MPD-RC 101 prospective study of reduced-intensity allogeneic hematopoietic stem cell transplantation in patients with myelofibrosis.

From 2007 to 2011, 66 patients with primary myelofibrosis or myelofibrosis (MF) preceded by essential thrombocythemia or polycythemia vera were enrolled into a prospective phase 2 clinical trial of reduced-intensity allogeneic hematopoietic stem cell transplantation (AHSCT), Myeloproliferative Disorder Research Consortium 101 trial. The study included patients with sibling donors (n = 32) receiving fludarabine/melphalan (FluMel) as a preparative regimen and patients with unrelated donors (n = 34) receiving conditioning with FluMel plus anti-thymocyte globulin (ATG). Patient characteristics in

Healing stories: narrative characteristics in cancer survivorship narratives and psychological health among hematopoietic stem cell transplant survivors.

OBJECTIVES: Survivors of hematopoietic stem cell transplant (HSCT) have experienced a life threatening and potentially traumatic illness and treatment that make them vulnerable to long lasting negative psychological outcomes, including anxiety and depression. Nevertheless, studies show that overcoming cancer and its treatment can present an opportunity for personal growth and psychological health (reduced symptoms of anxiety and depression and high levels of emotional well-being) through resilience. However, research has not yet clarified what differentiates HSCT survivors who experience psych

Clinical outcomes following hematopoietic stem cell transplantation for the treatment of mucopolysaccharidosis VI.

Mucopolysaccharidosis VI (MPS VI, Maroteaux-Lamy Syndrome) is one of approximately 50 known lysosomal storage disorders. MPS VI is characterized by an absence or deficiency of N-acetylgalactosamine 4-sulfatase (arylsulfatase B) resulting in accumulation of dermatan sulfate. Prior to the availability of enzyme replacement therapy (ERT), the clinical management of MPS VI was limited to supportive care and allogeneic hematopoietic stem cell transplantation (HSCT); however, due to the rarity of this disease, little is known about the long-term outcomes of HSCT for MPS VI. The following retrospecti

Mechanisms of developmental programming of the metabolic syndrome and related disorders.

There is consistent epidemiological evidence linking low birth weight, preterm birth and adverse fetal growth to an elevated risk of the metabolic syndrome (obesity, raised blood pressure, raised serum triglycerides, lowered serum high-density lipoprotein cholesterol and impaired glucose tolerance or insulin resistance) and related disorders. This "fetal or developmental origins/programming of disease" concept is now well accepted but the "programming" mechanisms remain poorly understood. We reviewed the major evidence, implications and limitations of current hypotheses in interpreting develop

Risk factors for invasive aspergillosis and related mortality in recipients of allogeneic SCT from alternative donors: an analysis of 306 patients.

Invasive aspergillosis (IA) is a serious complication in patients undergoing allogeneic haematopoietic stem cell transplantation (HSCT), particularly from donors other than HLA-identical sibling. All 306 patients who underwent alternative donor HSCT between 01 January 1999 and 31 December 2006 were studied. Late IA was defined as occurring >or=40 days after HSCT. The median follow-up was 284 days (range, 1-2709). Donors were matched unrelated (n=185), mismatched related (n=69), mismatched unrelated (n=35) and unrelated cord blood (n=17). According to European Organization for Research and Trea

Current status of reduced-intensity-conditioning allogeneic stem cell transplantation for acute myeloid leukemia.

Allogeneic stem cell transplantation (allo-SCT) is the most efficient antileukemic treatment for acute myeloblastic leukemia (AML). However, elderly patients can rarely benefit from standard myeloablative allo-SCT because of an unacceptable rate of procedure-related toxicities. This point is critical when considering AML patients in first complete remission. The development of the so-called reduced-intensity conditioning (RIC) regimens appears to decrease allo-SCT-related toxicities, and has emerged as an attractive modality in AML patients not eligible for standard allo-SCT. Such RIC regimens

Hyperbolastic growth models: theory and application.

BACKGROUND: Mathematical models describing growth kinetics are very important for predicting many biological phenomena such as tumor volume, speed of disease progression, and determination of an optimal radiation and/or chemotherapy schedule. Growth models such as logistic, Gompertz, Richards, and Weibull have been extensively studied and applied to a wide range of medical and biological studies. We introduce a class of three and four parameter models called "hyperbolastic models" for accurately predicting and analyzing self-limited growth behavior that occurs e.g. in tumors. To illustrate the

Advances in medical therapy for Crohn's disease.

Therapeutic research in Crohn's disease has been intensified in recent years. This has led to many novel approaches and insights into the mechanism of action of "classic" drugs. Antibiotics remain valuable but do not offer benefit when used in addition to corticosteroids. Immunomodulators remain the cornerstone for maintenance therapy, although certain corticosteroid-dependent patients can be switched to maintenance therapy with topical steroids. Azathioprine and 6-mercaptopurine remain efficient beyond 4 years in patients with relapses and elevated C-reactive protein in spite of this therapy.

Advanced Therapeutics in Parkinson's Disease: What's New?

PURPOSE OF REVIEW: To provide a concise, clinically oriented update on advanced therapies for Parkinson’s disease. RECENT FINDINGS: Subcutaneous infusion of foscarbidopa/foslevodopa and apomorphine reduce OFF time and improve “good ON time”, though infusion site reactions remain of some concern. Image-guided programming may shorten programming time while matching motor outcomes; while adaptive/closed-loop deep brain stimulation using local field potential signals may improve symptoms and quality of life with a lower energy use. Remote programming accelerates clinical benefit and expands access

Regeneration-on-a-chip: a planarian microfluidic device enabling automated cultivation, individual tracking and in vivo imaging for regeneration study.

Freshwater planarian, a model organism renowned for its remarkable regenerative capabilities, is widely used in studies of regenerative mechanisms and stem cell biology. However, current research faces challenges in automated culture, long-term tracking, and real-time in vivo imaging at the single-worm level, limiting the efficient acquisition of accurate, individual-specific data and impeding deep investigation into regeneration dynamics. To address this, we developed a planarian microfluidic chip and an integrated microdevice for the automated cultivation, individual tracking, and in vivo im

Current Landscape and Emerging Therapeutic Targets in Diabetic Kidney Disease.

Diabetic kidney disease (DKD) remains a major global health burden and the most common cause of ESKD. The therapeutic landscape has profoundly shifted from a glucocentric model to a multifaceted strategy focusing on the holistic management of cardiovascular-kidney-metabolic syndrome. This therapeutic paradigm is now anchored by four foundational pillars: renin-angiotensin-aldosterone system inhibitors, sodium-glucose cotransporter 2 inhibitors, glucagon-like peptide-1 receptor agonists, and non-steroidal mineralocorticoid receptor antagonists, which collectively target complementary pathogenic

The Gut-Bone Marrow Axis: Deciphering the Mechanistic Impact of Microbial Metabolites on Hematopoietic Homeostasis and Disorders.

The gut microbiota is increasingly recognized as a dynamic endocrine-like microbial network that exerts systemic effects far beyond the gastrointestinal tract. Emerging evidence supports the existence of a "gut-bone marrow axis" through which gut-derived signals orchestrate hematopoietic homeostasis. However, shifting from correlative observations to causal mechanisms remains a major challenge in defining precise microbial impacts on hematopoietic outcomes. In this review, we systematically synthesize current knowledge on the molecular mechanisms by which microbial products-specifically short-

hUC-MSCs via β-NGF Alleviate Cognitive Impairment After Tibial Fracture Surgery by Regulating the STMN2/NMNAT2-SARM1-NF-κB Signaling Pathway.

BACKGROUND: Perioperative neurocognitive disorders (PND) are common postoperative complications, particularly in elderly patients, marked by learning and memory deficits with limited treatment options. Human umbilical cord mesenchymal stem cells (hUC-MSCs) hold promise due to their neuroprotective and immunomodulatory effects, but their underlying mechanisms remain unclear. METHODS: This study established a PND model in aged mice using tibial fracture intramedullary fixation surgery, followed by intravenous hUC-MSCs administration. Subsequently, behavioral tests, pathological examination, pro

Bioengineered 3D Human Trabecular Meshwork Models for Outflow Physiology and Glaucoma Research.

Primary open angle glaucoma (POAG) is one of the leading causes of irreversible blindness and is associated with dysfunction of the trabecular meshwork (TM), a three-dimensional (3D) structure that regulates aqueous humor outflow and, consequently, intraocular pressure (IOP). IOP is the only modifiable factor for glaucoma. Outflow facility is the inverse of aqueous humor outflow resistance caused by the presence of the TM and adjacent tissues, and reflects the TM's central role in IOP control, representing the most physiologically relevant measure of human trabecular meshwork (HTM) function. Preclinical evaluation of antisense oligonucleotide therapy in a mouse model of HNRNPH2-related neurodevelopmental disorder.

Mutations in HNRNPH2 cause an X-linked disorder characterized by developmental delay, intellectual disability, motor and gait disturbances, and seizures. Murine models that reproduce key clinical features of HNRNPH2-related neurodevelopmental disorder suggest that it may result from a toxic gain of function of the mutant protein or a complex loss of normal HNRNPH2 function with impaired compensation by its paralog, HNRNPH1. In this study, we tested gapmer antisense oligonucleotides (ASOs) that target murine Hnrnph2 in a non-allele-specific manner. The lead ASO reduced Hnrnph2 messenger RNA (mR

The aging eye: navigating molecular mechanisms and innovative interventions.

The global demographic shift toward aging has precipitated a surge in age-related ocular pathologies, imposing a formidable public health challenge that demands urgent intervention. Blinding disorders such as age-related macular degeneration (AMD), cataracts, and dry eye disease exemplify this crisis, with their pathogenesis being intrinsically linked to tissue-specific aging processes in the eye. At the molecular level, core pathways including telomere attrition, oxidative stress, cellular senescence, and autophagic dysregulation orchestrate this degenerative cascade. This review systematical

Challenges and Opportunities of Drug Delivery for Treatment of Alzheimer's Disease.

Alzheimer's disease (AD) is a neurodegenerative disorder characterized by progressive deterioration in cognitive functions. It represents a global health concern with increasing prevalence and devastating outcomes for the quality of life that could ultimately lead to death. AD is associated with deposition of β-amyloid (Aβ) plaques and intracellular buildup of tau proteins forming neurofibrillary tangles (NFTs), which are the main characteristics for AD brain tissues. Approved AD therapy is based mainly on symptomatic relief, and conventional medicaments often fail due to either low bioavailab

Emerging CCR5-Based Therapeutic Potential of JAK/STAT Inhibitors and CCR5Δ32 Hematopoietic Stem Cell Transplantation.

C-C chemokine receptor type 5 (CCR5) is the primary co-receptor mediating the entry of R5-tropic human immunodeficiency virus type 1 (HIV-1) into CD4+ T cells and macrophages, making it one of the most promising therapeutic targets in HIV cure research. The naturally occurring CCR5Δ32 mutation which abolishes surface CCR5 expression in homozygous individuals has provided strong clinical evidence for CCR5-directed strategies after multiple cases of sustained HIV remission following allogeneic hematopoietic stem cell transplantation (HSCT) from CCR5Δ32 donors. This review summarizes the current
