Risk of anorectal cancer in patients with Crohn's disease and perianal fistula: a nationwide Danish cohort study.

AIM: Patients with Crohn's disease (CD) often suffer from perianal fistulizing disease. Their risk of anorectal cancer remains uncertain. We aimed to examine the long-term risk of anorectal cancer in a population-based cohort of CD patients with anorectal fistula. METHOD: Our study population covered all individuals (n = 7 987 520) aged 15+ years living in Denmark from 1978 to 2018. We identified all patients with CD and anorectal fistula in the Danish National Patient Register (NPR) and 50 matched noninflammatory bowel disease (IBD) individuals from the general population. Using Cox regressi

Determination of Oncogenic Human Papillomavirus (HPV) Genotypes in Anogenital Cancers in Myanmar.

Molecular and epidemiologic investigations suggest a causal role for human papillomavirus (HPV) in anogenital cancers. This study identified oncogenic HPV genotypes in anogenital cancers among men and women in a 2013 cross-sectional descriptive study in Myanmar. In total, 100 biopsy tissues of histologically confirmed anogenital cancers collected in 2008-2012 were studied, including 30 penile and 9 anal cancers from Yangon General Hospital and 61 vulvar cancers from Central Women's Hospital, Yangon. HPV-DNA testing and genotyping were performed by polymerase chain reaction-restriction fragment

Perianal apocrine adenocarcinoma arising in a benign apocrine adenoma; first case report and review of the literature.

Benign apocrine lesions have been described in the anogenital region, although according to the World Health Organisation convincing examples of anal apocrine adenocarcinomas have not been published. This report describes the case of an invasive apocrine adenocarcinoma arising in a benign adenoma in the perianal region of a 45 year old woman. The origin and invasiveness are supported by histological and immunohistochemical studies.

A case of rectosigmoid cancer metastasizing to a fistula in ano.

We herein report a case of rectosigmoid cancer metastasizing to a fistula in ano. A 53-year-old man complaining of anal bleeding consulted another hospital. He had been suffering from an anal fistula since 7 years. On the left upper side of the skin surface around the anus a fistula end was seen as a hole that tunneled down into the back passage, although no hard tumor was palpable on the hole. Complete colonoscopy revealed an ulcerative tumor in the rectosigmoid colon. On 5 February 2004, anterior resection and lymphadenectomy was performed. The post-operative pathological diagnosis was recto

Anal malignant melanoma and soft-tissue malignant fibrous histiocytoma in neurofibromatosis type 1.

A 48-year-old man with the nonfamilial form of neurofibromatosis type 1 (NF-1) developed a malignant fibrous histiocytoma in the deep soft tissue of the calf. Following excision and local radiotherapy, the tumor did not recur. At age 59, due to rectal bleeding, a polypoid malignant melanoma of the anal canal was detected and resected. One year later a local recurrence was removed, a pelvic mass was seen on computed tomography, and the patient died. The malignant melanoma was amelanotic and negative for HMB-45 (a melanoma-specific antibody), and the recurrent tumor showed areas of desmoplasia a

Neurologic complications of cloacogenic carcinoma.

Cloacogenic carcinoma is a rare tumor, originating from epithelium of the anal transition zone. We report a 63-yr-old man with pathologically proven cloacogenic carcinoma which caused a rapidly progressive paraparesis and changes in mental status. These were related to extramedullary deposits around the spinal cord and cauda equina and intramedullary deposits in the brain. This pattern of neural involvement with anal canal carcinoma has not been reported previously.

[Condylomata acuminata].

A review of the recent literature concerning the C.A. has been made. Two major facts appear to be confirmed: the rising incidence of the disease for adults as in other S.T.D. and the oncogenicity of some of the HPV's (Human Papillomavirus) isolated from the C.A.: HPV 16, 18, 31, 33 and 35. A peculiar attention has been paid to the para-clinical evaluation of the patient and his partner. The different therapeutic modalities are reviewed: their choice should be based on duration, spreading and localization of the lesions, age and immunological status of the patient. The problem of treating condy

[Melanoma of the anorectum].

The therapy of choice in cases with the prognostically poor (mean 5-year-survival rate maximal 12%) and rare anorectal melanoma is the surgical removal. Primary local excision should be done if technically feasible. On the basis of an exact pathohistological examination with special respect to tumor thickness a radical exstirpation or resection should be performed in cases with lesions more than 2 mm thick. In patients with tumors less than 2 mm a clinically and endoscopically follow-up in short intervals (3 months) after local excision can be recommended. At present, no effective radiotherapy

Lymphoscintigraphy for malignant melanoma. Surgical considerations.

Lymphoscintigraphy using technetium-99m atimony sulfur colloid was performed in 22 patients with melanoma referred to Duke University Comprehensive Cancer Center in an attempt to identify patterns of regional lymphatic drainage. Scans from six patients revealed lower extremity lesions with three located below the knee, one in the popliteal space, and two others proximal to the knee joint. Despite the location of the primary, all scans but one showed initial drainage to the inguinal nodes bypassing the popliteal lymph node group. Of the three patients who had primary melanoma of the posterior s

[Multiple basaliomas in pityriasis rubra pilaris].

This is a report of a 73 year old patient with pityriasis rubra pilaris and multiple basal cell carcinomas which especially appeared on the face. One of the tumors was located in the anal and perianal region. This is the first report of basal cell carcinomas in a patient with pityriasis rubra pilaris. Further observations will clarify whether there exists a closer relationship between the presence of basal cell carcinoma and pityriasis rubra pilaris. Therefore more attention should be paid to the existence of basal cell carcinoma in patients with pityriasis rubra pilaris.

Divergent immune ecosystems in HPV-associated and HPV-independent lower anogenital tract malignancies.

Lower anogenital tract malignancies comprise anatomically adjacent but biologically heterogeneous epithelial cancers of the cervix, vulva, vagina, anal canal and penis. This narrative and conceptual Review used a transparent literature search to examine how human papillomavirus (HPV) etiology intersects with tumor lineage, somatic alterations, antigen-presentation competence, immune-cell state and spatial organization. Evidence quality is heterogeneous: cervical cancer is supported by the largest genomic, single-cell, spatial and randomized-trial literature, whereas vaginal, vulvar and penile

Recto-anal high-grade squamous intraepithelial lesion with 5-year stability: curative endoscopic submucosal dissection.

High-grade squamous intraepithelial lesion (HSIL) is a recognized precursor of anal squamous cell carcinoma (SCC). Randomized controlled trials in human immunodeficiency virus (HIV)-positive individuals have shown that local treatment of HSIL reduces the risk of progression to SCC, but the natural history of HSIL in HIV-negative patients is not well defined. We describe a rare case of recto-anal HSIL in an HIV-negative patient that remained morphologically stable for 5 years. Endoscopically, the lesion measured 33 × 25 mm and extended from the lower rectum (Rb) to the anal canal. The lesion wa

Optimal approaches to flap reconstruction following abdominoperineal resection: A systematic review.

PURPOSE: Abdominoperineal resection (APR) frequently results in a large volume perineal defect. Flap reconstruction is commonly undertaken to reduce the rate of perineal complications associated with primary closure. Several techniques can be employed including vertical rectus abdominis (VRAM), gluteal myocutaneous and gluteal fasciocutaneous flaps. We aimed to compare perineal complication rates between flap reconstruction techniques. METHODS: A systematic review was conducted following PRISMA guidelines. Databases were searched for studies reporting perineal complications following flap rec

Metachronous splenic metastasis from squamous cell carcinoma of the anus.

A woman in her 70s with a history of metastatic squamous cell carcinoma (SCC) of the anus in systemic remission presented with a new splenic metastasis on surveillance CT imaging consistent with the primary site of the anus as confirmed by histology. The patient consented to surgical management via laparoscopic splenectomy, which was facilitated by a combination of laparoscopic and hand-assist ports. Mobilisation of the spleen revealed local tumour invasion into the diaphragm, requiring resection and resulting in capnothorax, which was appropriately repaired. Final pathology showed a well-circ

Long-term outcomes analysis of flap-based perineal reconstruction.

BACKGROUND: High-risk patients undergoing abdominoperineal resection and pelvic exenteration may benefit from immediate flap reconstruction. However, there is currently no consensus on the ideal flap choice or patient for whom this is necessary. This study aimed to evaluate the long-term outcomes of using pedicled gracilis flaps for pelvic reconstruction and to analyze predictors of postoperative complications. METHODS: This was a retrospective review of a single reconstructive surgeon's cases between January 2012 and June 2021 identifying patients who underwent perineal reconstruction second

Failure of Initial Curative Treatment for Non-Metastatic Anal Squamous Cell Carcinoma: From Prognostic Factors Analysis to Stratified Treatment.

In squamous cell anal canal neoplasms, persistent disease or recurrence after initial chemoradiotherapy are not the rule, yet their occurrence deserves to be analyzed to better identify prognostics factors. The aim of our study was to describe the patterns of failures of the initial treatment, their subsequent evolution and to identify prognostic factors in these relapsed patients. All patients with non-metastatic anal squamous cell carcinoma initially treated with curative intent at the Centre Antoine Lacassagne between 1999 and 2019, and who presented persistent disease or recurrence were an

Optimal surgical management strategy for treatment of primary anorectal malignant melanoma-a systematic review and meta-analysis.

BACKGROUND: Primary mucosal anorectal malignant melanoma (AMM) is an invasive malignancy with poor survival. Management options have been variable, due to limited data and lack of randomised control trials available on the optimal surgical strategy. The aim of this review was to compare local excision versus radical resection. METHODS: A systematic search of articles in PubMed, Ovid, Scopus, and the Cochrane Library database was performed according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The outcomes of interest were the impact that surgi

[A Case Report of Perianal Paget's Disease Treated with Robot-Assisted Surgery for Positive Margins after Local Excision].

The patient is a 73-year-old man who was diagnosed with perianal Paget's disease by skin biopsy. Biopsy from the dentate line did not show any tumor cells. The patient was considered to undergo sphincter-preserving local resection and subsequently underwent the procedure. Histopathological examination of the resected specimen revealed perianal Paget's disease with a positive anorectal margin. The patient was referred to our department due to postoperative anal stenosis. On the 32nd postoperative day, a double barreled sigmoid colostomy was performed. However, considering the inability to adequ
