Evaluation of the new, patient-adjustable socket system Varos in the early phase of prosthetic rehabilitation: a pilot study.

BACKGROUND: Current prosthetic sockets often provide limited anatomical fit, especially in patients with residual limb volume changes and fluctuations. AIM: To address these issues, Ottobock has developed the Varos Socket, a modular socket that can be adjusted by the user. The aim of this study was to evaluate the potential benefits and acceptance of a newly designed patient-adjustable socket in transfemoral amputees in early phase of prosthetic rehabilitation. DESIGN: A prospective A-B-A pilot study was conducted. SETTING: The setting of the study was an Orthopedic Rehabilitation Clinic.

[Body Integrity Identity Disorder (BIID): interrogation of patients and theories for explanation].

Apotemnophilia, Amputee Identity Disorder or Body Integrity Identity Disorder (BIID) is the intensive feeling that the body will be "more complete" after amputation of a limb. The article disputes the question of matching personality characteristics of these subjects and asks for motives. Based on reports of nine individuals, triggering experiences are referred. In contrast to other children, often these subjects were fascinated by the sight of a handicapped person. In the article is investigated, whether the concerned limb showed more affections. Described is typical pretending behavior. Para

Resurrection of the amputations of Lisfranc and Chopart for diabetic gangrene.

When a patient with neuropathic diabetic gangrene of the foot has sepsis, it is not always necessary to do a below-knee guillotine amputation or a Syme's amputation. In more than six years we have done 18 successful open Lisfranc's and Chopart's amputations. Improved ambulation has been achieved in three months using simple shoe prostheses. All flaps are fashioned immediately and never sutured. No equinus deformities have developed.

Staged below-knee amputations for septic peripheral lesions due to ischaemia.

Preservation of the knee joint is of paramount importance in lower limb amputation for ischaemia. Clinical predictors of healing are unreliable in patients with septic peripheral lesions due to ischaemia. Seventy-three patients in whom a below-knee amputation was considered likely to heal, based on the temperature and appearance of the skin and bleeding from skin and muscle flaps, were divided into two groups. Twenty-nine (Group A) had a primary below-knee (BK) amputation at the site of election with delayed primary skin closure, while 44 patients (Group B) initially had a guillotine BK amputa

Salvage of failed amputation about the hip in peripheral vascular disease by open wound care and nutritional support.

Five consecutive patients with wound and/or plastic surgical flap failure after hip disarticulation or amputation at the lesser trochanteric transfemoral level were treated with local tissue debridement, open wound management, culture-specific antibiotic therapy, and nutritional supplementation. All of the patients underwent amputation about the hip as a result of ischemic necrosis of the lower extremity. Four of the five patients were able to achieve wound healing by second intention. The fifth patient died 2 months after the surgery. None of the patients required revision surgery. One patien

Prevention of neuroma in elective digital amputations by utilization of neurovascular island flap.

Following hand surgery, painful neuromas may impair the quality of the functional result. In planned, elective digital amputation, neuroma formation is quasi-unavoidable because the nerve is cut. Several techniques have been proposed to prevent the formation of neuroma but, in our opinion, the only logical way to avoid it, is not to cut the nerve. For this purpose, we proposed, in cases of planned, elective digital amputation, an intervention inspired by the technique described by Littler in 1956. This technique consists of isolating as an island, the pulp of the amputated finger while preserv

A reliable surgical approach for the two-staged amputation in unsalvageable limb and life threatening acute progressive diabetic foot infections: tibiotalar disarticulation with vertical crural incisions and secondary transtibial amputation.

BACKGROUND: If all efforts to treat acute progressive diabetic foot infection remain insufficient, the decision of major amputation should be undertaken. For this purpose, guillotine amputation is usually performed first. However, guillotine amputation below the knee level may cause the corresponding infection to spread to preserved anatomical spaces. METHOD: First stage of our procedure consists of tibiotalar disarticulation and vertical incisions performed throughout the lower leg to remove the septic foot and drain the compartments. During the interval period, appropriate antibiotherapy an

[Current issues of rehabilitation of the disabled older persons with limb loss due to obliterative arterial disease.].

Utilization of prosthetic and orthotic care services among patients with limb stump due to obliterative diseases of peripheral arteries was analyzed based on population register of limb amputations. It was established that not more than 34% of patients apply for primary prosthesis after lower limb amputation. This rate decreases among patients older than 60 years, and significantly decreases among patients older than 70 years - down to 16,2%. In order to improve the mobility level and quality of life among this population of patients it is considered to design and implement interdepartmental r

Systematic review and meta-analysis of the efficacy of perineural local anaesthetic catheters after major lower limb amputation.

OBJECTIVE: The aim of this systematic review and meta-analysis was to evaluate the effects of using an intraoperatively placed perineural catheter (PNC) with a postoperative local anaesthetic infusion on immediate and long-term outcomes after lower limb amputation. METHODS: A systematic review of key electronic journal databases was undertaken from inception to January 2015. Studies comparing PNC use with either a control, or no PNC, were included. Meta-analysis was performed for postoperative opioid use, pain scores, mortality, and long-term incidence of stump and phantom limb pain. Sensitiv

Simultaneous above-knee amputation and ipsilateral total hip arthroplasty in a neglected neck of femur fracture with an insensate limb.

Total hip arthroplasty (THA) in patients with ipsilateral mid-thigh amputation is surgically challenging. We report a case of same setting THA and ipsilateral above-knee amputation in a male patient. The patient had 8 months old neglected neck femur fracture and ipsilateral femur shaft fracture with 14 cm bone loss. There was an associated neurovascular (femoral artery injury and sciatic nerve palsy) deficit at the initial insult; however, the limb survived because of well-formed collaterals. The limb was insensate at the time of presentation because of complete sciatic nerve palsy. The decisi

Penile amputation and successful reattachment and the role of winter shunt in postoperative viability: A case report and literature review.

Traumatic self-amputation of the penis by a psychotic patient is rare. Microvascular replantation is the favored management approach. There are no known cases of self-amputation followed by ingestion of the stump and subsequent replantation. A 51-year-old patient with paranoid schizophrenia presented 2 hours following penile amputation. He had swallowed the excised portion, which was endoscopically retrieved from the stomach in the emergency department. Successful reattachment was achieved including microvascular repair of the dorsal penile arteries without cavernosal arterial anastamoses. A W

Elective spare parts free flap-calcaneal fillet of foot flap.

Severe lower limb trauma with significant soft tissue injury can be managed with reconstruction or, if this is impossible, amputation. If amputation is considered, below-knee amputation preserving limb length is optimal for long-term functional outcome. At times, soft tissue/bony injury can limit the ability to preserve limb length, particularly with proximal tibial injuries. We present a case of elective below-knee amputation where leg length and adequate soft tissue coverage was only possible by using an osteocutaneous fillet of foot and lower leg spare parts free flap, maintaining the tibia

Ear replantation without microsurgery.

Reconstitution of the amputated ear remains a challenge to the plastic surgeon. Reattachment as a composite graft of the total or subtotal amputated ear is unreliable. Microsurgical replantation can be performed in a minority of cases; however, difficulty with adequate venous drainage complicates the technical complexity of these cases. To enhance survival of a reattached ear segment, Mladick et al. advocated use of the retroauricular pocket principle. This technique involves deepithelialization of the amputated part, followed by anatomic reattachment to the amputation stump and then burial in

Below-knee amputation in patients with vascular disease and prosthetic fitting problems.

A study was made of 544 cases with lower limb deficiencies caused by obliterative diseases; 262 cases were below-knee amputees. Of these, 106 (40%) were amputees transferred from other clinics for prosthetic fitting; in 156 cases (60%) the amputations were performed in the Institute. Amputations were carried out using one of two techniques according to the state of arterial and collateral circulation. The posterior flap below-knee amputation (Burgess, 1969) was employed in 94 cases, the other 62 amputations were carried out using a modification of that technique which was characterized by the

The use of Integra with a sensate fasciocutaneous pedicled flap for the salvage reconstruction of a below knee amputation after pedestrian vs train multi-planar degloving injury.

Degloving injuries occur when there is sufficient tangential force to a body surface to disrupt the structures connecting skin and subcutaneous tissues to the superficial fascia. There may also be associated injuries to the underlying soft tissues, bone, nerves and vessels. The injury may be so severe that the limb is non-viable and requires amputation. Our patient sustained a severe lower limb degloving injury after an accident where the leg was caught and dragged along the platform by a train. There was insufficient skin to cover a below knee amputation stump. So, the dermal regeneration tem

Ischaemic wound complications in above-knee amputations in relation to the skin perfusion pressure.

Healing of the stumps in 59 above-knee amputations was correlated with the local skin perfusion pressure (SPP) measured preoperatively as the external pressure required to stop isotope washout using 131I-(-) or 125I-(-) antipyrine mixed with histamine. Out of the 11 cases with an SPP below 30 mmHg no fewer than 9 (82 per cent) suffered wound complications. Out of the 48 cases with an SPP above 30 mmHg severe wound complications occurred in only 4 cases (8 per cent). The difference in wound complication rate is highly significant (P < 0.01). It is concluded that the SPP can be used to predict i

Acroangiodermatitis (pseudo-Kaposi sarcoma): a rarely-recognized condition. A case on the plantar aspect of the foot associated with chronic venous insufficiency.

Acroangiodermatitis, often known as pseudo-Kaposi sarcoma, is an uncommon angioproliferative entity related to chronic venous insufficiency, arteriovenous fistulae, paralysed limbs, amputation stumps, vascular syndromes and conditions associated with thrombosis. It presents most frequently as purple macules, papules or plaques in the dorsal aspects of the feet, especially the toes, and the malleoli. We report a case of acroangiodermatitis in the plantar aspect of the foot, misdiagnosed for two years, in which haematoxylin-eosin hystopathological stain and immunolabeling with CD34 histochemistr…
