Extensor Pollicis Longus Ruptures Following Distal Radius Osteotomy Through a Volar Approach.

PURPOSE: To investigate the cause and pathological process of extensor pollicis longus (EPL) ruptures after correction of distal radius malunion through a volar approach. METHODS: We included patients with EPL ruptures who underwent distal radius osteotomies performed through a volar approach. Data were pooled from members of the International Wrist Investigators Workshop. Patient demographics, initial injury parameters, imaging studies, preoperative and postoperative examination, intraoperative findings, surgical technique, and outcomes were compared and analyzed. Preoperative and postoperat

An olecranon chondral flap and osteochondral coronoid fracture in a spontaneously reduced elbow dislocation in a child.

Elbow injuries in children are very common and radiographs are often difficult to interpret because of the radiolucency of the cartilaginous anlage and the progressive appearance of multiple secondary ossification centres. Elbow dislocations are rare injuries in children. Coronoid fractures can occur during dislocation or relocation of the elbow and can be the only hallmark of a severe injury. The understanding of the mechanics of these injuries has undergone considerable evolution over the past decade. Intra-articular chondral flap fractures are a traumatic elevation of the hyaline cartilage

Regional variations in radial head bone volume and density: implications for fracture patterns and fixation.

BACKGROUND: Fractures of the radial head are common with most partial articular fractures resulting in an anterolateral fragment. The exact mechanism of radial head fracture is unknown; however, forces transmitted and variations in local bone density are believed important. This study quantifies the regional variations in bone density and volume of the radial head to better understand the pathomechanics of fracture patterns. METHODS: Computer tomography scan data of 18 cadaver elbows were imported into imaging analysis software. The radial head was divided into quadrants based on neutral fore

Delayed Horner's syndrome during a continuous infraclavicular brachial plexus block.

Horner's syndrome is a potential, albeit rare, feature of continuous infraclavicular brachial plexus local anesthetics infusion, mainly the result of anatomical considerations. Horner's syndrome may be described as an "unpleasant side effect" because it has no clinical consequences in itself. Nevertheless, patient discomfort and anxiety may reduce acceptance of the analgesic technique. Reassurance and close clinical monitoring of the patient are essential to enhance patient's safety and acceptance of the technique.

Standard surgical protocol to treat elbow dislocations with radial head and coronoid fractures. Surgical technique.

BACKGROUND: The results of elbow dislocations with associated radial head and coronoid fractures are often poor because of recurrent instability and stiffness from prolonged immobilization. We managed these injuries with a standard surgical protocol, postulating that early intervention, stable fixation, and repair would provide sufficient stability to allow motion at seven to ten days postoperatively and enhance functional outcome. METHODS: We retrospectively reviewed the results of this treatment performed, at two university-affiliated teaching hospitals, in thirty-six consecutive patients (

Complications and outcomes of open pediatric forearm fractures.

Few reports have documented the complications and outcomes of open pediatric forearm fractures. The authors completed of all patients (1987-1999) with open forearm fractures. Sixty-five patients with 65 injured extremities were identified, with an average age of 10.3 years. Fifty-two open fractures were grade I, 12 were grade II, and 1 was grade IIIA. Implants stabilized 40 extremities (62%), which improved alignment but not outcome. Forearms initially stabilized with implants did not undergo additional realignment procedures (0%), compared with 18.5% of forearms without stabilization. Eleven

[Treatment of old scaphoid fracture with transposition of vascularized periosteal flap and internal fixation].

OBJECTIVE: To introduce an effective method of treating old scaphoid fracture. METHODS: From April 1995 to April 2002, 11 patients with old scaphoid fracture were treated with transposition of vascularized periosteal flap and internal fixation; if necessary, the radial styloid was removed. Out of 11 patients( 8 males and 3 females, aged from 17 to 46 years), the fracture sites were medial scaphoid in 7, the proximal one in 2 and the distal one in 2. The X-ray films showed separated fracture lines. The fracture lines were less than 1 mm in 4 patients and more than 1 mm in 3 patients. Sclerosis

Traumatic forearm amputation with avulsions of the ulnar and median nerves from the brachial plexus.

A case of a traumatic forearm amputation and associated complete avulsions of the ulnar and median nerves from the brachial plexus due to a crush-traction injury of the distal part of the right forearm is reported. The patient also suffered a traumatic head injury. The injury of the upper limb and the general condition of the patient were so serious that an amputation at the 1/3 middle part of the forearm had to be performed.

The Y-shaped hypogastric--groin flap.

We present a technique of providing complete skin cover for total degloving injuries of the hand and distal third of the forearm. This involves the use of a Y-shaped hypogastric groin flap. The proximal portion of the Y is tubed allowing the distal portion of each flap to be applied to the dorsal and volar surfaces respectively. This technique allows complete skin cover of both surfaces and yet retains the advantages of the groin flap in that it still has a long pedicle allowing the hand to be put through a full range of motion, speeding rehabilitation.

Functional Outcome of Complex Elbow Fracture Managed With the Boyd Approach.

INTRODUCTION: The Boyd approach allows excellent access to the elbow and is used to treat complex elbow injuries using a single incision approach. MATERIALS AND METHODS: In this study, we retrospectively evaluated 16 patients with complex elbow injuries treated with open reduction and internal fixation using the Boyd approach between 2016 and 2018. RESULTS: All fractures were well united in anatomical position. Postoperatively, the range of motion was not significantly different between the affected and unaffected elbows. The mean Mayo Elbow Performance Index score was 95 ± 5 (range 90 to 10

Flexor Pollicis Longus Tendon Rupture After Volar Wrist Plating : Reconstruction With Palmaris Longus Interposition Graft.

Rupture of the flexor pollicis longus tendon is a rare complication after volar plating of distal radius fractures. Several surgical treatment options have been proposed but postoperative results are not well documented. The authors retrospectively studied the clinical outcome of 4 patients after flexor pollicis longus tendon reconstruction with a palmaris longus interposition graft. Minimal follow-up was 12 months. Mean active interphalangeal joint flexion (44°) and thumb opposition (Kapandji 8/10) were significantly reduced compared to the opposite side. Mean power grip (32 kg) and key pinch…

A new vascularized bone graft for scaphoid nonunion.

Nonunion and avascular necrosis after scaphoid fractures continue to be problem sequelae because of unrecognized injuries, inadequate immobilization techniques, or insufficient treatment time. Screw fixation and inlay bone grafting techniques remain the options of choice, with successful union reported in approximately 90% of patients. However, prolonged immobilization with plaster up to 4 to 6 months is required with conventional techniques. With the use of standard latex injection techniques with vascular filling of vessels to less than 0.1 mm diameter in ten fresh cadaver dissections, we di

Microcrystalline hydroxyapatite compound in prevention of bone loss in corticosteroid-treated patients with chronic active hepatitis.

To determine whether microcrystalline hydroxyapatite compound (MCHC) could reduce bone loss or its consequences in patients with chronic active hepatitis (CAH) on corticosteroid therapy, a controlled trial was conducted in 36 such patients over a period of 2 years. Both skeletal symptoms (back pain) and fractures were uncommon during the trial period but both showed non-significant differences in favour of the MCHC group and biochemical investigations were suggestive of a reduction in parathyroid over-activity. Continued reduction in bone mineral content of the radius (photon absorptiometry) w

Intraoperative fluoroscopic assessment of proper prosthetic radial head height.

BACKGROUND: Selecting a properly sized radial head prosthesis is imperative during radial head replacement. Although there has been much emphasis on avoiding overlengthening of the radius, little has been studied about how to avoid shortening. The purpose of this study was to characterize how a radial head replacement appears on intraoperative fluoroscopy depending on the height of the prosthetic radial head. METHODS: Articular cartilage thickness of the radial head was measured in 9 cadaveric elbows. Radial head replacement was performed in each specimen with 4 different prosthetic head heig

Radiographic loss of contact between radial head fracture fragments is moderately reliable.

BACKGROUND: Loss of contact between radial head fracture fragments is strongly associated with other elbow or forearm injuries. If this finding has adequate interobserver reliability, it could help examiners identify and treat associated ligament injuries and fractures (eg, forearm interosseous ligament injury or elbow dislocation). QUESTIONS/PURPOSES: (1) What is the interobserver agreement on radiographic loss of contact between radial head fracture fragments? (2) Are there factors associated with the observer such as location of practice or subspecialization that increase interobserver rel

Reconstruction of the interosseous membrane of the forearm with a graft substitute: a cadaveric study.

PURPOSE: Longitudinal radioulnar dissociation occurs when traumatic axial loading through the wrist disrupts the interosseous membrane (IOM) of the forearm and fractures the radial head (Essex-Lopresti injury). Proximal migration of the radius results in a wrist with a positive ulnar variance, which leads ultimately to painful ulnar-sided wrist degeneration and wrist pain during grasping activities that involve axial loading or ulnar deviation of the wrist. In theory reconstruction of the IOM with a graft substitute can limit proximal migration of the radius, thereby preserving wrist function.

Do functional deficits result from radial head resection?

The treatment of comminuted fractures of the radial head with primary or secondary head resection is controversial. To assess the outcome of patients after radial head resection, a retrospective study with clinical and radiologic follow-up including isokinetic testing was performed. Between 1981 and 1992, 151 patients underwent radial head resection for comminuted fractures. Fifty-nine patients were operated on during the first 2 weeks after injury (primary treatment), 47 patients were operated on between 3 weeks and 6 months after injury (early secondary), and 45 patients were operated on mor

To Splint or Not to Splint: Outcomes of Forearm Fracture Treatment With Elastic Intramedullary Nailing.

INTRODUCTION: When surgical treatment is indicated, closed reduction with elastic stable intramedullary nailing (ESIN) is a widely accepted technique for managing both-bone forearm fractures in children. However, the optimal postoperative protocol-particularly the necessity and duration of immobilization-remains a matter of debate. OBJECTIVE: This study aims to compare complication rates in patients with both-bone forearm fractures treated with ESIN, with or without postoperative splint immobilization. METHODS: This retrospective study included 137 patients with displaced and unstable both-b
