Chronic Plaque Psoriasis as a Predictor of Spinal Osteoproliferation on Magnetic Resonance Imaging (MRI).

Chronic plaque psoriasis is a systemic inflammatory condition that extends beyond the skin and has been linked to musculoskeletal changes, including excessive bone formation along the spine. Emerging evidence indicates that individuals with psoriasis are more likely to demonstrate spinal bony overgrowth than those without the disease. Although biologically plausible mechanisms have been proposed to explain this relationship, there is currently no clear framework for translating the association into clinical decision-making. Existing practice recommendations do not specify which patients might …

The night matters: sleep quality and evening chronotype associated with clinical severity of psoriasis.

INTRODUCTION: Psoriasis is a chronic inflammatory skin disease associated with metabolic dysfunction and obesity. Poor sleep quality appears more prevalent in psoriasis, yet the role of chronotype remains largely unexplored. We evaluated whether patients with psoriasis more frequently exhibit poor sleep quality and evening chronotype compared with healthy controls and assessed the impact of these factors on psoriasis clinical severity. METHODS: In this cross-sectional study, 213 adults with psoriasis and 213 age- and sex-matched healthy controls were included. Anthropometric measures (body ma…

Biologic Efficacy by BMI in Moderate-To-Severe Psoriasis: A Real-World Cohort Study.

Obesity reduces response to biologic therapy in psoriasis, but comparative evidence across drug classes in Chinese patients is limited. A real-world cohort of 891 adults with moderate-to-severe psoriasis initiating biologics between 2020 and 2025 was divided by baseline BMI (< 24 vs. ≥ 24 kg/m2). PASI90 achievement at 2, 4, 6 and 12 months and drug survival were compared amongst IL-17, IL-23 and TNF-α inhibitors. Amongst IL-17 users, PASI90 rates in the non-overweight group were consistently higher at 4, 6 and 12 months (all p < 0.05), whereas no BMI-related differences were seen with TNF-α or…

Repurposing copper diethyldithiocarbamate (CuET) in a polymeric nanofibrous patch for topical therapy of psoriasis.

The management of psoriasis remains challenging due to the limitations of current therapies. This study explores the repurposing of copper diethyldithiocarbamate (CuET)-a metabolite of the anti-alcoholism drug disulfiram-as a novel topical agent for psoriasis. To this end, we developed an advanced delivery platform by fabricating a CuET-loaded nanofiber patch via electrospinning of a polyvinylpyrrolidone/polyethylene glycol/citric acid (PVP/PEG/CA) blend. This rationally designed matrix provided optimal mechanical strength, moisture-activated adhesion, and precise local drug delivery performan…

Psoriasis as a systemic inflammatory disease: an immune set-point framework for comorbidities and relapse.

Psoriasis is increasingly recognized as a systemic immune-mediated disorder that extends beyond cutaneous inflammation. Although skin lesions are its most visible manifestation, growing evidence indicates that psoriasis reflects a broader disruption of immune homeostasis accompanied by sustained systemic inflammatory activation. Persistent immune activation, altered myeloid programming, and dysregulated immunometabolic pathways together establish a state of chronic inflammatory priming. This systemic inflammatory state contributes to multi-organ comorbidities and also favors disease relapse th…

Comparative Efficacy of Interleukin Inhibitors and JAK Inhibitors in Moderate to Severe Plaque Psoriasis.

Plaque psoriasis is a chronic autoimmune skin condition characterized by erythematous, scaly plaques that commonly affect the scalp, trunk, and extensor surfaces. As the most prevalent form of psoriasis, it results from immune system dysregulation, particularly involving cytokines. Cytokines such as interleukin-17 (IL-17) and interleukin-23 (IL-23), as well as downstream Janus kinase (JAK)/tyrosine kinase 2 (TYK2) signal pathways, lead to accelerated skin cell turnover. While an expanding array of treatment options exists, including topical agents, conventional oral medications, and targeted b…

Prevalence of Steatotic Liver Disease with Fibrosis and Diagnostic Screening Tools in Patients with Psoriasis.

The prevalence of steatotic liver disease (SLD) with fibrosis in patients with psoriasis is unknown and no consensus on screening exists. Therefore, patients from The Copenhagen Translational Skin Immunology Biobank and Research Programme (BIOSKIN) were examined. SLD was defined as controlled attenuation parameter (CAP) > 250 dB/m and fibrosis as median liver stiffness measurement (LSM) ≥ 8 kPa using transient elastography. Biomarkers included fibrosis-4 index (FIB-4), N-terminal propeptide of collagen (type III), and enhanced liver fibrosis (ELF) test. A total of 403 patients (median age 51 y…

A Serum N-Glycan Ratio Associated With Disease Severity and Treatment Response in Psoriasis.

Psoriasis is a chronic, immune-mediated inflammatory disease with heterogeneous manifestations. Reliable biomarkers reflecting disease severity and treatment response remain limited. Glycans-carbohydrate structures attached to proteins and lipids-play key roles in biological processes, contributing to functional specificity. Abnormal glycosylation has been implicated in the pathogenesis of inflammatory and neoplastic diseases, highlighting their potential as therapeutic targets and biomarkers. This study aimed to evaluate serum N-glycan profiles as potential biomarkers for psoriasis, focusing …

[Guideline for the diagnosis and treatment of psoriasis arthropathica (psoriatic arthritis) in China (2026 edition)].

Psoriasis arthropathica, also known as psoriatic arthritis (PsA), is a highly heterogeneous systemic inflammatory disease. In addition to involving the skin with characteristic psoriatic lesions such as erythematous scaly plaques, it may also affect the joints, leading to joint pain, swelling, stiffness, and functional impairment, thereby substantially reducing patients' quality of life. To develop clinical practice guidelines that align with international standards for guideline development while reflecting Chinese clinical practice, the China Dermatologist Association, in collaboration with …

Arterial stiffness in patients with moderate to severe psoriasis on long term treatment with infliximab.

BACKGROUND: Systemic anti-inflammatory therapies reduce arterial stiffness during short term treatment in patients with psoriasis, but less is known about their long-term effects. We assessed arterial stiffness in patients with psoriasis on long-term treatment with infliximab. METHODS: Forty-eight patients with psoriasis (age 46 ± 14 years, 31% women) receiving treatment with infliximab were matched to 103 controls based on age, sex and body mass index (BMI). Arterial stiffness was assessed by carotid-femoral pulse wave velocity (cfPWV). Psoriasis severity was assessed by the psoriasis area a…

Identification of key miRNAs and target genes in psoriasis vulgaris and obesity co-morbidity.

BACKGROUND: Psoriasis is a chronic inflammatory dermatological disorder, with psoriasis vulgaris (PA) being the most prevalent form. Obesity is a comorbid condition and a risk factor for psoriasis, sharing common susceptibility genes and inflammatory pathways. MicroRNAs (miRNAs) play a pivotal role in the pathogenesis of psoriasis. Exosomes, nanoscale extracellular vesicles, facilitate the stable intercellular transfer of miRNAs, reflecting their cellular origin, which makes them promising biomarkers for various diseases. Although much of the research on psoriasis has focused on tissue miRNAs,…

Biomarkers for diagnosis, disease progression, and therapeutic response in psoriasis vulgaris: A mini-review.

Psoriasis vulgaris is a chronic immune-mediated inflammatory disease with substantial clinical, psychosocial, and public health impact. Despite advances in therapeutic options, disease management continues to rely predominantly on clinical assessment, which remains limited in its ability to detect early disease, quantify subclinical inflammation, monitor disease progression, and anticipate long-term outcomes. These limitations are further compounded by marked interindividual heterogeneity in disease course, systemic inflammatory burden, comorbidity risk, and treatment response. Although a grow…

Targeting the IL-36 Pathway: Spesolimab as a Therapeutic for Acute Flares of Pustular Psoriasis.

Generalized pustular psoriasis (GPP) is a rare and severe subtype of psoriasis characterized by widespread sterile pustules, erythema, and systemic inflammation. The condition is mediated by dysregulation of the IL-36 signaling pathway, which promotes excessive cytokine release and neutrophilic infiltration. Traditional therapies, including corticosteroids and immunosuppressive agents such as cyclosporine or methotrexate, often fail to achieve rapid or sustained disease control. Spesolimab, a monoclonal antibody that inhibits the IL-36 receptor, represents a novel targeted therapy approved for…

Clinical and Immunological Characteristics of Bullous Pemphigoid Patients With Psoriasis Comorbidity: A Retrospective Study.

Despite the recognized association between bullous pemphigoid (BP) and psoriasis, the clinical and immunological profiles, and inflammation patterns of coexistence remain undefined. We therefore conducted a retrospective cohort study of 140 BP patients without psoriasis (BP alone group) and 24 BP patients with comorbid psoriasis (BP-PsO group) to characterise these features. Average age of BP onset was significantly lower in the BP-PsO group, compared with the BP alone group (median [IQR]: 67.00 [58.00-75.75] years vs. 75.00 [64.00-82.00] years) (p = 0.021). In the BP-PsO group, 21 patients (8…
