Ehrlichiosis is a rare tick-borne zoonosis caused by Ehrlichia chaffeensis, E. ewingii and related species. In solid organ transplant recipients, immunosuppression can result in atypical presentations, delayed diagnosis and poor outcomes. The disease remains under-recognised among lung transplant recipients despite its potentially fatal course. A man in his early 70s with a history of bilateral lung transplantation for chronic obstructive pulmonary disease presented with a high-grade fever and shortness of breath. Bronchoalveolar lavage (BAL) culture grew Pseudomonas aeruginosa and meropenem w

Heart failure is a global health challenge with significant impacts on quality of life and healthcare systems. Heart transplantation remains the gold standard for select patients with end-stage heart failure, yet access in Latin America is limited by economic constraints, systemic barriers, and inequities in infrastructure. Despite contributing less than 7% of global Heart transplant procedures, LATAM has made important strides, with countries like Argentina demonstrating success in increasing donor rates through presumed consent legislation. However, disparities in donor availability, central

Heart transplantation has transformed survival in end-stage heart failure but remains constrained by donor scarcity, lifelong immunosuppression, and late graft-related morbidity. Over the next 5 decades, converging advances in immune tolerance, xenotransplantation, bioengineered organs, and fully implantable artificial hearts are likely to reshape the therapeutic landscape of cardiac replacement. Early signals of these transitions are already visible in tolerance-induction trials, first-in-human cardiac xenograft implantation, cardiac tissue engineering, and next-generation mechanical circulat

PURPOSE OF REVIEW: We provide a review on the incidence, consequences, and management of obesity in patients before and after pancreas transplant.
RECENT FINDINGS: Obesity is common in patients with both type 1 and type 2 diabetes. Obesity at the time of pancreas transplant is associated with worse graft and patient survival, while weight gain after transplant is associated with insulin resistance and posttransplant diabetes. Currently, lifestyle interventions are the backbone of obesity management and can improve insulin sensitivity, but result in only modest weight loss. Metabolic and baria

In solid organ transplantation, chimerism inevitably occurs via the coexistence of donor-derived cells from the graft and host cells throughout the recipient. However, long-term immunosuppressive treatment is needed to suppress host immune responses to the foreign organ graft. The deliberate induction of stable mixed bone marrow chimerism to achieve donor-specific immunological tolerance in solid organ graft recipients is an ambitious goal that may significantly contribute to the long-term survival of solid organ grafts and their recipients. While this strategy has been effectively established

PURPOSE OF REVIEW: Transplantation in candidates with psychotic disorders has been a controversial topic for over 40 years. Reviewing the outcomes of these candidates may inform decisions going forward, though it is unclear whether outcomes with kidney recipients is generalizable to other organs, which are life-sparing not mainly life-enhancing.
RECENT FINDINGS: Outcomes in recipients with psychotic disorders after kidney transplants were described in three studies. Outcomes with heart, lung, pancreas, and small bowel or multivisceral transplants is sparse to nonexistent. There were 26 cases …

Phaeohyphomycosis is a rare subcutaneous fungal infection caused by dematiaceous fungi, commonly presenting as cystic swellings or subcutaneous abscesses. These infections predominantly affect immunocompromised individuals, including transplant recipients, due to impaired cell-mediated immunity, which plays a crucial role in fungal defense. We report three cases of post-renal transplant phaeohyphomycosis, presenting within six to 12 months post transplant. The diagnosis was based on clinical features and potassium hydroxide (KOH) mount findings showing pigmented septate hyphae. Histopathologic

PURPOSE OF REVIEW: To explore the emerging use of immune checkpoint inhibitors (ICIs) in hepatocellular carcinoma (HCC) patients eligible for liver transplantation (LT), particularly as bridging and downstaging therapies. This review also addresses the clinical challenges of integrating ICIs into transplant protocols, including graft rejection, immune-related toxicities, and gaps in evidence.
RECENT FINDINGS: ICIs have shown potential as bridging and downstaging therapies before LT, with multicentric studies reporting 75.6% successful downstaging, 85% 3-year post-LT survival, and 7.2% rejecti

Bone health after lung transplantation has not been comprehensively reviewed in over 2 decades. This narrative review summarizes the available literature on bone health in the context of lung transplantation, including epidemiology, presentation, and postoperative management. Osteoporosis is reported in approximately 30% to 50% of lung transplant candidates, largely due to disease-related impact on bone and lifestyle, and corticosteroid-related effects during end-stage lung disease (interstitial lung diseases, chronic obstructive pulmonary disease, and historically cystic fibrosis). After lung

Islet transplantation is a promising therapy for insulin-dependent diabetes. However, immune rejection and insufficient vascularization hinder the survival and function of transplanted islets. Here, we show effective engraftment of vascularized and functional mouse and rat islets transplanted into biomaterial-remodeled spleens of nonimmunosuppressed rodents and human islets transplanted into the remodeled spleens of nonhuman primates (NHPs) on varying degrees of immunosuppression. We found evidence that konjac glucomannan-modified silica nanoparticles (KSiNPs) remodeled the spleen into an extr

This letter provides a critical appraisal of the comprehensive meta-analysis by Hou et al, which synthesizes the incidence and risk factors for postoperative delirium (POD) in organ transplant recipients. Their work establishes a pooled POD incidence of 20%, with significant variability across organ types (lung 34%, liver 22%, kidney 6%), and identifies key risk factors including primary graft dysfunction, hepatic encephalopathy, and high model for end-stage liver disease/acute physiology and chronic health evaluation II scores. This commentary acknowledges the study's strength in providing a

Interventional radiology (IR) is rapidly expanding in paediatric care due to technological advancements and research, as well as its minimal invasiveness compared to surgery. New therapeutic options have been developed to manage both benign and malignant conditions refractory to medical treatments or for which surgery has failed or is not feasible, particularly regarding locally advanced liver tumours, vascular malformations and vascular tumours of the liver, lymphatic disorders, portal hypertension and complications of liver transplantation. A multidisciplinary approach is essential for the o

Degeneration or damage of neuronal circuits in the central nervous system can lead to an irreversible loss of neurons and function in the affected brain region. Neuronal transplantation is a promising therapeutic approach consisting of introducing healthy cells into the damaged or diseased regions to restore lost circuits. To achieve successful neuronal transplantation, proper integration of the graft in the host circuitry is necessary. This includes the restoration of connectivity as well as the recapitulation of the physiological characteristics of the lost endogenous neurons. An often-overl
