Six human epikeratophakia lenticules were removed 1 to 23 months after refractive keratoplasty for aphakia (n = 2) and myopia (n = 4). The myopic group included one epi-lenticule treated with ArF-excimer laser for hyperopia. Reasons for removal were epithelial defect (n = 4), edema of host cornea (n = 1), and overcorrection (n = 1). Epikeratophakia is a new refractive corneal surgery since 1980. The authors treated 30 cases of unilateral aphakia with corneal lenses prepared by a self-made cryolathe. After a mean postoperative follow-up of 161 days, all patients obtained visual improvement. 19 eyes (63.3%) achieved uncorrected visual acuity over 0.5, and 5 eyes (16.7%) were 1.0 or over. 8 eyes (26.7%) were corrected to within 1 diopter by the surgery, and the mean postoperative corneal astigmatism was 2.53 diopters. Presented was the comparison of early and late complications arising after implantation of the anterior chamber lenses of the type Kelman Omnifit and Pannu. The period of postoperative observations amounted 1-3 years. It was demonstrated that the Pannu lenses are giving less complications; the authors are connecting this fact with an easier operative technique of their implantation and their greater ability of autoadaptation.

The authors analyze the problem of intraocular correction in patients with penetrating missile wounds of the eyeball. A total of 292 patients were examined; besides clinical, electrophysiologic and ultrasonic methods were used, as well as endothelial microscopy. The case reports are given of two adult patients who lost all fusion ability and had the signs and symptoms of central fusion disruption and intractable diplopia. Both patients suffered binocular sensory deprivation for several years caused by a combination of a traumatic cataract and uncorrected unilateral aphakia. Seventeen patients, ranging in age from 40 to 86 years, developed pupillary block detected one week to three years after intraocular lens implantation. Spectrophotometric measurements indicated that polymethyl methacrylate intraocular lenses do not mimic the crystalline lens in absorption of shortwave visible light and near-UV light. These wavelengths are particularly effective in producing photochemical damage to the retina. Scotopic spectral sensitivity functions directly confirmed that patients with intraocular lens implants are much more sensitive than normal to these wavelengths (by as much as a factor of 46 at 380 nm).

An assessment of the quality of care being provided to meet the needs of postcataract aphakes for refractive correction is presented. The procedure used was a review of patient records originally developed for inpatient care but was here applied to outpatient care in a centralized health facility. This paper demonstrates the feasibility and value of this method of quality assessment.

The series of cases discussed in this chapter consisted of 135 consecutive cases performed in 120 patients over a two-year period. Mechanically damaged ciliary epithelium in aphakic and pseudophakic rabbit eyes was studied for up to 2 years after extracapsular lens extraction. Histopathological examination of the epithelium revealed more severe damage in pseudophakic eyes.
