Rates of union and risk factors for continued nonunion following exchange nailing of tibial nonunion.

INTRODUCTION: The objective of this study was to evaluate the rate of nonunion repair success in tibia nonunions treated with exchange nailing. This retrospective cohort study was conducted across five academic Level 1 trauma centers and included 63 patients with tibia nonunions. MATERIALS AND METHODS: Patients who sustained a tibia fracture (AO/OTA 42) treated with intramedullary fixation that developed a nonunion and were subsequently treated with exchange nailing were retrospectively reviewed. The primary outcome measure was nonunion repair success based on osseous union. Additional analys…

Balloon Pericardiotomy: A Comprehensive Review and Case Series.

Pericardial effusions can occur owing to a variety of reasons such as trauma, infection, autoimmune disease, and malignancy. Cardiac tamponade depends on the rate of fluid accumulation and not solely on the volume of the pericardial effusion. Rapid accumulation of pericardial fluid can lead to impaired cardiac filling and output with hemodynamic consequences, requiring urgent or emergent intervention. Despite initial intervention on patients with cardiac tamponade, recurrence of pericardial effusions has been estimated at approximately 20%, with a mean interval to recurrence of approximately 1

Rates of anxiety disorders, mood disorders, and substance use disorders in federally qualified health centers, 2019-2022.

Federally qualified health centers (FQHCs) provide comprehensive health care services to underserved communities throughout the U.S. and are an important setting to examine changes in mental health and substance use disorders during the COVID-19 pandemic. National data from 1359 unique FQHCS serving over 29 million patients from 2019-2022 were analyzed. Diagnoses of mental health and substance use disorders were based on the Uniform Data System. From 2019-2022, the number of patients seen in FQHCs increased by 661,521 patients (2.26 % increase). Across FQHCs, diagnoses of anxiety and trauma-re

Surgical site infections: a comprehensive review.

Surgical site infections (SSIs) represent a major public health challenge, contributing to increased morbidity, mortality, and healthcare costs worldwide. This paper presents a comprehensive review of the epidemiology, classification, risk factors, microbiological aspects, treatment modalities, and prevention strategies for SSIs, based on contemporary research and evidence-based practice protocols. An extensive literature review was conducted, synthesizing existing studies on SSIs. A comprehensive search was performed in PubMed, Embase, Cochrane Library, and guidelines from leading organizatio

PURPOSE: The risk of venous thromboembolism (VTE) after surgery for lower limb trauma may be reduced with pharmacologic prophylaxis upon hospital admission and hospital discharge. To determine the rate and duration of prescription of VTE pharmacologic prophylaxis upon hospital discharge in patients who have surgery for a lower limb fracture. METHODS: Retrospective analysis of patients who had surgery for a lower limb fracture at 3 National Health Trust hospitals in the North-West of England. FINDINGS: Data from 127 patients were collected. All patients were prescribed pharmacologic VTE proph

Association between alcohol sales and facial fracture rates: an ecological analysis.

AIMS: This study aimed to evaluate national alcohol sales and their association with the number of maxillofacial fractures in Southern Finland. METHODS: Patient data of all facial fracture patients admitted to tertiary trauma centers (Helsinki University Hospital, Helsinki, Finland) from January 2014 to October 2020 were reviewed retrospectively. Information on alcohol sales in Finland was obtained from the Finnish Institute for Health and Welfare. RESULTS: The annual number of facial fractures increased, as did the number of facial fractures caused by interpersonal violence. Unexpectedly, w…

Surgical Management of Pediatric Capitellar Fractures: A Case Series of Five Patients.

Capitellum fractures are rare in pediatric patients and pose unique diagnostic and management challenges due to the presence of developing ossification centers and growth plates. Timely recognition and appropriate treatment are essential to prevent long-term complications such as stiffness, avascular necrosis, or growth disturbances. This case series presents five pediatric patients with capitellum fractures, classified as Dubberley type 2A, detailing their surgical management and their postoperative outcomes. The cases were treated at a single institution over a two-year period. Fractures wer

Gun Violence in the COVID-19 Era: Using Multiple Databases to Describe the Experience in Buffalo, NY.

ObjectivesIn 2020, the public health crises of gun violence and novel coronavirus (COVID-19) collided and interventions to decrease COVID-19 transmission displaced millions of Americans from normal activity. We analyzed the effects of COVID-19 and its resultant shutdowns on gun violence in Buffalo, NY.MethodsWe queried the Gun Violence Archive (GVA) and the hospital databases from the 2 level 1 trauma centers which serve Buffalo firearm victims between March 15th and June 24th, 2020 ("COVID") and the same time period for years 2013 (hospital data)/2014 (GVA data) through 2019 ("pre-COVID") and…

The Epidemic of Congenital Syphilis in the Indigenous and Rural Populations of South Dakota.

From 2020 to 2023, South Dakota witnessed a substantial increase in cases of congenital syphilis (CS), with the highest rates identified in rural and Native American (NA) communities. Here, we discuss 3 severe cases of CS in premature infants born to NA individuals and communities in South Dakota with poor access to prenatal care. The infants in these 3 cases presented with varying clinical conditions, including respiratory failure, persistent pulmonary hypertension of the newborn, severe direct hyperbilirubinemia, feeding intolerance, and necrotizing enterocolitis. Lack of prenatal care and o

BACKGROUND: Suture buttons and metal screws have been used and compared in biomechanical, radiographic, and clinical outcome studies for syndesmotic injuries, with neither implant demonstrating clear superiority. The aim of this study was to compare clinical outcomes of both implants. METHODS: Patients who underwent syndesmosis fixation at 2 separate academic centers from 2010 through 2017 were compared. Thirty-one patients treated with a suture button and 21 patients treated with screws were included. Patients in each group were matched by age, sex, and Orthopaedic Trauma Association fractur

The aim of the study is mainly to study the subject of BoNT-A injection to improve IAH in rats undergoing abdominal angioplasty. The study problem in surgery, especially in ICU, burn, and trauma centers, intra-abdominal hypertension (IAH), and abdominal compartment syndrome (ACS) are common complications. At present, there are various treatments for IAH. The intramuscular injection of Botulinum toxin type A (BoNT-A) into the abdominal wall has received a lot of attention. Based on this, this study proposes a method for measuring abdominal pressure, applies BoNT-A to reduce abdominal pressure i…

TRPV4: Cell type-specific activation, regulation and function in the vertebrate eye.

The architecture of the vertebrate eye is optimized for efficient delivery and transduction of photons and processing of signaling cascades downstream from phototransduction. The cornea, lens, retina, vasculature, ciliary body, ciliary muscle, iris and sclera have specialized functions in ocular protection, transparency, accommodation, fluid regulation, metabolism and inflammatory signaling, which are required to enable function of the retina-light sensitive tissue in the posterior eye that transmits visual signals to relay centers in the midbrain. This process can be profoundly impacted by no

Management of Bleeding Pyogenic Granulomas in Acute Care Settings.

BACKGROUND: Pyogenic granulomas are acquired, benign growths of capillary blood vessels that are commonly seen in the pediatric population. Patients with these lesions often present to emergency departments and urgent care centers with persistent bleeding after minor trauma. Much of the published literature describing the management of pyogenic granulomas, however, is focused on outpatient or definitive therapies, and there is limited information on the management of acute bleeding. OBJECTIVE: We conducted a narrative review to present and evaluate strategies and therapies available to emerge
