[Electrophysiological analysis of the degree of damage and regeneration of the facial nerve in tumors of the cerebellopontile angle].

In 7 patients with peripheral facial nerve paralysis during the development of pontocerebellar angle tumor or caused by damage to the nerve during surgical removal of the tumour the blink reflex and the peripheral nerve conduction were studied. Additionally, in 5 of these cases clinical and electrophysiological follow-up was carried out after operations of anastomosing of the hypoglossal nerve to the facial nerve. In all these cases the blink reflex was absent after the operation. On the other hand, from the third month on after the anastomotic operation a return was observed of the conduction…

Sulfonamide-associated keratoconjunctivitis sicca and corneal ulceration in a dysuric dog.

Long-term sulfonamide therapy for a urinary tract disorder was believed to have caused toxicosis of the lacrimal gland, and subsequently, dry eyes. Initial topical treatment of the ulcers may have potentiated the dry eye condition. The dog was referred with negligible tear production and bilateral corneal ulcers. Diagnostic evaluation of the urinary tract indicated reflex dyssynergia, a neurologic disorder causing functional urinary tract obstruction. The combination of appropriate topical and surgical therapy of the eyes, discontinuation of sulfonamide treatment, and initiation of bethanechol…

[Effect of oxygen in the tympanum on inner ear function (author's transl)].

Possible effects of extreme variations of oxygen concentration from 0% to 100% in the tympanic lumen on inner ear function were investigated through animal experiments. The experiments were performed with Ketanest anaesthetized cats under short-time acoustic irradiation (3 min 115 dB, pink noise). Inner ear function was measured by means of the stapedius reflex audiometry. On the nitrogen-flushed side (pO2 approx. 0%), the stapedius reflex threshold dropped significantly more after high-frequency irradiation than on the O2-flushed side. Even without acoustic irradiation the stapediusreflex thr…

Control of the internal anal sphincter (manometric study with human subjects).

Manometric studies of the functions of the internal anal sphincter were performed in 73 children. Twenty-one out of these patients were norm … etic pathways, but also of fibers of the sacral outflow. It is shown that the recto-anal inhibitory reflex is specific and independent of peristalsis. Arguments are given to prove that this reflex is an activity proper to the intra-mural plexus which is, however, subject to and regulated by the sacral cord.

Investigating short latency subcortical vestibular projections in humans: what have we learned?

Vestibular evoked myogenic potentials (VEMPs) are now widely used for the noninvasive assessment of vestibular function and diagnosis in humans. This review focuses on the origin, properties, and mechanisms of cervical VEMPs and ocular VEMPs; how these reflexes relate to reports of vestibular projections to brain stem and cervical targets; and the physiological role of (otolithic) cervical and ocular reflexes. The evidence suggests that both VEMPs are likely to represent the effects of excitation of irregularly firing otolith afferents. While the air-conducted cervical VEMP appears to mainly a…

Reaching with the sixth sense: Vestibular contributions to voluntary motor control in the human right parietal cortex.

The vestibular system constitutes the silent sixth sense: It automatically triggers a variety of vital reflexes to maintain postural and visual stability. Beyond their role in reflexive behavior, vestibular afferents contribute to several perceptual and cognitive functions and also support voluntary control of movements by complementing the other senses to accomplish the movement goal. Investigations into the neural correlates of vestibular contribution to voluntary action in humans are challenging and have progressed far less than research on corresponding visual and proprioceptive involvemen…

Effects of real and sham whole-body mechanical vibration on spinal excitability at rest and during muscle contraction.

We examined the effects of whole-body mechanical vibration (WBV) on indices of motoneuronal excitability at rest and during muscle contraction in healthy humans. Real and sham WBV at 30 Hz had no effect on reflexes measured during muscle contraction. Real WBV at 30 and 50 Hz depressed the H-reflex ∼45%. These depressions diminished across the five inter-bout rest intervals. The depression converted to 27% and 7% facilitation over the 15-min long recovery period following real WBV at 30 and 50 Hz, respectively. The depression, measured during the inter-bout rest, correlated r = 0.48 (P = 0.007)…

Effects of vision and lumbar posture on trunk neuromuscular control.

The goal of this study was to determine the effects of vision and lumbar posture on trunk neuromuscular control. Torso perturbations were applied with a pushing device while the subjects were restrained at the pelvis in a kneeling-seated position. Torso kinematics and the muscle activity of the lumbar part of the M. Longissimus were recorded for 14 healthy subjects. Four conditions were included: a flexion, extension and neutral lumbar posture with eyes closed and the neutral posture with eyes open. Frequency response functions of the admittance and reflexes showed that there was no significan…

[Diagnostics of latent vestibular dysfunction based on the characteristics of provocative nystagmus].

The objective of the present study was to develop a new method for the diagnostics of latent vestibular dysfunction and estimate its effectiveness based on the characteristics of spontaneous nystagmus, provocative nystagmus, and vestibuloocular optokinetic reflexes in servicemen operating moving systems. Moreover, the study was designed to assess the diagnostic and prognostic value of the new method. The study involved a total of 95 patients in whom the vestibular function was evaluated by clinical methods. The analysis included parameters of spontaneous nystagmus and provocative nystagmus, ve…

[A case of Fisher's syndrome with upbeat nystagmus].

We reported a case of Fisher's syndrome with upbeat nystagmus. The syndrome manifested itself with the appearance of diplopia and ataxic gait preceded by cold-like symptoms. Although slight blephaloptosis, marked bilateral upward gaze palsy, and restricted abduction of the right eye were observed, light reflexes and Bell's phenomenon were normal. A slight decrease in the muscular power of the distal extremities, bilateral fingertip dysesthesia, and diminished sensation of vibration were observed. All muscle stretch reflexes were absent, and no pathologic reflexes were noted. Truncal ataxia and…

Sweating and vascular responses in the face: normal regulation and dysfunction in migraine, cluster headache and harlequin syndrome.

At least four neural mechanisms influence facial blood flow. Firstly, sympathetic vasoconstrictor fibres exert a tonic constrictor influence on the vasculature of the ears, lips and nose, and sparsely supply other parts of the face. Secondly, the sympathetic nervous system actively dilates the cutaneous vasculature of the face during heat stress and emotion. Thirdly, parasympathetic vasodilator reflexes in the facial and glossopharyngeal nerves increase blood flow to the exocrine glands and tissues of the eyes, nose and mouth when these tissues are irritated. Fourthly, axon reflexes release va…

Role of visceral afferent mechanisms in functional bowel disorders.

This report analyzes the clinical and physiological evidence supporting a role for altered visceral afferent mechanisms in the pathogenesis of two functional bowel syndromes: noncardiac chest pain and the irritable bowel syndrome. Considerable recent evidence indicates that increased contractility is present only in a minority of patients and that hypercontractile episodes are not temporally related to abdominal pain. In contrast, altered sensation and motor reflexes in response to physiological stimuli, such as mechanical distention or acid, is common when appropriately investigated. The vaga…

Night pain associated with diminished cardiopulmonary compliance. A concomitant of lumbar spinal stenosis and degenerative spondylolisthesis.

Twenty patients admitted to hospital with congestive heart failure were evaluated for severe concomitant lumbosacral and leg pain. In each instance the discomfort was worse at night and progressively decreased with a resolution of the pulmonary edema. Neurologic and electromyographic examinations in all but four patients were normal with reflexes, strength and straight leg raising testing normal. An absent Achilles reflex was recognized in two, a diminished knee jerk reflex in one and in a third, weakness in the extensor hallucis longus. Lumbar spinal stenosis was identified in all of the pati…

Central projections of the ethmoidal nerve of the cat as determined by the horseradish peroxidase tracer technique.

The ethmoidal nerve innervates the nasal mucosa and constitutes the afferent limb of several upper airway protective reflexes. Protective reflexes, such as sneezing, coughing, and apnea, are those reflexes that either expel foreign substances from the respiratory tract or stop them from gaining access to the lungs. The afferents for nasal receptors are thought to be a part of the trigeminal system rather than olfactory in nature. The objective of this study was to localize the cell bodies of these ethmoidal afferents and to trace the central projections of these neurons. Horseradish peroxidase…

Abnormal oculovestibular response.

In normal individuals, the visual and vestibular systems interact through a common subcortical center located near the vestibular nuclei. When the patient is healthy, this interaction allows appropriate integration of what might otherwise be conflicting information regarding environmental motion and moving within the environment. In patients with active peripheral vestibular lesions, such as those seen in Meniere's disease, this delicate interaction loses effectiveness. In such patients, optokinetic stimuli are capable of precipitating episodes of incapacitating vertigo with its associated veg

Neurological manifestations of accidental hypothermia.

In a series of 97 patients with accidental hypothermia, alcohol abuse and Wernicke's encephalopathy were prominent causes. Pulse, systolic blood pressure, and respiratory rate were all found to decline with decreasing temperature, and there were significant changes (p less than 0.01) in level of consciousness, pupillary response, reflexes, and muscle tone. However, even in the lowest temperature range (20 degrees to 27 degrees C, or 68 degrees to 80 degrees F), 6 of 18 patients remained verbally responsive and 10 had intact reflexes. Neither eye movement abnormalities nor extensor plantar resp…
