Ten-Year Follow-Up of Oral Implants in Bone With Limited Bucco-Oral Dimensions: A Prospective Case Series.

BACKGROUND: It has been suggested that 1-2 mm of bone width at the buccal and lingual aspect is required for a successful long-term implant outcome. Low levels of evidence support this minimum threshold of bone width. This prospective study aimed at evaluating the outcome of implants placed in alveolar ridges with limited bucco-oral dimensions. MATERIALS AND METHODS: One-hundred implants (dia. 3.5 mm) were placed in 28 patients with narrow alveolar ridge dimensions (<4.5 mm width) without augmentation procedures and conventionally loaded. Intra-oral radiographs were taken at implant placement

Effectiveness of PRF alone versus PRF plus biphasic calcium phosphate as a scaffold for gingival mesenchymal stem cells in induced vertical alveolar ridge defects.

This research evaluated the effectiveness of platelet-rich fibrin (PRF) membrane alone versus the combined use of PRF membrane and biphasic calcium phosphate (BCP) block as a scaffold for gingival mensenchymal stem cells (GMSCs) in treatment of induced vertical alveolar ridge defects. Three standardized saddle-type defects were created in 10 adult mongrel dogs. Following a 4-week recuperation period, the defects were randomly assigned to one of three groups based on the treatment protocol: group I: defects augmented with only PRF membranes loaded with GMSCs, group II: treated with a combinatio

Immediate Augmentation of Compromised Extraction Sockets in Chronic Periodontitis Patients: 1-Year Results of a Case Series on Volumetric and Histologic Response.

The present case series evaluated three-dimensional volumetric bone tissue changes and new bone formation in severely resorbed extraction sockets augmented with Bio-Oss collagen and a covering collagen membrane in nine chronic periodontitis patients. Healing was by secondary intention. After 12 months of healing, the augmentation procedure appeared not only to compensate for bone remodeling but also appeared to repair a significant portion of the buccal wall. The mineralized tissue filled the 91.49% ± 6.77% of the maximum volume for regeneration. Overall, a mean of 49.6% new bone, 27.1% residu

[Reconstraction of severe anterior maxillary ridge atrophy: combination of several surgical techniques].

Achieving an esthetic and functional implant supported restoration in the maxillary anterior segment can be challenging especially in a severe atrophy. The reconstruction of atrophic alveolar ridges using autologous Onlay Bone Grafting (AOBG) was originally reported in 1975. It's still considered as the 'gold standard' bone-grafting material, as if combines all properties required in a bone graft material; osteogenesis, osteoinduction, and osteoconduction. The use of intraoral donor sites, such as the mandibular symphysis, and ramus, offers no cutaneous scarring, minimal discomfort, and less m

A contemporary approach for treatment planning of horizontally resorbed alveolar ridge: Ridge split technique with simultaneous implant placement using platelet rich fibrin membrane application in mandibular anterior region.

Treatment of edentulous sites with horizontal atrophy represents a clinical situation in which the positioning of endosseous implants might be complex or sometimes impossible without a staged regenerative approach. This case report presents management of horizontally deficient mandibular anterior ridge with a contemporary approach to treatment planning and application of platelet-rich fibrin membrane for ridge split technique and simultaneous implant placement. Implants in anterior mandibular area are considered to be most predictable, stable, with high success rate and patients' satisfaction

[Atraumatic bone expansion: Interest of piezo-surgery, conicals expanders and immediate implantation combination].

The durability of dental implants depends on the presence of a 1mm coating bone sheath all around the fixture. Therefore, bone resorption represents a challenge for the practitioner. Bone expansion is a surgical technique that allows the management of horizontal bone atrophy. Cortical bone splitting allows for an enlargement of the residual crest by displacement of the vestibular bone flap. The immediate placement of implants secures the widening and allows for a 97% survival rate. However, bone expansion is hard to undertake in sites with high bone density. Furthermore, the use of traditional

The use of free fat tissue transfer from the buccal fat pad to obtain and maintain primary closure and to improve soft tissue thickness at bone-augmented sites: technique presentation and report of case series.

PURPOSE: Atrophic ridges usually have both hard and soft tissue shortages, and bone augmentation can make soft tissue closure difficult. The aim of this case series is to present a new method utilizing transfer of free fat tissue grafts (FFG) from the buccal fat bad (BFP) during bone augmentation to enhance immediate primary soft tissue closure and improve long-term soft tissue thickness and quality at the recipient site. MATERIALS AND METHODS: Patients with moderate to severe ridge atrophy in different regions of the jaws were treated over a 4-year period with bone augmentation and FFG. The

Augmentation of an anterior edentulous ridge for fixed prosthodontics with combined use of orthodontics and surgery: a clinical report.

Untreated periodontitis may lead to tooth and tissue loss that can result in diminished masticatory function and esthetic deformities. Combined periodontal/prosthodontic treatment for patients with advanced periodontal disease has been well documented, and treatment results can often be improved with the use of adjunctive orthodontic treatment such as forced eruption. This clinical report describes a method of preprosthetic preparation of an edentulous ridge for a fixed partial denture that incorporates the combined use of orthodontics and surgical ridge augmentation.

Applications of guided tissue regeneration with dental implants.

This article has discussed and illustrated various applications of GTR associated with dental implants. At present, in the opinion of this author, the material of choice for these procedures is GTAM. Other barriers are currently available while still more are under development. It is not within the scope of this article to include a comprehensive discussion of all available materials but it is noteworthy that resorbable membranes such as GUIDOR (Butler) show promise. At present the predictability of the various GTAM techniques discussed in this article, in the experience of the author are as f

[The treatment and rehabilitation of patients with secondary adentia and atrophy of alveolar process of maxilla].

The novel method of surgical treatment and rehabilitation of patients with secondary adentia and atrophy of alveolar process of maxilla was suggested. The method allows to preserve satisfactory microcirculation of the implantation area and to achieve hermetizm by wound closure. The use of the endoscope for visualizing the operation field minimizes the possibility of the sinus membrane intraoperative perforation and decreases the overall surgical traumatizm.

Closure of large perforation of sinus membrane using pedicled buccal fat pad graft: a case report.

Although the sinus lift procedure is relatively safe, it does pose some potential problems. The most prevalent intraoperative complication is perforation of sinus membrane. Various techniques and materials have been developed for the repair of the perforated sinus membrane. In this case, pedicled buccal fat pad (BFP) was used for the closure of the large perforation on the sinus membrane and dental implants were placed simultaneously. No serious infections have occurred, and clinical and radiographic findings at the 1-year follow-up were adequate.

Clinical, radiographic, histologic, and histomorphometric comparison of PepGen P-15 particulate and PepGen P-15 flow in extraction sockets: a same-mouth case study.

PURPOSE: Although the efficacy of PepGen P-15 particulate has been conclusively demonstrated in two multicenter clinical studies, the effect of a new hydrogel formulation containing PepGen P-15 on bone repair has not been previously demonstrated. MATERIALS AND METHODS: PepGen P-15 Flow contains PepGen P-15 particulate in a biocompatible hydrogel, which provides spacing between the particles. In this case study, bone repair of both PepGen P-15 particles and PepGen P-15 Flow was compared to natural bone in same-mouth extraction sockets. RESULTS: The radiographic, histologic, and histomorphomet

[Implants in avascular iliac crest bone transplants. Prospective study of 176 implants].

Total and subtotal defects of the upper and lower alveolar ridge require bone grafts to make masticatory rehabilitation possible using alloplastic dental implants. The aim of this study was to establish the survival rate of dental implants inserted in avascular iliac bone grafts. Furthermore, we aimed to discover whether the prognosis of implants can be influenced by choosing one-stage or a two-stage procedure. In a prospective study, jaw defects in 40 patients were treated with avascular mono- or bicortical bone transplants taken from the right iliac wing. A total of 134 of 176 dental implant

[Restoration of the alveolar crest with a biomaterial filler. A clinical and histological study].

The implantation of biocompatible hydroxyapatite is used for human mandibular ridge augmentation in edentulous patients. Histologically, we studied such a material after a long period of implantation (1 1/2 to 2 years) in two patients. The histologic study showed a narrow meshed net of collagen tissue surrounding the hydroxylapatite particles. In the periphery of that tissue, we could see an osteogenesis: the bony trabeculae thus formed firstly were made of an immature bony matrix with an irregular "woven" pattern; secondarily, a mature bone, quite similar to the normal bone, occurred. In our

Extraction Socket Augmentation with Autologous Platelet-Rich Fibrin (PRF): The Rationale for Socket Augmentation.

After tooth extraction, the alveolar ridge undergoes a physiological process of remodelling and disuse atrophy. Socket augmentation (SA) has been shown to preserve alveolar bone volume in order to facilitate implant placement and reduce the need for staged grafting at a later date. Although autogenic grafting has been shown to be the gold standard in bone regeneration, it has significant disadvantages. To prevent post-extraction volumetric alterations and alveolar bone resorption occurring, alternative grafting materials, including xenografts, alloplasts, and allografts, have been used success
