Delayed Appendectomy Versus Early Intervention: A Systematic Review of Outcomes in Complicated Appendicitis.

The management of complicated appendicitis remains a subject of debate, particularly concerning the optimal timing of surgical intervention. While early appendectomy has been the traditional approach, there is growing clinical interest in delayed surgery with initial antibiotic therapy. The existing evidence, however, presents conflicting outcomes, creating uncertainty for surgeons. Therefore, the rationale for this study is to synthesize the available evidence to provide a clearer understanding of the comparative outcomes of early versus delayed surgical strategies. This systematic review aim…

Robotic right hemicolectomy for persistent colocutaneous fistula following laparoscopic appendectomy: A case report.

The article presents a case report of a patient who was referred to our hospital after laparoscopic appendectomy with several revisions, resulting in a persistent colocutaneous fistula to the hepatic flexure. After preoperative planning, a robotic right hemicolectomy was performed. The patient was discharged on postoperative day 7 with no postoperative complications. We concluded that robot-assisted colectomy after preoperative patient management can be performed as an effective technique for colocutaneous fistula treatment with the benefits of minimal invasive surgery, including reduced posto…

Appendectomy and appendicitis do not increase colorectal cancer risk: evidence from Mendelian randomization.

BACKGROUND: Acute appendicitis (AA) is one of the most prevalent acute abdominal diseases and appendectomy is the definitive treatment of appendicitis. However, whether appendicitis and appendectomy cause colorectal cancer (CRC) is controversial. The results of observational studies are contradictory, but randomized controlled trials (RCT) cannot be conducted. METHODS: Data of appendectomy, AA, and CRC were obtained from the IEU Open GWAS project. We selected several Genome-wide association studies (GWAS) summary statistics for CRC: statistics for colon cancer (CC) were obtained from MRC-IEU …

Surgical Complications Following Appendectomy in Obese Patients: A Single Tertiary Care Center Study.

Background The escalating global prevalence of obesity raises concerns about its implications for health outcomes. While obesity is acknowledged as a major risk factor for various diseases, its impact on appendicitis and appendectomy outcomes remains less explored. Methods Data on overweight and obese adults aged 18 to 65 treated for appendicitis in King Abdulaziz Medical City in Riyadh, Saudi Arabia were collected retrospectively. The sample included 1,471 patients who underwent laparoscopic appendectomy between January 2015 and January 2022. The sample did not include patients who were outsi…

Trends and Predictors of Pediatric Negative Appendectomy Rates: A Single-Centre Retrospective Study.

BACKGROUND: Appendectomy is still the standard treatment for acute appendicitis in the majority of centers. Despite all available diagnostic tools, the rates of negative appendectomies are still relatively high. This study aimed to determine negative appendectomy rates and to analyze the demographic and clinical data of the patients whose histopathology report was negative. METHODS: All patients younger than 18 years who underwent appendectomy for suspected acute appendicitis in the period from 1 January 2012 to 31 December 2021 were included in the single-center retrospective study. Electron…

Postoperative organ space infection (OSI) following appendectomy: early term evaluation for pediatric population.

No consensus has been reached on the duration of antibiotic prophylaxis for postoperative organ space infection (OSI) following appendectomy. This study investigated the influence of antibiotic administration on postoperative OSIs in children with complex appendicitis. A multicenter, retrospective study was conducted in patients with OSI following complicated appendicitis between 2017 and 2019 at 3 hospitals in China. The qualified patients were dichotomized into a long-duration antibiotic group (> 5.5 days) and a short-duration antibiotic group (< 5.5 days) based on the median duration (5.5 d…

"Misra's appendicular hitch" stich - Innovative technique for simplifying endo-suturing during laparoscopic appendectomy.

Laparoscopic suture transfixation and free tie ligation are the most commonly used cost-effective technique of managing the base of the appendix during laparoscopic appendectomy in low resource settings such as India. This becomes technically cumbersome especially in the presence of the long friable appendix and for novice surgeons. We hereby describe an innovative technique of suspending the appendix using a transfacial suture to ease the placement of suture at the base of the appendix during laparoscopic appendectomy.

Identification of New Potential Therapies for Colitis Amelioration Using an Appendicitis-Appendectomy Model.

The appendix contains copious lymphoid tissue and is constantly exposed to gut flora. Appendicitis followed by appendectomy (AA), when done at a young age, prevents or significantly ameliorates inflammatory bowel diseases (IBDs) in later life. Inflammatory bowel disease comprises Crohn's disease and ulcerative colitis. Our unique murine AA model is the only existing experimental model of AA. Herein, the appendiceal pathology closely resembles the pathological features of human appendicitis. Our AA model protects against experimental colitis in an age-, bacteria- and antigen-dependent manner. A…

Volvulus caused by a free intraperitoneal staple after laparoscopic appendectomy: A case report.

INTRODUCTION: Laparoscopic appendectomies are routinely performed using linear staplers. Few case reports have discussed complications from free intraperitoneal staples after appendectomy. We present the first case of a volvulus caused by a free staple that subsequently required bowel resection. PRESENTATION OF CASE: A 27-year-old female underwent laparoscopic appendectomy for uncomplicated appendicitis. The base of the appendix was divided using a laparoscopic gastrointestinal anastomosis (GIA) stapler and the mesoappendix was divided using a LigaSure device. The patient was discharged the f…

Laparoscopic versus open appendectomy in pediatric patients with complicated appendicitis: a meta-analysis.

BACKGROUND: Acute appendicitis is a common condition in the pediatric population. In patients with uncomplicated appendicitis, laparoscopic appendectomy (LA) is preferred as compared to open appendectomy (OA). However, in patients with complicated appendicitis (CA), as defined as suppurative, gangrenous or perforated appendicitis, or appendicitis with periappendicular abscess formation, the decision to perform OA or LA remains unclear. METHODS: The PRISMA guidelines were adhered to. An electronic database search from 1997 to 2017 was performed using the Cochrane, Medline, PubMed, Scopus, Ovid…

Abdominal drainage to prevent intra-peritoneal abscess after open appendectomy for complicated appendicitis.

BACKGROUND: Appendectomy, the surgical removal of the appendix, is performed primarily for acute appendicitis. Patients who undergo appendectomy for complicated appendicitis, defined as gangrenous or perforated appendicitis, are more likely to suffer from postoperative complications. The routine use of abdominal drainage to reduce postoperative complications after appendectomy for complicated appendicitis is controversial. This is an update of the review first published in 2015. OBJECTIVES: To assess the safety and efficacy of abdominal drainage to prevent intra-peritoneal abscess after open a…

Appendix extraction after laparoscopic appendectomy in children: An easy, safe, and inexpensive technique.

BACKGROUND: Laparoscopic appendectomy is becoming an increasingly common operation among children. It is well known that the abdominal wall should not be exposed to the appendix's inflamed tissue during its removal. Several ways to protect the retrieval site have been described, with controversial results. Elevated costs, large surgical wounds and high contamination risks are typical reported issues. OBJECTIVE: We describe a safe, cheap and easy technique to remove the appendix after a laparoscopic appendectomy in pediatric patients. METHODS: We retrospectively collected and analyzed the fil…

Faecolith migrating from the appendix to produce liver abscess after subhepatic laparoscopic appendectomy.

The subhepatic position of an appendix may lead to difficulty in diagnosis of appendicitis, its surgical management as well as bizarre complications. A 'lost' appendicular faecolith is not an uncommon complication due to spillage during the removal of the appendix or due to perforation. Here we report a case of such a faecolith migrating into the liver parenchyma following intraoperative hepatic injury, and producing a liver abscess. The calcified lesion seen in the liver corresponded to a free-lying faecolith on a preoperative computed tomography scan. Liver abscess was presumed to arise from
