Hydromorphone ameliorates postoperative pain and depressive disorder in women receiving cesarean section under spinal anesthesia.

BACKGROUND: Pain and depressive mood disorders during cesarean sections negatively impact both mothers and infants. Studies have shown that hydromorphone has positive effects on both pain management and depressive mood disorders. This study examines how hydromorphone influences postoperative pain and depressive disorders in parturients undergoing cesarean sections under spinal anesthesia. METHODS: This single-center, controlled, randomized trial involved 130 patients. Parturients in the intervention (H) group received patient-controlled intravenous analgesia (PCIA) with hydromorphone combined

Spinal Anesthesia Using the Double-Needle Technique for Thoracolumbar Spine Fracture Surgery: A Case Series.

Spinal anesthesia (SA) has emerged as a viable alternative to general anesthesia (GA) for thoracolumbar spine surgeries. The double-needle technique (DNT) aims to enhance the precision and efficacy of SA in these procedures. The DNT (SA at two levels) involves performing two separate spinal punctures at different vertebral interspaces during the same procedure, often used to optimize anesthesia spread. This case series evaluates the feasibility, safety, and clinical outcomes of DNT in patients undergoing thoracolumbar spine fracture fixation. This case series includes five patients diagnosed w

Subdural hematoma following head trauma and spinal anesthesia for elective cesarean delivery: a case report.

Subdural hematoma is a rare but life-threatening complication of obstetric spinal anesthesia. We report a case of acute subdural hematoma following spinal anesthesia for elective cesarean delivery that likely promoted rebleeding from an unrecognized prepartum head trauma. A healthy 32-year-old primiparous patient (ASA II) underwent a cesarean delivery with spinal anesthesia performed with a 27-gauge Quincke needle. The perioperative course was uneventful; however thirty hours postoperatively, the patient developed a sudden severe fronto-orbital headache followed by progressive deterioration in

Minimally Invasive Unilateral Lumbar Decompression in the Lateral Position Under Spinal Anesthesia: A Retrospective Study.

Introduction Lumbar spinal stenosis (LSS) is a debilitating condition characterized by the narrowing of the lumbar spinal canal, often affecting older adults. This study evaluates the efficacy and clinical outcomes of a novel minimally invasive decompression technique via a unilateral approach under spinal anesthesia in a lateral position. Methods A retrospective monocentric observational study was conducted from January 2023 to January 2024, including 74 patients who underwent surgery between 2018 and 2023. Data was collected through a 30-question survey, utilizing validated scales from the l

Prophylactic effect of Tamsulosin on postoperative urinary retention in Inguinal hernia repair under spinal anesthesia.

BACKGROUND AND PURPOSE: Inguinal hernioplasty is a common surgical procedure, often associated with complications such as post-operative urinary retention (POUR). POUR, characterized by an inability to urinate despite a full bladder following a surgery that may need foley catheterization that on its own can lead to urinary tract infection, stricture, prolonged hospitalization, and increases cost of hospital care. Tamsulosin is a selective alpha-1 adrenergic blocker that can increase urine flow by relaxing the smooth muscle of urethra and prostate, thereby as a less invasive method may be effec

General Anesthesia Blocks Pain-Induced Hemorrhage and Locomotor Deficits After Spinal Cord Injury in Rats.

Research has shown that engaging pain (nociceptive) pathways after spinal cord injury (SCI) aggravates secondary injury and undermines locomotor recovery. This is significant because SCI is commonly accompanied by additional tissue damage (polytrauma) that drives nociceptive activity. Cutting communication with the brain by means of a surgical transection, or pharmacologically transecting the cord by slowly infusing a sodium channel blocker (lidocaine) rostral to a thoracic contusion, blocks pain-induced hemorrhage. These observations suggest that the adverse effect of pain after SCI depends o

Thermodiluted relative tidal volume estimation using a thermal camera in operating room under spinal anesthesia.

BACKGROUND: Estimating relative tidal volume is an important factor when monitoring breathing status. The relationship between temperature and respiration volume has rarely been studied. In this paper, a formula was derived for calculating thermodiluted respiration volume from temperature changes in the nasal cavity. To evaluate the proposed formula, the study compared the relative tidal volume estimated by the proposed formula with that recorded by a respiration volume monitor (Exspiron1Xi, RVM). Thermal data were obtained for 8 cases at a rate of 10 measurements per second. Simultaneous reco

Inferior Vena Cava Collapsibility Index Can Predict Hypotension and Guide Fluid Management After Spinal Anesthesia.

PURPOSE: We hypothesized that inferior vena cava collapsibility index (IVCCI)-guided fluid management would reduce the incidence of postspinal anesthesia hypotension in patients undergoing non-cardiovascular, non-obstetric surgery. METHODS: A receiver operating characteristic (ROC) curve was used to determine the diagnostic value of IVCCI for predicting hypotension after induction of spinal anesthesia and calculate the cut-off value. Based on the cut-off variation value, the following prospective randomized controlled trial aimed to compare the incidence of postspinal anesthesia hypotension b

Evaluation of Amino Acid Infusion preventive effect on Hypothermia during Spinal Anesthesia for Hip Arthroplasty.

BACKGROUND: Hypothermia increases bleeding during surgery, risk of ischemic heart disease and postoperative wound infection. Intravenous amino acid increases cell synthesis and produces heat. Our goal was evaluating of the effect of amino acid on intraoperative hypothermia under spinal anesthesia. METHODS: This is a randomized, double-blinded clinical trial that 36 adults undergoing Hip Arthroplasty were randomly assigned into two groups of 18 each. One group received Amino Acids solution (Aminoven 10%) 500ml (240ml/h) throughout spinal anesthesia, and control group received saline solution.

Combined spinal-epidural anesthesia for abdominoplasty and liposuction in Limb-Girdle Muscular Dystrophy: case report.

We report the anesthetic management with combined spinal-epidural in a patient with limb-girdle muscular dystrophy type 2A, submitted to abdominoplasty and liposuction. The patient had onset of symptoms at 8 years old, diagnosed by muscular biopsy, presenting muscle weakness in the scapular and pelvic girdles, with reduced mobility. We performed monitorization with noninvasive blood pressure, oximeter, thermometer, and electrocardiogram. In the postoperative period, she showed no clinical signs of rhabdomyolysis, myotonia, or adverse effects, maintaining hemodynamic stability. The anesthesia t

Persistent cauda equina syndrome after cesarean section under combined spinal-epidural anesthesia: a case report.

A 29-year-old pregnant woman was delivered by cesarean section under a combined spinal-epidural anesthesia. Thirty hours after an uneventful surgery, she complained of weakness in her lower extremities and developed fecal and urinary incontinence. Lumbosacral magnetic resonance imaging showed thickening and clumping of cauda equina nerve roots at L2-3 level, consistent with the diagnosis of arachnoiditis. The patient was included in an intense rehabilitation program with a diagnosis of cauda equina syndrome. Most of the symptoms resolved within a few days, but right side foot drop persisted fo

Spinal anesthesia mediates improved early function and pain relief following surgical repair of ankle fractures.

BACKGROUND: To our knowledge, no study to date has compared the use of spinal and general anesthesia in patients undergoing operative fixation of an unstable ankle fracture. The purpose of this study was to assess the effects of anesthesia type on postoperative pain and function in a large cohort of patients. METHODS: Between October 2000 and November 2006, 501 patients who underwent surgical fixation of an unstable ankle fracture were followed prospectively. Patients receiving spinal anesthesia were compared with a cohort who received general anesthesia. All patients were evaluated at three,

A randomized trial comparing colloid preload to coload during spinal anesthesia for elective cesarean delivery.

BACKGROUND: Hypotension after spinal anesthesia for cesarean delivery is common. Previous studies have demonstrated that a crystalloid fluid "coload" (rapid administration of a fluid bolus starting at the time of intrathecal injection) is superior to the conventional crystalloid preload (fluid administered before the intrathecal injection) for preventing hypotension. Colloid preload provides a sustained increase in central blood volume. We hypothesized that, in contrast to crystalloid, a colloid preload may be more effective than colloid coload for reducing the incidence of spinal anesthesia-i
