Toward an acoustic typology of motor speech disorders.

Acoustic methods have progressed to the point that an acoustic typology of the motor speech disorders can be constructed from a parameteric assessment of the speech subsystems (e.g., phonation, nasal resonance, vowel articulation, consonant articulation, intonation, and rhythm). The results of this analysis can be interpreted in respect to global functions in speech (e.g., voice quality, intelligibility, and prosody). This paper reviews studies showing that specific acoustic analyses have demonstrated or potential value toward the overall goal of constructing acoustic profiles of dysarthria an

Synaptophysin in the cochlear nucleus following acoustic trauma.

Chinchillas are notable for a low-frequency hearing range similar to that of humans and a marked sensitivity to loud noise. A single noise exposure that produces cochlear damage may lead to progressive loss of synaptic endings in the cochlear nucleus, followed by new axonal growth. As an index of synaptic regulation during such changes, we have examined the expression of a synaptic vesicle protein, synaptophysin, in the cochlear nucleus following a damaging acoustic stimulus in adult chinchillas. With one ear protected by a plug, following a 3-h exposure to an octave-band noise of 108 dB sound

Acoustic neuroma presenting as exercise-induced vertigo.

We present two subjects with previously undiagnosed acoustic neuromas who complained of vertigo whenever they ran. One had normal hearing while the other already had a unilateral sensorineural deafness. Hyperventilation for 30 seconds provoked an ipsilateral beating nystagmus and reproduced the vertiginous sensation in both subjects. Hyperventilation is a simple bedside test that should be performed when assessing a subject with vertigo or when there is a clinical suspicion of an acoustic neuroma.

[Enlarged translabyrinthine approach in large acoustic neurinomas].

The translabyrinthine approach has long been, and in some centers is still, considered inadequate for the removal of large acoustic neuromas (AN). Over the years, with experience, the original technique has been modified, extending the approach to what is now called the enlarged translabyrinthine (ET) approach. Applying these modifications, between April 1987 and February 2000, the Gruppo Otologico in Piacenza, Italy removed 132 ANs, 3 cm or larger, from the cerebello-pontine angle. These tumors accounted for 25.9% of the 510 cases of AN to undergo surgery during that period. Of the 132 cases

Characteristics of acoustic noise in echo-planar imaging.

Characteristics of the acoustic noise generated by magnetic resonance imagers of different systems and performance levels were studied when operating in echo-planar imaging (EPI) sequence. Continuous equivalent A-weighted sound pressure levels (Leq) and peak impulse sound pressure levels (Lpeak) during EPI were measured in 12 clinical super-conducting MRI systems (0.5-1.5 T). Sound pressure levels and frequency spectra of EPI were compared with those of nine different pulse sequences. EPI sound pressure levels differed among institutions (Leq = 94.2 +/- 2.7 dBA. Lpeak = 109.1 +/- 3.5 dB), but

Acoustic measures of symptoms in abductor spasmodic dysphonia.

Speech of patients with abductor spasmodic dysphonia (ABSD) was analyzed using acoustic analyses to determine: (1) which acoustic measures differed from controls and were independent factors representing patients' voice control difficulties, and (2) whether acoustic measures related to blinded perceptual counts of the symptom frequency in the same patients. Patients' voice onset time for voiceless consonants in speech were significantly longer than the controls (p = 0.015). A principle components analysis identified three factors that accounted for 95% of the variance: the first factor include

Acoustic rhinometry assessment of the nasal cycle in neonates.

In this study the nasal cycle in newborns was assessed using acoustic rhinometry with miniprobe (RHIN 2100). Enrolled in the study were 67 newborns aged 2 to 4 days. Each subject was assessed for 3 h, examinations taking place every 30 min. At least six measurements, three from each nostril, were taken at each examination. The miniprobe RHIN 2100 (SR Electronics ApS) was used for evaluation of nasal geometry. The minimal cross-sectional area (MCA) and the total volume of the anterior 45 mm (VOL45) were calculated and used for statistical analysis. The mean volume of MCA and VOL45 in the examin

[Asymptomatic acoustic neurinoma associated with hypophyseal macroadenoma].

UNLABELLED: The widespread use of gadolinium-enhanced MRI (Gd-MRI) in our hospitals has supposed an increase in detection of asymptomatic acoustic neuromas (AN) as incidental finding. Concurrent primary intracranial tumors of different cell types are extremely uncommon. CLINICAL CASE: A 23 year-old woman complained of irregular menstrual cycles and galactorrhea for two years. A Gd-MRI scan showed a pituitary macroadenoma and an AN of 1.4 cm. with intracanalicular and cisternal extension. The patient did not have any otologic or neuro-otologic symptoms or signs. Audiograms revealed bilateral n

A cluster of acoustic neuromas in fish hatchery workers.

The National Institute for Occupational Safety and Health (NIOSH) received a request from the United States Fish and Wildlife Service (FWS) to determine if there is a relationship between acoustic neuroma and fish hatchery work. The request was triggered by a report from an FWS employee who suspected the possibility of such an association. Investigators used data provided by the personnel office of the FWS to calculate the incidence of acoustic neuroma among fish hatchery workers, and then to compare it with national rates. Four confirmed cases of acoustic neuroma were found among former fish

[Stereotaxic irradiation of vestibular schwannoma (acoustic neuroma)].

A vestibular schwannoma (acoustic neurinoma) is a benign tumour localized in the cerebellopontine angle; it can give rise to cranial nerve symptoms. In recent years stereotactic irradiation has become an alternative to radical surgery. Stereotactic irradiation is administered with a gamma knife unit or with an adapted linear accelerator, as a single fraction (radiosurgery) or fractionated (stereotactic radiation therapy). Stereotactic irradiation gives local control rates of over 90%. Post treatment hearing preservation rate is over 60% and treatment related toxicity is low. Comparable treatme

The effects of amplitude modulation on acoustic reflex decay.

The temporal decay of the acoustic reflex provides the basis for an objective audiological test that differentiates cochlear from retrocochlear pathologies. The classic sign of a neural lesion is a rapid decay of the reflex under conditions of pure-tone stimulation for frequencies of 1,000 Hz and below. This restriction to lower frequencies is due to the fact that even normal ears show decay for higher-frequency signals. At present, it is unclear whether the acoustic reflex decay (ARD) seen in normal ears is related to frequency-specific channels or whether the critical variable is the timing
