Sagittal- or coronal-plane deformity considerably complicates the diagnosis and treatment of lumbar spinal stenosis. Although decompressive laminectomy remains the standard operative treatment for uncomplicated lumbar spinal stenosis, the management of stenosis with concurrent deformity may require osteotomy, laminectomy, and spinal fusion with or without instrumentation. Patients with cranial base tumors often present with cranial neuropathies or develop cranial deficits as a consequence of skull base surgery. Deficits involving cranial nerves IX, X, XI, and XII can lead to significant morbidity for patients. A standard evaluation of a patient with a suspected skull base lesion or a postsurgical patient would consist of a complete history and physical examination, including a bedside examination and videofluoroscopy. Other modalities, including fiberoptic endoscopic examination of swallowing safety, ultrasound, and manofluorography, are used less frequently.

A program was designed to instruct medical students on how to obtain a history pertinent to breast abnormalities and to perform a breast examination. The use of clinical teaching associates provides direct student instruction and critiques during the examination. In addition, students are taught how to instruct patients in the breast self-examination and to properly document breast abnormalities. Such a program should be considered for incorporation in all physical diagnosis courses as well as in residency training programs and seminars for practicing physicians.

OBJECTIVE: To determine the prevalence and clinical impact of murmurs detected during routine physical examination in neonates.
METHODS: In a 4 years retrospective study, 6333 healthy newborn babies were screened for the presence of a heart murmur during routine neonatal physical examination. Prematures or those who were admitted to neonatal intensive care unit or any neonate with a risk factor that is known to be associated with increased incidence of congenital heart disease were excluded from the study.
