The authors report the clinico-anatomical analysis of 23 observations on malignant tumors of the adrenal cortex. The greatest diagnostic difficulties are encountered in the so-called "non-functioning" tumors, devoid of normal activity and mostly observed in adults. To pinpoint further the diagnosis, a wide use of morphological methods, especially needle biopsy, is recommended. A prolonged preclinical period, a tendency to predominantly hematogenic metastases spread, the fibrinolytic activity of parenchymal cells, pyrogenic and allergic effects of the catabolism products should be referred to a

A 56-year-old man presented with a history of hypertension; clinically, the patient had primary aldosteronism (PA) and a 4-cm left adrenal tumor. The left adrenal glands, resected by adrenalectomy, also contained ectopic thyroid tissue (ETT). An immunohistochemical analysis of steroid-converting enzymes revealed an aldosterone-producing adenoma (APA). Among 19 previously reported cases of adrenal ETT, 4 had adrenal hormonal abnormalities, all of which were PA. This is the first case of adrenal ETT coexisting with APA, confirmed by steroid-converting enzyme expression. Further analyses using cu

BACKGROUND: Adrenocortical carcinoma (ACC) is a rare, heterogeneous malignancy with a poor prognosis. ACCs are classified as functioning and non-functioning. The pathogenesis of ACC remains elusive, and diagnosis of ACC is currently based on pathology. In the absence of other effective approaches, surgical resection is the preferred treatment option.
CASE PRESENTATION: Here, we report a case of ACC in the retroperitoneum. The patient underwent radical adrenalectomy and remained disease-free throughout a 6-month follow-up.
CONCLUSIONS: Radical surgical resection is an efficient therapy for AC

BACKGROUND: The distinction between unilateral aldosterone-producing adenoma or bilateral adrenal hyperplasia as causes of primary aldosteronism is usually made by adrenal CT or by adrenal vein sampling (AVS). Whether CT or AVS represents the best test for diagnosis remains unknown. We aimed to compare the outcome of CT-based management with AVS-based management for patients with primary aldosteronism.
METHODS: In a randomised controlled trial, we randomly assigned patients with aldosteronism to undergo either adrenal CT or AVS to determine the presence of aldosterone-producing adenoma (with

PURPOSE OF REVIEW: To illustrate the steps for clinical management of primary aldosteronism from screening evaluation to surgical and/or medical treatment.
RECENT FINDINGS: It is now widely accepted that primary aldosteronism represents the most common form of endocrine hypertension and its early diagnosis is crucial for hypertensive patients who can be cured by the surgical removal of an aldosterone-secreting adenoma or benefit from a specific medical treatment with mineralocorticoid receptor antagonists. Recent evidence indicates that hyperaldosteronism is indeed associated with detrimental

This is a report of a rare and unusual case of adrenal pathology. A patient presented with clinical and biological signs of primary aldosteronism and computed body tomography scan led to our suspecting the presence of a left adrenocortical carcinoma. The in vitro studies performed on the resected tumour showed very low synthesis of mineralocorticoids and glucocorticoids. The patient could not be reexamined until 15 months later, when he still suffered hypertension; another tomography scan revealed a mass on the right adrenal gland. The studies performed on this second tumour confirmed the diag

In the surgical treatment of patients with primary aldosteronism, differentiation of idiopathic hyperaldosteronism due to bilateral adrenal hyperplasia should be kept in mind. Controversies exist, on the other hand, concerning the diagnosis and management of multiple nodular hyperplasia which often coxists with adrenal adenoma. We studied correlations between pathological findings and clinical diagnosis on 13 patients with primary aldosteronism who were operated on during the 19 year period from 1963 to 1981, and the results were as follows; (1) Histologically, adrenal glands of these patients

BACKGROUND: Primary aldosteronism (PA) is the most prevalent cause of secondary hypertension (HTN), and adrenocorticotropic hormone (ACTH)-independent Cushing's syndrome (CS) represents a distinct form of adrenal-derived secondary HTN. Their coexistence resulting from aldosterone- and cortisol-producing adenomas (A/CPAs) is an extremely rare clinical entity. This is particularly true for the subtype characterized by bilateral adrenocortical adenomas, in which one adenoma predominantly secretes aldosterone (ALD) and the other predominantly secretes cortisol (COR), for which only limited clinica

CONTEXT: The adrenal cortex consists of zona glomerulosa (ZG), fasciculata (ZF), and reticularis. Aldosterone-producing cell clusters (APCCs) that strongly express aldosterone synthase (CYP11B2) are frequently found in adult adrenals and harbor somatic mutations that are also detected in aldosterone-producing adenomas (APAs). Primary aldosteronism is mainly caused by APAs or idiopathic hyperaldosteronism (IHA). We presume that APCCs are causing IHA and are precursors of APAs. However, the gene expression characteristics and especially the development of APCCs are not well understood.

The aim of this review was to determine the impact of primary aldosteronism on health-related quality of life (HRQoL) and mental health. We performed a systematic literature search up to July 2017 in six electronic databases. First, we screened the articles derived from this search based on title and abstract. Second, the selected studies were systematically reviewed and checked for our predefined inclusion criteria. The search yielded 753 articles, of which 15 studies met our inclusion criteria. Untreated patients with primary aldosteronism showed an impaired physical and mental HRQoL as comp

Primary aldosteronism is a potentially curable cause of hypertension, especially when caused by an adrenal adenoma. Aldosteronomas because of their small size often elude techniques to locate them. This case illustrates the advantages, disadvantages and complications of noninvasive techniques used for their diagnosis. A patient with hypertension and hypokalemia underwent an adrenal venous effluent sampling for measurement of aldosterone concentrations. This procedure was complicated by an injury to the right adrenal gland. Subsequently, it was difficult to control the patient's hypertension an

Chemical shift imaging allows unambiguous separation of lipid and water signals. Useful images include opposed-phase (water minus fat) and fat-suppressed images, which can be obtained with spin-echo or gradient-echo methods. Chemical shift techniques can be used to reduce artifacts, to expand dynamic range, to diagnose fatty infiltration of the liver, to distinguish between benign and malignant adrenal lesions, and to diagnose benign teratomas in women.

We explored a 61 year old woman with mild hirsutism. An adrenal tumor was found in the left adrenal, which was held responsible for the androgen secretion. The in vitro incubation of the tumor tissue showed an impaired 11 beta-hydroxylation of 11-deoxycortisol. This is a rare and unusual case of adrenal pathology showing that a deficiency in 11 beta-hydroxylase activity does not rule out the presence of an adrenocortical adenoma.

Autonomous hypersecretion of aldosterone (primary hyperaldosteronism) is caused by either hyperplasia (usually bilateral) or an adenoma (frequently unilateral) of the adrenal cortex. Systemic hypertension due to an aldosteronoma is a potentially curable condition through surgical extirpation of the offending organ. In our experience with 37 patients clinically suspected to have primary hyperaldosteronism, radiological methods contributed significantly in preoperative diagnosis. These included (1) selective bilateral adrenal vein catheterization and blood sample collection, (2) adrenal venograp
