A novel technique of abdominoplasty for prune belly syndrome.

Prune belly syndrome is an extremely rare congenital condition occurring predominantly in males. This triad syndrome comprises of partial or complete deficient abdominal wall muscles, undescended testes and dilated urinary collecting system. We present the case of a 2-year-old male patient, who presented with classic prune belly syndrome, operated with modification in classical Monfort technique of abdominoplasty. The basis of this approach was to save and use the supraumbilical abdominal wall which has better tensile strength. The infraumbilical skin was mostly discarded. The outcome was bett

Bullhorn hernia: a rare traumatic abdominal wall hernia.

Traumatic abdominal wall hernia (TAWH) is rare despite the high prevalence of blunt abdominal trauma. Bullhorn hernia occurs as a result of a direct blow to the abdominal wall by the horn of a bull, which disrupts the muscles and fascia and leads to hernia formation. We report a rare case of bullhorn TAWH in a 70-year-old patient who presented with swelling at the left lumbar region. The patient was managed by immediate surgical intervention. A surgeon must have high index of suspicion for the diagnosis of this condition as missed hernias in this setting pose a high risk of strangulation and g

Higher matrix stiffness promotes VSMC senescence by affecting mitochondria-ER contact sites and mitochondria/ER dysfunction.

Abdominal aortic aneurysm (AAA) is a prevalent condition characterized by the weakening and bulging of the abdominal aorta. This study aimed to investigate the impact of a stiff matrix on vascular smooth muscle cells (VSMCs) in AAA development. Bioinformatics analysis revealed that differentially expressed genes (DEGs) in VSMCs of an AAA mouse model were enriched in cellular senescence and related pathways. To simulate aging-related changes, VSMCs were cultured on stiff matrices, and compared to those on soft matrices, the VSMCs cultured on stiff matrices exhibited cellular senescence. Further

Enhancing core stability and strength through abdominal drawing-in maneuver training using a sphygmomanometer: A narrative review.

BACKGROUND: The transversus abdominis (TrA) muscle plays a crucial role in lumbar spine stability and core strength. Activation of the TrA is commonly achieved through the abdominal drawing-in maneuver (ADIM). Dysfunction in the deep abdominal muscles is often linked to low back pain and instability. The ADIM technique involves drawing the navel inward toward the spine while maintaining a neutral spine position and normal breathing, thereby engaging the TrA. Sphygmomanometer is a valuable tool for measuring muscle activation, tracking changes in pressure, and ensuring core stability and streng…

Measuring transversus abdominis activity using pressure biofeedback unit-A technical report.

BACKGROUND AND PURPOSE: Pressure biofeedback is a tool that can detect the movement of a structure and may be used as an indicator of muscle function. It's widely used to measure the transversus abdominis (TrA) muscle activity. Pressure biofeedback (PBU) monitors the abdominal wall movement by measuring the pressure change during abdominal hollowing and can indirectly evaluate the TrA muscle function, which is considered as a valuable tool. The evaluation of the training of core muscles, including the transversus abdominis, requires a reliable outcome. Various methods using different positions

Postherpetic Abdominal Pseudohernia Evaluated by Standing Ultrasonography: A Case Report.

Abdominal pseudohernia is a condition characterized by abdominal wall bulging without structural defects. We report a case of pseudohernia following herpes zoster infection diagnosed and followed up using ultrasonography. A 60-year-old woman developed right flank pain with vesicular eruptions, treated with valacyclovir. While her pain improved, she noticed new right lateral abdominal bulging. Standing-position ultrasonography revealed relaxation of the right rectus abdominis muscle without fascial defects, confirming an abdominal pseudohernia. Subsequent ultrasounds showed improvement in muscl

Absence of a hemidiaphragm: mechanical implications.

Respiratory system mechanics were evaluated in a 22-year-old asymptomatic man with absence of the left hemidiaphragm. We described changes in esophageal pressure (Pes), gastric pressure (Pga), chest wall configuration, and mediastinal motion during tidal breathing, breaths to total lung capacity (TLC), and Mueller maneuvers in the upright and supine position. We predicted that contraction of the single hemidiaphragm would drive the abdominal contents caudal on the side with the intact hemidiaphragm and displace the abdominal contents cephalad on the other side. This would drive the mediastinum

Primary and secondary causes of constipation in older people.

Older people are at risk of constipation because of age-related changes that may weaken intestinal muscles, decrease peristalsis and reduce physical activity. Primary causes of constipation include decrease in fluid intake, decrease in ingestion of high fibre foods which may be associated with reduced ability to chew food, and weak abdominal muscles reducing effective evacuation. Secondary causes include disease processes such as dementia, Parkinson's, hypothryroidism, diverticular disease and prescribed medications, including narcotics, calcium channel blockers, anticholinergics and antidepre…

Intra-Arterial, but Not Intrathecal, Baclofen and Codeine Attenuates Cough in the Cat.

Centrally-acting antitussive drugs are thought to act solely in the brainstem. However, the role of the spinal cord in the mechanism of action of these drugs is unknown. The purpose of this study was to determine if antitussive drugs act in the spinal cord to reduce the magnitude of tracheobronchial (TB) cough-related expiratory activity. Experiments were conducted in anesthetized, spontaneously breathing cats (n = 22). Electromyograms (EMG) were recorded from the parasternal (PS) and transversus abdominis (TA) or rectus abdominis muscles. Mechanical stimulation of the trachea or larynx was us

Impact of Lumbar Postures on the Functioning of Pelvic Floor Muscles Among Osteoporotic Post-Menopausal Women.

Introduction Maintaining continence and providing support to the abdominal contents and sexual functioning are among the primary roles of pelvic floor muscles. The pelvic floor muscles work in synergy with the abdominal muscle to perform these functions. Abdominal muscle activation in the sagittal plane is influenced by the lumbar spine posture. As pelvic floor dysfunction is common among post-menopausal women, this study aims to find out the relationship between lumbar posture and electromyographic (EMG) activity of the rectus abdominis (RA) muscle and vaginal pressure (VP) as a functioning o

[Results of surgical treatment of diastasis of the rectus abdominis muscles].

The work gives a comparative analysis of the treatment of 249 patients with diastasis of the rectus abdominis muscles according to the degree of restoration of the working capacity, the existence of gastrointestinal discomfort, recurrence of the disease, and the dynamics of changes of the morphological and functional values of these muscles. The clinical and functional-morphological results were best after plastics of the anterior abdominal wall by the method suggested by the authors. With the use of this method the results were good in 40 (71.4%) and satisfactory in 16 (28.6%) of patients. Re

Scaling of axial muscle architecture in juvenile Alligator mississippiensis reveals an enhanced performance capacity of accessory breathing mechanisms.

Quantitative functional anatomy of amniote thoracic and abdominal regions is crucial to understanding constraints on and adaptations for facilitating simultaneous breathing and locomotion. Crocodilians have diverse locomotor modes and variable breathing mechanics facilitated by basal and derived (accessory) muscles. However, the inherent flexibility of these systems is not well studied, and the functional specialisation of the crocodilian trunk is yet to be investigated. Increases in body size and trunk stiffness would be expected to cause a disproportionate increase in muscle force demands an

Physiologic tension of the abdominal wall.

BACKGROUND: Tension-free abdominal closure is a primary tenet of laparotomy. But this concept neglects the baseline tension of the abdominal wall. Ideally, abdominal closure should be tailored to restore native physiologic tension. We sought to quantify the tension needed to re-establish the linea alba in patients undergoing exploratory laparotomy. METHODS: Patients without ventral hernias undergoing laparotomy at a single institution were enrolled from December 2021 to September 2022. Patients who had undergone prior laparotomy were included. Exclusion criteria included prior incisional hern

[Incisional hernia: open abdominal wall reconstruction. Current state of the technique and results].

The treatment of complex midline hernias remains a particular challenge. The currently refined knowledge of the anatomy in the cadaver laboratory and advancing clinical experience have changed our present approach. The aim of this review is to present a description of the updated surgical procedures and outcomes. We favor the retromuscular or preperitoneal layer for mesh implantation, including the Rives-Stoppa procedure (sublay mesh) and posterior component separation with the Madrid modification. We operated on 334 complex midline incisional hernias: 6.3% retromuscular preperitoneal, 15% aft

Pathways of the preperitoneal plane: from the "fatty triangle" in Rives to the "fatty trident" in extended retromuscular abdominal wall reconstruction. A tribute to Prof. Schumpelick.

PURPOSE: Extended retromuscular dissection performed for abdominal wall reconstruction in complex abdominal wall repair has progressively exposed the anatomy between the peritoneal layer and abdominal wall muscles. This study aimed to assess the morphology and distribution of preperitoneal fat in a cadaveric model and its influence in retromuscular preperitoneal dissections. METHODS: Thirty frozen cadaver torsos were dissected by posterior component separation. The shape of the preperitoneal fat was identified, and the dimensions and more significant distances were calculated. RESULTS: The r
