A prospective clinical study of allergy progression in identical and fraternal twin pairs of children.

BACKGROUND: Understanding the factors leading to the development of allergic disease is a critical area of research. We studied the development of allergic disease in identical and fraternal twins to identify potential differences in environment versus genetic factors. METHODS: Twins aged up to 4 years were selected for inclusion in this long-term follow-up study. Regular questionnaire results, allergen levels, and other indicators were examined. RESULTS: A total of 80 twins were included in this study. Over time, the incidence of atopic dermatitis (AD) decreased, and the incidence of rhinit

Timing of Newborn Hearing Screening Effects on Passing Rates: A Prospective Cohort Study.

Newborn hearing screening using transient evoked otoacoustic emissions (TEOAEs) is essential for early detection of hearing impairment. The Joint Committee on Infant Hearing recommends screening near hospital discharge but does not specify an optimal timing. To determine the optimal timing for TEOAE screening in neonates at low risk of hearing impairment and to identify perinatal factors influencing pass rates.Neonates underwent sequential TEOAE screening based on postnatal age at the time of testing: less than 24, 24 to 36, 36 to 48, and more than 48 hours, with follow-up at 1 month for persi

Skilled Nursing Facility Population Characteristics and Length of Stay: A Prospective Cohort Study.

OBJECTIVES: Skilled nursing facilities (SNFs) offer short-term, subacute care following injury or hospitalization, with Medicare fully covering stays ≤20 days. As the US population ages, SNF demand is increasing, yet this population remains poorly characterized. Prolonged stays increase pathogen exposure, with shared spaces and visitors potentially elevating viral transmission. We estimated the prevalence of self-reported respiratory symptoms, shared space use, and visitor frequency among SNF residents and their associations with length of stay (LOS). DESIGN: Prospective cohort study. SETTIN

Post-Kidney Transplant Hospitalizations and Long-Term Outcomes: A Prospective Multicenter Study.

KEY POINTS: Nonimmunologic AKI-related rehospitalizations post–kidney transplant are associated with poorer long-term allograft and recipient outcomes. These data are not reported to United Network for Organ Sharing, and there are no datasets highlighting this important association; thus, our findings are valuable to clinicians. We hope to help the community create a cohort of higher risk transplant patients who need closer monitoring to potentially increase graft longevity. BACKGROUND: AKI is associated with an increased risk of CKD and ESKD. However, only limited studies have investigated t

Development and validation of a composite digital balance score for spinocerebellar ataxia: a prospective study.

BACKGROUND: Clinical trials in spinocerebellar ataxia are currently limited by the large sample sizes required by available clinical endpoints. We aimed to devise a digital composite measure of standing and walking balance using wearable inertial sensors that would require smaller sample sizes. The new score is called the Score of Integrated Balance in Ataxia (SIBA). METHODS: In this study, we developed the SIBA based on a retrospective sample of adults (aged 18-75 years) with spinocerebellar ataxia types 1, 2, 3, or 6, recruited during clinical visits at five sites (four in the USA and one i

An Optimal Initial Ventilation Mode in Congenital Diaphragmatic Hernia: A Prospective Cohort Study.

OBJECTIVES: Respiratory management is crucial for patients with congenital diaphragmatic hernia (CDH). This prospective study aimed to assess the optimal initial ventilatory mode for CDH by comparing conventional mechanical ventilation (CMV) and high-frequency oscillatory ventilation (HFO). METHODS: Fifteen participating institutes used CMV or HFO as the initial ventilation mode for CDH, we thus divided them into two groups (CMV and HFO). Clinical data were prospectively collected from patients with isolated left CDH born at a gestational age ≥ 34 weeks between 2017 and 2021. The primary outc
