Out of the ring and into a sling: acute latissimus dorsi avulsion in a professional wrestler: a case report and review of the literature.

We present a 29-year-old professional wrestler who sustained a traumatic latissimus dorsi tendon rupture from its humeral insertion. To our knowledge, this report is the first to describe the use of two small anterior axillary incisions to repair a traumatic avulsion of the latissimus dorsi. Our new surgical approach is an alternative treatment for highly competitive, muscular athletes, while taking the associated nerves and cosmetic appearance into consideration.

[An underestimated technique for the management of shoulder dystocia: the all-fours manoeuvre].

In two pregnant women, parturition was complicated by severe shoulder dystocia. Conventional techniques for the management of this complication of labour failed. By means of the all-fours manoeuvre in combination with conventional techniques it was possible to deliver the babies. Both mothers had received epidural anaesthesia, which did not cause any difficulty during the use of the all-fours procedure. Both newborn infants had low 1-minute Apgar scores and suffered from a brachial-plexus injury. No major maternal morbidity was associated with the use of this procedure. These cases emphasize t

Shoulder dystocia.

Shoulder dystocia can be one of the most frightening emergencies in the delivery room. Although many factors have been associated with shoulder dystocia, most cases occur with no warning. Calm and effective management of this emergency is possible with recognition of the impaction and institution of specified maneuvers, such as the McRoberts maneuver, suprapubic pressure, internal rotation, or removal of the posterior arm, to relieve the impacted shoulder and allow for spontaneous delivery of the infant. The "HELPERR" mnemonic from the Advanced Life Support in Obstetrics course can be a useful

Subjective perceptual methods for comparing backpacks in the field.

Subjective perceptual methods have provided useful information in the laboratory about small differences in backpack design when physiological and biomechanical comparisons are ineffective, but have never been used in the field. This study therefore evaluated, in a controlled field trial with 10 male participants, the suitability of quantitative and qualitative subjective perceptual approaches to distinguish between subtle design differences in two backpacks, each loaded to 15 kg. In addition, initial quantitative subjective impressions about the two backpacks during a 15 min simulated 'in-sho

Electrothermal assisted shoulder capsulorraphy--monopolar.

Monopolar electrothermal stabilization of the shoulder shows considerable promise as a treatment alternative in athletes and patients with recurrent instability. Range of motion is preserved, recovery is faster than with open procedures, there is little disruption or alteration of inherent anatomy, and, most importantly, results at 2 years appear comparable to other surgical procedures in high-demand populations. The procedure is technically easy to perform, and the complication rate is low. Success depends on surgeon's understanding of the applications of RF energy, the use of proper surgical

Expanded shoulder flap in burn sequela.

The expanded shoulder flap is presented as an alternative for the treatment of cervical retraction secondary to burning. The flap has an axial pattern, mainly based on the transverse cervical artery. The anterior limit of the flap is a line that goes from ear lobule, towards the shoulder, to the deltoid muscle. The incision for inclusion of the tissue expander is made at the shoulder, and on that line, with extension and posterior, lateral and medial detachments broad enough to slightly exceed the expander dimensions, with the valve being placed in infraclavicular position. In the immediate po

Sonography of tendons.

Recent advances in ultrasound technology and the development of high-resolution ultrasound transducers have enabled detailed depiction of superficial musculoskeletal structures. While in the past considered as complementary to magnetic resonance (MR) imaging, modern ultrasound has clearly become competitive. It is now the imaging modality of choice for evaluating tendon pathology. The major advantages of ultrasound include dynamic evaluation of structures, low cost and wide availability. The main disadvantage is a high degree of operator dependency. This article discusses the current ultrasoun

[Shoulder dystocia--a rare obstetrical complication].

Shoulder dystocia is an uncommon complication of delivery with a high neonatal morbidity rate. This review describes the use of the most important shoulder dystocia maneuvers and underlines the demand to perform a written description of all details of the management during delivery.

Superior humeral dislocation. A complication following decompression and debridement for rotator cuff tears.

Very large rotator cuff tears may be surgically irreparable and under such circumstances debridement of the edges of the cuff and bursal decompression may relieve pain. In this paper, four cases are described of the development of superior migration of the humeral head following a debridement and bural decompression. In all four patients attempts were made to repair the rotator cuff. In each case the repair failed and the humeral head migrated. Two of the patients had the humeral head replaced because of a fracture; in these patients the prosthesis displaced upwards. This serious complication

[Surgical treatment of anterior instability of the shoulder using an anteglenoid stop].

102 instable shoulders (either instable shoulders in athletes or true recurrent dislocation) have been stabilized by a Latarjet-type anteglenoid stop. In 3/4 of cases the patients were young men under 30 years of age, and 3/4 of the patients were athletes, most often of a high level. The diagnosis is based on history taking, as the clinical examination is virtually negative. The radiological assessment is easy: no arthrography, CT or other complex examinations are required. Bernageau's false-profile glenoid view shows lesions of the anterior edge of the glene in 95% of all cases. The operated

[Arthroscopic revision of the sub-acromial space in impingement syndrome].

Degenerative lesions of the subacromial structures can cause chronic anterior pain and disability of the shoulder. They are most frequently found in patients whose arms are repeatedly exposed to force while raised above head level, e.g. during tennis, volleyball or swimming. This is often due to impingement of the supraspinatus tendon, the subacromial bursa or the biceps tendon against the anterior edge and lower surface of the anterior third of the acromion, the coracoacromial ligament and, in some cases, the acromioclavicular joint. Open anterior acromioplasty as described by Neer is therefo

Three-dimensional cinematographic analysis of water polo throwing in elite performers.

Thirteen members of the United States Men's Water Polo Team were filmed using two synchronized cameras while shooting at a goal. Three-dimensional (3D) coordinates of the throwers' shoulder, elbow, wrist, and the ball were used to estimate elbow angle, elbow angular velocity, and ball velocity at release. Ball release velocities ranged from 14.5 to 25.8 m/sec, with peak elbow angular velocities averaging 1137 degrees/sec. Peak elbow angular velocity typically was reached just prior (x = 28 msec) to release as the elbow approached full extension. Results are significant in establishing the effi

Wound botulism.

Wound botulism, a potentially fatal disease, presents with signs and symptoms of food-borne botulism without the gastrointestinal manifestations. All cases have been associated with extremity wounds. The diagnosis is clinical, and aggressive ventilatory support is the cornerstone of successful therapy. A case is reported and the pathophysiology, diagnosis, and treatment are reviewed.

The modified Bristow procedure for recurrent dislocation of the shoulder.

A review of fifty-one cases of the modified Bristow procedure for recurrent anterior shoulder instability is presented. The results were favorable. The redislocation rate was 2% with few complications. The average limitation of motion was 11 degrees of external rotation. Athletic individuals with involvement of the dominant shoulder were not capable of returning to high performance levels of overhead sports activity (particularly throwing) after the operation.
