Establishing the cause of vomiting can be challenging in 10% of patients. In such situations, the clinicians need to consider non-gastrointestinal causes including metabolic and endocrine disorders. Clinical and laboratory clues may prompt further relevant tests that will lead to the correct diagnosis. We discuss three such patients with chronic vomiting.

Graves' disease is an autoimmune thyroid condition characterized by the production of autoantibodies against the thyrotropin receptor. It is known to be associated with autoimmune conditions such as myasthenia gravis, Addison's disease, type 1 diabetes mellitus, and vitiligo. We present a case of rare autoimmune association of Graves' disease with thymic hyperplasia which regressed after treatment with antithyroid drugs. Exact pathophysiology of thymic hyperplasia in Graves' is not well understood; it is likely to be the result of rather than the cause of Graves' disease.

A previously healthy 60-year-old Japanese male farmer presented with shock, initially unresponsive to fluid resuscitation. Upon incidental observation of brown nail discoloration, adrenal insufficiency was suspected, confirmed by elevated ACTH levels. The timely recognition of nail hyperpigmentation led to the diagnosis of tuberculous Addison's disease, allowing for appropriate treatment and resolution of the condition.

Bilateral adrenal masses were found in a 64-year-old man with pulmonary tuberculosis and a pulmonary mass lesion suspicious for carcinoma. CT-guided fine needle biopsy of the left adrenal gland revealed tuberculosis. Adrenal metastases were thus excluded and probably Addison's disease, considered to be a potential complication of adrenal tuberculosis, could be prevented.

The authors present three women with early hypergonadotropic ovarian failure. In one patient autoimmune aetiology of the disease was suggested by the co-existing Addison's disease and idiopathic hypothyroidism. In the other two women the causes of hypogonadotropic amenorrhea remained unclear. All three women were delivered of normal children: one after two spontaneous pregnancies and the other two after a pregnancy induced by short-term estrogen therapy.

UNLABELLED: Autoimmune polyglandular syndrome type 2 (APS-2) features autoimmune Addison's disease, autoimmune thyroid disease, and/or type 1 diabetes mellitus. Addison's disease is occasionally associated with depressive symptoms, therefore patients with APS-2 might present primarily in a psychiatric clinic. Such atypical primary presentation can possibly lead to delayed and/or inadequate diagnosis and management.
CASE PRESENTATION: A 57-year-old female patient was referred to our psychiatric clinic from an internal medicine hospital presenting severe depressive symptoms. Upon admission, she

BACKGROUND: A new ELISA for autoantibodies to steroid 21-hydroxylase (21-OH Ab) is described.
METHODS: In the assay test sample autoantibodies form a bridge between 21-OH coated onto the plate well and liquid phase 21-OH-biotin. Bound 21-OH-biotin is detected by the addition of streptavidin peroxidase and colorogenic peroxidase substrate.
RESULTS: Of 100 samples from patients with autoimmune Addison's disease, 86 (86%) were positive for 21-OH Ab ELISA whereas 84 (84%) were positive in an immunoprecipitation assay based on 125I-labeled 21-OH. Six (0.6%) of 928 healthy adult blood donors and 1

The binding of CRH-binding protein (CRH-BP) in plasma to labeled human CRH has been examined in patients with hypothalamic-pituitary-adrenal disorders. Compared with that in normal subjects, CRH-BP binding decreased in patients with Cushing's syndrome of pituitary or adrenal origin and in patients who were treated with a high dose of glucocorticoids over a long period of time. On the other hand, CRH-BP binding increased in patients with Addison's disease or hypopituitarism. In patients with Addison's disease, the high level of CRH-BP binding fell to the control level after glucocorticoid repla

BACKGROUND: Previous studies have identified correlations between autoimmune and aortic diseases; however, the causal connection between these conditions remains uncertain and lack of comprehensiveness. This study aimed to investigate the potential causal relationships between autoimmune and aortic diseases from a genetic perspective.
METHODS: A two-sample Mendelian randomization (MR) study was implemented to investigate the relationships between 17 common autoimmune diseases and aortic diseases. To serve as instrumental variables (IVs), we extracted single-nucleotide polymorphisms from both

The aim of this narrative review was to discuss manifestations of endocrine disease other than diabetic foot in the lower extremities. Acromegaly, Addison's disease, Cushing's syndrome, hypo- and hyperthyroidism, hypo- and hyperparathyroidism, autoimmune polyglandular syndrome, hypopituitarism, and glucagonoma are among the endocrine diseases that may present with clinical manifestations in the lower extremities. Clinical signs vary depending on the underlying condition. Clinical suspicion is necessary for early diagnosis. Treatment of the underlying endocrine disease usually results in improv

Diseases of the adrenal gland are as important for the general practitioner as for the endocrine specialist. The high prevalence of some adrenal endocrinopathies, such as adrenal incidentalomas (1-2% of the population) and primary aldosteronism (6% of hypertensives), which affect millions of patients, makes adrenal diseases a relevant health issue. The high morbidity and mortality of some of the rarer adrenal diseases, i. e., Addison's disease and Cushing's syndrome (Table 1), make early detection and appropriate treatment such a challenge for the health care system.

Chronic mucocutaneous candidiasis is a primary immunodeficiency characterized by persistent or recurrent candidal infections of the skin, nails and/or mucosal tissues. CMC can be associated with endocrinopathies such as hypoparathyroidism, Addison's disease, hypothyroidism, type 1 diabetes mellitus or hypogonadism. Other associated conditions include autoimmune diseases such as autoimmune gastritis and autoimmune hepatitis. We report a patient with CMC and decreased T cell, natural killer cells without other associated condition.

Spontaneous Achilles tendon rupture associated with long-term oral steroid use is not uncommon, particularly in older patients who use these drugs daily to treat systemic diseases. Rupture often results in a large defect, which complicates surgical repair. The authors review Achilles tendon rupture associated with systemic and local steroid use and present a case of rupture due to chronic oral steroid use in a patient with Addison's disease.

Adrenocortical insufficiency (Addison's disease) is a functional diagnosis. At the beginning of this century, tuberculosis was recognized as the main etiological cause (50-70%). Today, however, tuberculosis represents only 10% of the cases. Adrenocortical insufficiency is mainly caused by autoimmune adrenalitis (more than 50%). Unilateral adrenocortical disease is usually asymptomatic or presents, as in the case described, with minor symptoms. This case report of adrenal tuberculosis illustrates the current challenges of diagnosis and therapy.

Particular aspects of plasma ACTH radioimmunoassay are examinated. The results obtained with this method in adrenocortical diseases (Addison's disease, Cushing's syndrome, congenital adrenal hyperplasia) are reported and clinical value in etiological diagnosis of Cushing's syndrome is outlined. Moreover the results obtained in other syndromes of endocrinologic interest (massive obesity, Turner's and Klinefelter's syndromes, Laurence-Moon-Biedl syndrome, true precocious puberty, primary hypothyroidism, hypopituitary dwarphism) are reported and discussed.

Primary adrenal insufficiency (PAI), or Addison's disease, is a rare, potentially deadly, but treatable disease. Most cases of PAI are caused by autoimmune destruction of the adrenal cortex. Consequently, patients with PAI are at higher risk of developing other autoimmune diseases. The diagnosis of PAI is often delayed by many months, and most patients present with symptoms of acute adrenal insufficiency. Because PAI is rare, even medical specialists in this therapeutic area rarely manage more than a few patients. Currently, the procedures for diagnosis, treatment and follow-up of this rare di

Autoimmune diseases and sarcoidosis may be related and, especially, the association between sarcoidosis and autoimmune thyroid disease has long been recognized. The frequency and type of endocrine autoimmunity was examined in a series of Swedish patients with sarcoidosis. Of all patients (N = 89) with documented sarcoidosis attending the Department of Pulmonary Medicine between January 1980 and December 1991, 78 patients (44 males and 34 females; median age at the time of the study 48 years. range 22-81 years) were examined at the Department of Endocrinology, Malmö University Hospital, in the

Pigmented lesions of the oral cavity are important entities. The wide range of their clinical differential diagnosis includes such diverse systemic conditions as Addison's disease, Peutz-Jeghers syndrome, malignant melanoma, Kaposi's sarcoma, as well as specific oral lesions, such as amalgam tattoo or gingival melanosis. This paper reports a very rare cause of oral pigmentation, a melanic type, related to the use of Premarin.
