Patterns of care and expenditures by California Medicaid for peptic ulcer and other acid-related diseases.

We examined California Medicaid (Medi-Cal) service use and expenditures for peptic ulcer and other acid-related diseases during the 10-year period fiscal year (FY) 1976-1985. Between FY 1976 and FY 1980, hospitalizations rose irregularly, operations declined by 37%, endoscopies tripled, physician visits fluctuated from year to year, antacid prescriptions increased by 36%, and anticholinergic medication use was stable. From FY 1981 to FY 1985, hospitalizations declined by 52%, operations by 91%, endoscopies by 45%, physician visits by 30%, antacid use by 12%, and anticholinergic medication use

[Laryngeal granuloma. Aetiology, clinical signs, diagnostic procedures, and treatment].

Vocal cord granulomas are benign inflammatory lesions of the vocal cords. They are usually located over the vocal process of the arytenoid c … necessary. The primary treatment is speech therapy or voice counselling, if necessary, supported by antacids. Surgical excision is not helpful because contact granulomas tend to recur. We present two typical cases of vocal cord granulomas and discuss their management.

Duodenal colonization by Geotrichum candidum in a child with transient low serum levels of IgA and IgM.

Fungal colonization was observed in the duodenal biopsy specimens of a nine-year-old girl who complained of anorexia and epigastric pain. Endoscopy revealed gastric redness and Geotrichum candidum was identified in the duodenum by histopathology and tissue culture. Immunologic work-up showed low serum levels of IgA and IgM. She was treated with ranitidine and antacids for gastritis. At follow-up, the levels of immunoglobulins had normalized, no fungal elements were detected, and clinical symptoms had disappeared. Duodenal invasion by G. candidum has not been described so far. Duodenal coloniza…

Cutaneous gangrene secondary to metastatic calcification in end stage renal failure--a case report.

Metastatic calcification is a frequent association of chronic renal failure but skin involvement is accepted as a rare, albeit well recognised, complication accompanying the secondary hyperparathyroidism that results from renal impairment, although the aetiology remains unknown. Skin involvement can take the form of metastatic calcinosis cutis or cutaneous gangrene resulting from vascular calcification. There have been reported cases which describe the healing of gangrenous areas following parathyroidectomy as well as use of dietary restriction of phosphorus and phosphate binding antacids(1-6)…

Gastroesophageal reflux disease: diagnosis and medical therapy.

Gastroesophageal reflux disease (GERD) is a common clinical condition that affects all age groups, particularly older adults. Its presentati … of esophagitis. All patients should be instructed to make dietary and other lifestyle adjustments. Antacids may be needed for mild symptoms. Pharmacologic options for moderately severe cases include H2 blockers, a proton pump inhibitor, or others. Surgery is reserved for cases of severe refractory disease.

Seronegative juvenile rheumatoid arthritis and mast cell-associated gastritis.

We describe 4 children with seronegative inflammatory arthritis who had persistent, severe nausea and abdominal pain over several months, in spite of vigorous medical therapy, including antacids and histamine H2 receptor antagonists. Endoscopy and biopsy of gastric and duodenal mucosa showed antral gastritis and an increased number of mast cells in 3 of the 4 patients. In the fourth patient, urinary histamine levels were elevated. These findings suggest an association between inflammatory arthritis and localized mast cell disease in some individuals. Further studies are needed to determine whe…

[Combination of glandular and foveolar hyperplasia of the gastric mucosa--a rare mixed type].

In a 28 year-old man with epigastric pain endoscopy detected giant gastric folds and multiple superficial erosions. Histologically a mixed form of glandular and foveolar hyper plasia of the gastric mucosa was suggested (Ming's type III). Basal and pentagastrin stimulated secretion volume and acid output were moderately elevated, hypersecretion of protein was not found. Serum levels of calcium and gastrin were normal, also after secretin stimulation. The mucosa of the corpus was extensively infested with Campylobacter pylori. Therapy with cimetidine, antacids, pirenzepine and metronidazol resul…

Common mistakes in managing metabolic disorders.

Three mistakes are commonly made in managing metabolic disorders. 1. Oral fluids may be pushed to treat simple volume depletion. However, almost all fluids used for this purpose are sodium-poor and do not restore salt and water balance. 2. The physician may not be aware of common causes of hypophosphatemia, such as hyperventilation, sepsis, stress, use of antacids or diuretics, and alcoholism. If the patient is not monitored adequately, severe hypophosphatemia may develop with serious consequences. 3. Low serum bicarbonate levels may be attributed to metabolic acidosis only, when in actuality …

Complications associated with the adult respiratory distress syndrome.

The adult respiratory distress syndrome is frequently fatal. Attempts to decrease mortality must include attention to pulmonary and extrapulmonary complications of this syndrome. Pulmonary complications include pulmonary emboli, barotrauma, fibrosis, and pneumonia. Mechanical ventilation, Swan-Ganz catheters, and tracheal intubation also result in pulmonary complications. Extrapulmonary complications such as gastrointestinal hemorrhage, renal failure, infection, and thrombocytopenia may increase mortality. The incidence of gastrointestinal hemorrhage can be reduced with prophylactic antacids. …

Peptic ulceration in kidney transplantation.

Gastroduodenal ulceration occurred in 45 patients during the post-transplantation period in a series of 500 transplantations of 434 patients. The mortality rate of this complication was high, 42%. Bleeding and perforation were the main problems. These complications occurred frequently during treatment for acute rejection. Present day prophylaxis, which is based on the use of antacids, seems to be inadequate for controlling these complications. Other possibilities for reducing the incidence of gastroduodenal ulceration in transplant patients are discussed. Since increased serum gastrin concentr…

Gastroesophageal reflux disease (GERD) is one of the most frequently encountered illnesses in the Western Hemisphere. GERD encompasses a spe … e continuously evolving, but as yet no drug has been shown to cure this chronic, relapsing disease. Antacids, prokinetics, and gastric antisecretory agents are the principal drugs currently used to treat GERD in conjunction with life-style modifications. Due to their ultrashort duration of buffering action, antacids are primarily used as self-medication fo …

[Superoxide-removing activity of gastric juice in gastroduodenal diseases].

A decrease in the superoxide anion-obviating activity of gastric juice was detected in patients with chronic gastroduodenitis and duodenal ulcerous disease, whose rate depended on the disease stage, functional state of the stomach and nature of the disease course. Deficiency in H+, Cu2+ and a decrease of glutathione content in gastric juice were found to be responsible for the phenomenon observed. The superoxide anion-obviating activity of gastric juice exhibited the regulatory effect on resistance of the duodenal mucosal membrane, which is prerequisite to restoration of the pattern studied du

Peptic (contact ulcer) granuloma of the larynx.

Review of published work and analysis of clinical data and pathology of four biopsy specimens from two patients with laryngeal contact granuloma showed that its peptic origin was derived from a gastro-oesophago-laryngeal reflux. It is proposed that the term "peptic granuloma" should be given to this phenomenon. This term is given further support on account of the spectacular recovery of the laryngeal lesion following antacid and antireflux treatment, rather than the traditional method of using vocal rest and speech therapy, assumed to be the best way of treating a result of mechanical irritati

Oral antibiotic drug interactions of clinical significance to dermatologists.

Oral antibiotics are commonly prescribed by dermatologists in clinical practice. When prescribing an oral antibiotic, as with other systemic medications, it is important to consider potential interactions with other drugs, including over-the-counter medications. The most common drug interaction mechanisms that may lead to clinically significant sequelae are inhibition of GI drug absorption and alterations in drug metabolism. Tetracycline and quinolones undergo chelation interactions with many metal ions found in antacids and mineral supplements. Some macrolides, such as erythromycin, inhibit t…
