BACKGROUND: Postinflammatory hyperpigmentation (PIH) is a skin disorder characterized by hyperpigmentation resulting from heightened inflammation and/or damage to the basement membrane, melanocytes, and keratinocytes, leading to abnormal and excessive pigment deposition. Oral tranexamic acid (TXA), originally used as an antifibrinolytic for managing excessive bleeding, has garnered attention for its demonstrated safety and efficacy in treating melasma. There is a growing body of evidence regarding the use of TXA in the treatment of PIH.
OBJECTIVE: To review the mechanism of action of oral TXA

Seborrheic keratosis, which is a benign tumor composed of epidermal keratinocytes, develops common in the elderly. Uric acid generated by upregulated guanine deaminase (GDA) has been identified to cause UV-induced keratinocyte senescence in seborrheic keratosis. Seborrheic keratosis is also frequently pigmented. Growing evidences indicate that hyperuricemia is a risk factor of acanthosis nigricans, an acquired skin hyperpigmentation. The objective of this study was to investigate role of GDA and its metabolic end product, uric acid, in hyperpigmentation of patients with seborrheic keratosis us

BACKGROUND: Acute localized exanthematous pustulosis (ALEP) is a rare variant of acute generalized exanthematous pustulosis and is also defined by diagnostic criteria established by the EuroSCAR study group. Some twenty cases of ALEP have been described in the literature.
PATIENTS AND METHODS: A 61-year-old woman suddenly developed erythematous and edematous lesions after subcutaneous heparin calcium injections in the thighs, with bullous detachment at the center of the lesions and multiple vesicles and non-follicular pustules on the edges. These lesions were centered around the injection poi

The incidence of cutaneous melanoma continues to increase in pale skinned peoples in Europe and elsewhere. Epidemiological studies identified genetically determined phenotypes such as pale skin, freckles and red hair, and sunburn as risk factors for this cancer. The development of many melanocytic naevi is also genetically determined and a strong melanoma risk phenotype. Not surprisingly then, genome wide association studies have identified pigmentation genes as common risk genes, and to a lesser extent, genes associated with melanocytic naevi. More unexpectedly, genes associated with telomere

Melanocytes, a neural crest cell derivative, produce pigment to protect keratinocytes from ultraviolet radiation (UVR). Although melanocytic lesions such as nevi and cutaneous malignant melanomas are known to be associated with sun exposure, the role of UVR in oncogenesis is complex and has yet to be clearly elucidated. UVR appears to have a direct mutational role in inducing or promoting melanoma formation as well as an indirect role through microenvironmental changes. Recent advances in the modeling of human melanoma in animals have built platforms upon which prospective studies can begin to

BACKGROUND: Cutaneous melanoma is one of the most serious skin cancers. It is caused by neural crest-derived melanocytes - pigmented cells normally present in the epidermis and, sometimes, in the dermis.
METHODS: We performed a review of current knowledge on the risk factors of cutaneous melanoma. Relevant studies were identified using the PubMed, Science Direct, Medline, Scopus, Scholar Google and ISI Web of Knowledge databases.
RESULTS: Melanoma incurs a considerable public health burden owing to the worldwide dramatic rise in incidence since the mid-1960s. Ultraviolet radiation exposure i

INTRODUCTION: The increase in detected vitamin D deficiency appears to be multifactorial: an increasingly multicultural society, reduced exposure to sunlight due to concern about skin cancer and a more sedentary lifestyle and dietary changes within the population.
METHODS: This was a retrospective survey of children found to be vitamin D deficient after attending the emergency department from March 2009 until March 2010. These data were then subdivided according to their age, ethnic origin, presenting complaint and biochemical associated features.
RESULTS: We identified 89 patients with a lo

Oculocutaneous albinism is an autosomal recessive disorder characterized by a lack of pigment in the hair, skin, and eyes. Albinism is caused by defective or absent tyrosinase, an enzyme necessary for melanogenesis. Although rare in the western world, albinism is quite common in sub-Saharan Africa, likely as a result of consanguinity. Albinism has long been associated with stigma and superstitions, such as the belief that a white man impregnated the mother or that the child is the ghost of a European colonist. Recently, a notion has emerged that albino body parts are good-luck charms or posses

BACKGROUND: Neurofibromatosis type 2 (NF2) has long been regarded as an adult onset disease. However, it is now known that many NF2 patients present clinical signs and symptoms in early childhood. We here report an illustrative case of a male adolescent with an infantile onset clinical symptomatology.
PATIENT: A 15-year-old male adolescent presented with a history of congenital peripheral facial palsy, amblyopia, a retinal "membrane", and weakness of the left lower limb. Clinical, electrophysiological, radiological, and molecular studies of the patient are shown. A peripheral axonal neuropath

Type 1 diabetes mellitus is characterized by dysregulation of the immune system leading to inflammation and selective destruction of pancreatic beta cells. Mild to moderate pancreatic exocrine insufficiency is found in patients with type 1 diabetes. Diabetes mellitus may also be part of a syndrome occasionally involving hair and skin abnormalities. We report our observations on two siblings with insulin-dependent diabetes, severe exocrine pancreatic deficiency, pigmented hypertrichotic skin patches with induration and chronic inflammation. The first sibling presented at the age of 9 months wit

Scytalidium dimidiatum is a pigmented dematiaceous coelomycete that typically causes chronic superficial skin diseases and onychomycosis, as well as deeper infections, such as subcutaneous abscesses, mycetoma, and even fungemia in immunocompromised patients. A second species, Scytalidium hyalinum, has hyaline hyphae and arthroconidia and is considered by some authors to be an albino mutant of S. dimidiatum. This study aimed to confirm the presence of melanin or melanin-like compounds (which have been previously implicated in the virulence of other fungal pathogens) in S. dimidiatum from a pati

BACKGROUND: Data on the dermoscopic features of fungal melanonychia are limited.
AIM: To identify the dermoscopic features of fungal melanonychia.
METHODS: We reviewed patient files, clinical history and dermoscopic images of all cases with a diagnosis of fungal melanonychia seen at our dermoscopy unit within the past year.
RESULTS: In total, 14 cases with 20 involved nails were reviewed. The most common type of melanonychia was melanonychia striata (7/20). Multicoloured pigmentation was observed in 19 of the nails. The main dermoscopic pattern was homogeneous pigmentation; however, black p

Lichen amyloidosis (LA) is an intractable, chronic pruritic skin disorder characterized by amyloid deposition within the dermal papillae. It manifests as severely itchy, hyperpigmented papules and cobblestone plaques mainly distributed on the extremities. Conventional treatment modalities often yield partial or temporary therapeutic effects with unstable efficacy. This paper describes the case of a 60-year-old Chinese male patient presenting with a 3-year history of progressive, refractory lichen amyloidosis involving bilateral limbs. Over a 2-year period, the patient had failed multiple succe

OBJECTIVES: Oxidative stress contributes to skin damage and hyperpigmentation through excessive melanin production. Tyrosinase plays a central role in melanogenesis by catalyzing the oxidation of tyrosine to melanin precursors. Although several tyrosinase inhibitors are available, their clinical use is limited due to adverse effects. Therefore, identifying safer inhibitors with concomitant antioxidant properties remains of considerable interest. In ethnomedicine, Urtica urens L. (Urticaceae) is traditionally used to treat inflammatory conditions. This study aims to evaluate the antioxidant, ty

BACKGROUND: A "symptom cluster" is commonly known as two or more symptoms that occur concurrently. Patients with breast cancer often experience multiple symptoms simultaneously. The purpose of this study was to investigate acute radiation dermatitis symptom clusters in breast cancer patients using Mepitel film (MF) versus standard of care (SOC) aqueous cream during radiation therapy (RT).
METHODS: A secondary analysis was performed of the MF versus SOC randomized control trial (n = 376) for the prophylaxis of radiation dermatitis in patients undergoing breast cancer radiation treatment. To id

INTRODUCTION: Trichoteiromania is a friction-induced hair shaft disorder caused by repetitive rubbing/scratching and is often considered the hair shaft expression of lichen simplex chronicus ("scratching-pseudo-alopecia"). It is classically described on the scalp and presents with lichenified plaques and characteristic trichoscopy such as broom hairs and V-hairs. Pruritus or an urge to rub is common and may help distinguish it from trichotillomania (hair pulling), which is often asymptomatic. To date, the condition has been described almost exclusively as a scalp disorder.
