PURPOSE: Metastatic castration-resistant prostate cancer (mCRPC) shows marked clinical heterogeneity and poor long-term survival, highlighting the need for simple and reliable risk stratification tools beyond the limited usability of existing complex prognostic models. In this study, we developed and validated PROGRESS (Prostate Cancer Global Risk Evaluation and Stratification Score), a simplified prognostic score derived through machine learning‑guided feature selection distilled into a simple laboratory-based tool.
METHODS: Baseline data from 2,035 patients with mCRPC (training set) were em

INTRODUCTION: 177Lu-PSMA-617 (LuPSMA) is a radiopharmaceutical approved for the treatment of metastatic castration resistant prostate cancer (mCRPC). Consensus guidelines recommend multimodal disease monitoring during LuPSMA therapy, but there are no definitive recommendations regarding frequency and modality of imaging. Here, we sought to describe patterns in imaging utilization during LuPSMA therapy.
PATIENTS AND METHODS: mCRPC patients treated with ≥ 2 cycles of LuPSMA between June 2022 and April 2024 were identified. Demographic, clinical, and imaging data were extracted from the medical

BACKGROUND: Optimal follow-up after a negative MRI-targeted biopsy in men with PI-RADS 3-5 lesions remains uncertain. Repeat biopsy may detect missed cancers but exposes many patients to unnecessary risk. We aimed to characterize real-world outcomes and develop a risk-adapted framework.
METHODS: We conducted a retrospective cohort study of 86 men with PI-RADS 3-5 lesions and benign index MRI-targeted biopsy. The primary outcome was detection of prostate adenocarcinoma on repeat biopsy or during surgery for benign prostatic obstruction (BPO). Clinically significant prostate cancer (csPCa) was

To evaluate biochemical recurrence-free survival (BRFS), metastasis-free survival (MFS), cancer-specific survival (CSS), and the prognostic value of an undetectable postoperative prostate-specific antigen level (PSA < 0.1 ng/mL) in patients with lymph node-positive (pN1) prostate cancer after robot-assisted radical prostatectomy (RARP), for whom optimal management remains undefined. We retrospectively reviewed 1,689 patients who underwent RARP with standard or extended pelvic lymph node dissection (PLND) between March 2008 and March 2023 at a single institution; 37 were excluded for missing da…

Multi-parametric magnetic resonance imaging (mpMRI) is a valuable medical technology for detecting clinically significant prostate cancer (csPCa). The diagnostic accuracy of mpMRI for csPCa in negative mpMRI (PI-RADS 1-2) remains suboptimal, underscoring the need for improvements for csPCa. This study aimed to build a visual predictive nomogram for early detection of csPCa in negative mpMRI. We retrospectively reviewed 303 men from our institution who simultaneously underwent 68Ga-PSMA-11 PET/CT and mpMRI before a biopsy between March 2020 and July 2022 and 130 men from the outside institution

Robotic-assisted radical prostatectomy (RARP) has become the gold standard treatment for localized prostate cancer. However, predicting post-operative outcomes remains challenging. This study aims to develop and validate predictive models for key outcomes using machine learning approaches and compare them with traditional risk stratification systems. We conducted a retrospective analysis of 758 consecutive patients who underwent RARP between 2014 and 2018. Pre-operative variables included PSA, Gleason score, clinical stage, and IPSS scores. Primary outcomes were biochemical recurrence (BCR), p

INTRODUCTION: Androgen receptor pathway inhibitor (ARPI) plus androgen deprivation therapy (ADT) is a guideline-endorsed standard for metastatic castration-sensitive prostate cancer (mCSPC). Robust comparative data on individual ARPIs and their subsequent management after castration resistance remain limited.
METHODS: This multicenter retrospective study included consecutive patients who received first-line ARPI for mCSPC between January 2018 and December 2023. Inverse probability of treatment weighting (IPTW) balanced the baseline clinicopathological variables across the abiraterone (n = 136

BACKGROUND: Lutetium-177 (177Lu)-PSMA-617, a targeted radioligand therapy, has demonstrated significant survival benefits in patients with metastatic castration-resistant prostate cancer (mCRPC). Its application in patients with a superscan pattern, indicative of extensive skeletal metastases, is less studied due to concerns about hematologic toxicity from bone marrow involvement.
METHODS: This study analyzed 133 mCRPC patients treated with 177Lu-PSMA-617, divided into Superscan and Non-superscan groups. PSA response (≥ 50% decline), PSA progression free survival (PFS), overall survival (OS),

PURPOSE: The impact of persistent hypogonadism post-androgen deprivation therapy (ADT) on overall survival (OS) of patients with prostate cancer (PCa) is poorly documented. We compared OS between patients who recovered testosterone and those who did not after ADT.
METHODS AND MATERIALS: Patients with PCa with intermediate- or high-risk disease were treated with ADT for 6, 18, or 36 months plus radiation therapy in 2 randomized trials. We compared OS between patients who recovered testosterone to a normal level to those who did not, using the log rank test. Multivariable Cox analysis to predic

BACKGROUND: There is a complex relationship between body weight and survival in prostate cancer. While increased BMI is associated with increased prostate cancer incidence and death, obesity is associated with improved survival in metastatic castrate-resistant prostate cancer (mCRPC). However, little is known about the effect of weight change before the treatment of mCRPC on survival. We assessed the association between BMI, weight loss before treatment, PSA levels, and overall survival in mCRPC.
METHODS: Veterans treated with abiraterone or enzalutamide for de novo mCRPC from May 2011 to Jun

INTRODUCTION: The PROSTest is a novel machine learning-based liquid biopsy assay that functions as a diagnostic and prognostic tool in prostate cancer (PCa). The algorithm outcome (scored 0-100) has a cutoff of >50 to indicate PCa. In this study, we evaluated the screening utility of the test in comparison with the commonly used PSA test.
MATERIALS AND METHODS: 509 men (median age: 62 [40-88]), predominantly Afro-Caribbean (n=449, 88.2 %) were enrolled in this study at a referral centre for prostatic cancer screening and management. These included 417 symptomatic patients with urinary tract (

OBJECTIVES: The geriatric Nutritional Risk Index (GNRI) is an effective tool to assess the nutritional status of the elderly. However, the relationship between the GNRI and the risk for prostate cancer (PCa) remains uncertain in middle-aged and older men. The aim of this study was to investigate the association between the GNRI and the risk for PCa by analyzing the serum total (tPSA) and free prostate-specific antigen (fPSA) levels (including percent fPSA [%fPSA]).
METHODS: Data for this study were obtained from 7396 men ≥40 y of age from the 2001-2010 National Health and Nutrition Survey (NH…
