The purpose of this paper is to introduce a surgical approach to treatment of severe forefoot deformities of rheumatoid arthritis. Briefly, the surgery consists of base of the first metatarsal osteotomy to correct metatarsus primus varus, and metatarsal head resection beginning with the fifth metatarsal and carried around in crescentic fashion through the necks of the other metatarsals, so that as an end result the great toe is the longest, the second next to the longest, etc. The short extensor tendons are dissected to their insertions and, since they have drifted laterally into the "valleys"

Augmentation genioplasty can serve as a valuable adjunct to orthodontic treatment. The borderline extraction patient with a good nasolabial angle, protruding lower incisors and a deficient chin often can be treated better by nonextraction orthodontics followed by advancement of the chin than by any regimen involving orthodontic extraction. An attempt is made to depolarize current positions regarding intraocular implant lens surgery by presenting a reasonable list of advantages and disadvantages of the procedure. A current position on this controversial subject is presented and is based on currently available knowledge. Although this is a personal view, it is hoped that a middle ground can be created which would be acceptable to advocates and opponents of lens implant surgery.

A birth of interest in lens implantation occurred especially in the United states in 1974. This coincides with the beginning of four major American lens manufacturers. This interest expoded in 1975 and 1976 and seems to be continuing to grow. Only a few surgeons have stopped implants, most of them after having done ten or less. A technique for biliary drainage in orthotopic liver transplantation has been developed in which the gall bladder is used as a pedicle graft between the donor and recipient common bile ducts. In 7 patients this technique has produced encouraging results. There has been no incidence of bile leakage or stenosis of the anastomoses. Eighty patients were operated upon with subcutaneous mastectomy and reconstruction with prosthesis. Seventy operations were performed on both breasts. The indications used were carcinoma in situ in a previous biopsy, pronounced epithelial proliferations on at least two previous occasions or recurring fibrocystic disease after at least two previous biopsies.
