somatosensory stimulation approach where afferent signals from the vibrated muscle and tactile mechanoreceptors modulate cortical activity. Previous studies have shown that VTS could be an effective neuromodulation ther…
