[A not-so-typical ankle sprain].

BACKGROUND: An estimated hundreds of thousands of patients are treated in the Netherlands every year for an ankle sprain. We describe two cases where the course of an ankle sprain differed from the usual presentation. CASE PRESENTATION: Case 1: A 22-year-old male presented to the Emergency Department following an inversion sprain to the ankle. The pain and swelling were localized around the subtalar joint rather than the lateral ankle ligament(s), with limited range of motion in the subtalar joint. Imaging revealed a fracture through a tarsal coalition. The patient was treated with cast immob…

Ankle Instability as a Global Concept.

Ankle sprains most commonly injure the superior fascicle of the anterior talofibular ligament, often initiating a cascade of biomechanical changes termed the "domino effect." Chronic ankle instability can lead to both intra-articular and extra-articular alterations, which may become symptomatic, sometimes even without overt instability symptoms. Accurate clinical examination is crucial, as standard imaging may miss ligament injuries. Management of ankle instability is essential before addressing secondary changes. While surgical treatment is recommended after failed conservative therapy, most …

Foot and Ankle Injuries in Baseball.

Foot and ankle injuries in baseball provide unique challenges to the provider. Most foot and ankle injuries (ie, ankle sprains and contusions) can be managed non-operatively using a similar approach as in other athletic populations, but a few unique mechanisms of injury in baseball are important to be aware of. While upper extremity injuries such as ulnar collateral ligament (UCL) tears and labral tears are extensively reported on in the literature, there is a need for further baseball-specific research on foot and ankle injuries, especially given the increasing prevalence of these injuries ov…

Orthotic Devices for the Foot and Ankle.

Millions of Americans wear ankle-foot orthotic devices for protection, pain relief, and deformity correction. Inquiries about off-the-shelf and custom devices are a common reason for evaluation with a foot and ankle surgeon or general orthopaedic surgeon. Despite limited high-quality evidence for their use, these devices can have a notable clinical impact on physical function. An up-to-date understanding of orthotic device options and their appropriate use in managing musculoskeletal pathologies applies to all orthopaedic providers. This review aims to categorize orthosis types and provide spe…

Osteochondral Lesions of the Ankle and Foot.

Osteochondral lesions (OCLs) in the ankle are more common than OCLs of the foot, but both share a similar imaging appearance. Knowledge of the various imaging modalities, as well as available surgical techniques, is important for radiologists. We discuss radiographs, ultrasonography, computed tomography, single-photon emission computed tomography/computed tomography, and magnetic resonance imaging to evaluate OCLs. In addition, various surgical techniques used to treat OCLs-debridement, retrograde drilling, microfracture, micronized cartilage-augmented microfracture, autografts, and allografts…

[Update: endoprosthetics on the ankle joint].

The ankle joint has to bear the entire body weight on a relatively small joint surface. Incongruities, instabilities and deformities lead to painful arthrosis and considerable restrictions in everyday life. For many years, arthrodesis has proven to be the gold standard for end stage arthrosis; however, considering modern endoprostheses for the ankle joint it is no longer appropriate to offer only arthrodesis. The very good results of the 3rd generation 3‑component prostheses and the 4th generation 2‑component prostheses show how much the surgical treatment options for the ankle joint are curre…

[Diagnostics and Therapy of Ankle Instability].

Diagnostics and Therapy of Ankle Instability Abstract. Ankle sprains are among the most common musculoskeletal injuries and therefore often treated in the emergency department or in the general practitioner's office. In the majority of cases, the lateral ligamentous complex is affected. If treated correctly, ligamentous ankle injuries have a good prognosis and in about 80% of cases full recovery can be achieved. Risk factors for the development of chronic ankle instability are an inappropriate treatment of the ankle sprain, injury of the deltoid ligament, hyperlaxity and rearfoot deformities (…

Postoperative MRI of the Ankle and Foot.

Many surgical procedures and operations are used to treat ankle and foot disorders. Radiography is the first-line imaging for postoperative surveillance and evaluation of pain and dysfunction. Computed tomography scans and MR imaging are used for further evaluation. MR imaging is the most accurate test for soft tissues assessments. MR imaging protocol adjustments include basic and advanced metal artifact reduction. We chose a surgical approach to select the common types of procedures and discuss the normal and abnormal postoperative MR imaging appearances, highlighting potential complications.…

[Ankle fractures in children and adolescents].

In children and adolescents, injuries of the ankle are seen frequently in routine practice. The fracture pattern is less dependent on the mechanism of injury than on the maturity of the growth plate. Therefore, a differentiation is made between fractures with open epiphyses and fractures with a closing growth plate in adolescents. The potential for growth-related spontaneous deformity correction is limited, so axial alignment especially in the frontal plane has to be achieved during initial treatment. In displaced articular fractures an anatomical reconstruction of the articular surface and st…

Foot and Ankle Injuries in Soccer.

The ankle is one of the most commonly injured joints in soccer and represents a significant cost to the healthcare system. The ligaments that stabilize the ankle joint determine its biomechanics-alterations of which result from various soccer-related injuries. Acute sprains are among the most common injury in soccer players and are generally treated conservatively, with emphasis placed on secondary prevention to reduce the risk for future sprains and progression to chronic ankle instability. Repetitive ankle injuries in soccer players may cause chronic ankle instability, which includes both me…

Management of syndesmotic injuries of the ankle.

Injuries to the tibioperoneal syndesmosis are more frequent than previously thought and their treatment is essential for the stability of the ankle mortise.Recognition of these lesions is essential to avoid long-term morbidity.Diagnosis often requires complete history, physical examination, weight-bearing radiographs and MRI.Treatment-oriented classification is mandatory. It is recommended that acute stable injuries are treated conservatively and unstable injuries surgically by syndesmotic screw fixation, suture-button dynamic fixation or direct repair of the anterior inferior tibiofibular liga…

[Osteoarthritis of the upper ankle joint].

The majority of cases of upper ankle joint (UAJ) osteoarthritis are due to secondary causes. Clinically, osteoarthritis is usually characterized by an increasing limitation in dorsal extension of the UAJ with often relatively mild symptoms. In the course of time the full scale of the typical symptoms and the progressive restriction of the global function of the joint develop. Conservative therapy is often able to provide long-term improvement of the symptoms for the majority of patients by means of intermittent analgesics and orthoses as well as shoe modifications. Operative treatment strategi…

Knee and Ankle Arthroplasty in Hemophilia.

Today, major surgical procedures can be safely performed in hemophilic patients with chronic arthropathy, using available factor concentrates. In this setting, total knee replacement is considered the "gold standard", while the use of total ankle replacement is still debated. Indeed, the unsatisfactory results obtained with the previous available design of implants did not raise enthusiasm as knee or hip replacement. Recently, the introduction of new implant designs and better reported outcomes have renewed the interest in total ankle replacement in people with hemophilia. In this review, the …
