Breaking Through the Glass Ceiling-Having That Conversation: A Constructivist Grounded Theory Study Exploring the Experiences of Nephrology Nurses' Practice in Kidney Supportive Care in Canada.

BackgroundDialysis as treatment for kidney failure can result in significant physical and psychosocial symptom burden. Kidney supportive care (KSC), encompassing advance care planning (ACP), is an approach to care involving early identification and treatment of symptoms that improves the quality of life of people receiving dialysis. However, ACP is underused and often initiated late in the illness. The delay or lack of engagement in KSC by nephrology nurses until near the end of life may result in people receiving care that is discordant with their values, wishes, and preferences.PurposeThe pu

Optimizing Hereditary Cancer Risk Assessment and Genetic Testing Workflow in Interprofessional Women's Health Practice: A Multicenter Survey of Advanced Practice Providers.

INTRODUCTION: Advanced practice providers (APPs), including certified nurse-midwives (CNMs)/certified midwives and nurse practitioners, are well positioned to meet the growing demand for women's health care services. In interprofessional obstetric and gynecologic settings, APPs' scope of practice includes performing hereditary cancer risk assessments (HCRAs) and genetic evaluations, comparable with physicians. However, time constraints, limited confidence, and inadequate resources may hinder APPs' ability to routinely and effectively incorporate HCRA and genetic testing into practice. PROCESS

Nursing Roles in Diabetic Foot Ulcer Prevention, Wound Management, and Digital Monitoring.

Diabetic foot ulcers (DFUs) remain a major cause of infection, hospitalization, impaired quality of life, and non-traumatic lower-limb amputation among people with diabetes. Effective management requires coordinated prevention, structured foot-risk assessment, pressure offloading, wound care, infection surveillance, patient education, and multidisciplinary follow-up. This narrative review examines the nurse-led and nurse-supported components of contemporary DFU care. Nurses contribute through routine foot and wound assessment, reinforcement of preventive behaviors and treatment adherence, supp

A conceptual framework for developing a boundary-based confidentiality training programme for primary healthcare nurses managing patients living with HIV.

BACKGROUND: Loss to follow-up and treatment defaulting in HIV care led to hospitalisations for advanced complications, undermining retention strategies. Breaches of confidentiality compel patients into transferring clinics to conceal their HIV status. The shift from traditional nurse-patient relationships to team-based care, while improving comprehensive services, complicates confidentiality maintenance and can increase risks of unintended disclosures, eroding trust. Observations across ten community health centres with nurse-led HIV and AIDS services in the OR Tambo district revealed high rat

Optimizing vascular access for patients receiving intravenous systemic therapy for early-stage breast cancer-a survey of oncology nurses and physicians.

BACKGROUND: Despite advances in systemic therapy choices for patients with early-stage breast cancer, optimal practices for intravenous (IV) access remain unknown. That lack of knowledge holds particularly true for the use of central venous access devices (cvads) such as peripherally inserted central catheters (piccs) and implanted vascular access devices (ports). METHODS: Using a survey of Canadian oncologists and oncology nurses responsible for the care of breast cancer patients, we evaluated current access practices, perceptions of complications, and perceptions of risk, and we estimated c

The Effect of Simulation-Based Electroconvulsive Therapy Education on Perception and Knowledge of Nursing Students: A Semiexperimental Study.

OBJECTIVES: Despite the recognized benefits and safety of electroconvulsive therapy (ECT) for treating mental disorders, negative perceptions persist among both the public and mental health professionals. This stigma is often rooted in historical practices and media portrayals, despite modern advancements in the technique. This study aimed to assess the impact of simulation-based ECT education on nursing students' perceptions and knowledge regarding ECT before and after the procedure. METHODS: This study was planned to use a single-group pretest and posttest quasi-experimental design with a 3

Communication with patients and their families, who undergo bone marrow transplantation.

Bone marrow transplantation (BMT) is a widely accepted treatment for malignant and nonmalignant diseases. With advances such as autologous and allogenic stem cell transplant, umbilical cord blood transplant, and improved biomedical support therapies, the need to evaluate the impact technology on the patient and the family continues to be an important aspect of medical and nursing research. BMT has been described as a procedure associated with isolation of the patient, prolonged hospitalizations, rapid fluctuations in medical conditions, frequent and often life-threatening infections, and graft

[End-of-Life Nursing Experience Providing Care to a Father With Terminal Rectal Cancer With Dependent Children].

This report describes the end-of-life nursing experience of the author in providing care to a 44-year-old father with terminal rectal cancer with two dependent children. Diagnosed in young adulthood with rectal cancer and multiple metastases, the patient initially pursued aggressive treatment. However, rapid disease progression during the terminal stage led to disrupted family processes, role conflict, and significant caregiver role strain. The author's motivation for compiling this report stemmed from the limited research addressing end-of-life care for "parents with advanced cancer and their

Identification and validation of end-of-life nursing care competencies for Kenyan nurses: A modified Delphi technique.

BACKGROUND: The number of patients requiring end-of-life care in acute care hospitals in Kenya continues to increase due to increases in non-communicable diseases. Despite advances in the field of palliative care (PC), its utilization is deficient in our setting, particularly at the end of life. Many patients and family members have unmet needs during the end-of-life phase. The aim of this study was to identify and validate end-of-life nursing competencies required by non-specialized nurses in Kenya. Validation of competencies for end-of-life care would help to develop relevant professional de

Decisions to treat or not to treat pneumonia in demented psychogeriatric nursing home patients: development of a guideline.

Non-treatment decisions concerning demented patients are complex: in addition to issues concerning the health of patients, ethical and legal issues are involved. This paper describes a method for the development of a guideline that clarifies the steps to be taken in the decision making process whether to forgo curative treatment of pneumonia in psychogeriatric nursing home patients. The method of development consisted of seven steps. Step 1 was a literature study from which ethical, juridical and medical factors concerning the patient's health and prognosis were identified. In step 2, a questi

Bedside Placement of the Postpyloric Tube in Infants.

BACKGROUND: Patients in the neonatal intensive care unit are a vulnerable population with specific nutritional requirements, which include increased protein and caloric needs for adequate growth. Some infants cannot tolerate gastric feeds and need to have postpyloric feeds to grow. Placement of a postpyloric tube can be done by gastric insufflation. Gastric insufflation is a technique where air is inserted into the stomach as a nasogastric tube is advanced through the pylorus to the duodenum. There is research to support this technique in pediatrics, but scant evidence exists for placement of
