Supported arm training in patients recently weaned from mechanical ventilation.

STUDY OBJECTIVES: To evaluate the effects of early exercise training in patients recovering from acute respiratory failure needing mechanical ventilation (MV). DESIGN: Prospective, randomized, and controlled study. SETTING: Three respiratory intermediate ICUs (RIICUs). PATIENTS: Of 228 patients admitted to an RIICU, 66 patients weaned from MV from > 48 to < 96 h were considered eligible and enrolled in the study. INTERVENTION: Sixty-six patients were randomized to either supported arm exercise training plus general physiotherapy (gPT) [group 1, 32 patients] or to gPT alone (group 2, 34 pat…

Radiation exposure to the hands from mini C-arm fluoroscopy.

PURPOSE: To quantify the level of radiation exposure to the hands of hand surgeons using intraoperative mini C-arm fluoroscopy and to compare the actual level of exposure with predicted levels and acceptable limits. METHODS: Five hand surgeons were given ring dosimeters to measure radiation exposure to their hands during surgery of the finger, hand, and wrist. A total of 81 rings were analyzed. After the clinical study a phantom was used to measure scatter at close range from the mini C-arm. RESULTS: Surgeons' hands were exposed to an average +/- SD of 20 +/- 12.3 mrem/case. For comparison a…

Radiation exposure in hand surgery: mini versus standard C-arm.

PURPOSE: The use of intraoperative fluoroscopy in hand surgery is common. Two types of fluoroscopic units are available: the mini C-arm and the standard C-arm. There is little literature on the radiation exposure from the mini C-arm, therefore, the primary goal of this study was to quantify and compare the amount of radiation exposure to members of the surgical team (surgeon, first assistant, nurse, anesthesiologist) using both standard and mini C-arms in a simulated wrist surgery setup. Mini C-arm positioning was also examined to determine the safest configuration to minimize radiation exposu…

Monitoring functional arm movement for home-based therapy after stroke.

The goal of this project is to develop a means for individuals with stroke to practice arm movement therapy at home with remote monitoring. We previously developed a Web-based system for repetitive movement training (Java Therapy). This paper describes a new input device for the system that measures and assists in naturalistic arm movement, as well as software enhancements. The new input device is an instrumented, adult-sized version of Wilmington robotic exoskeleton (WREX), which is a five degrees-of-freedom orthosis that counterbalances the weight of the arm using elastic bands. To test the …

[Evidence-based arm rehabilitation--a systematic review of the literature].

Based on a systematic MEDLINE search and informal sources, 40 references were identified that evaluate training therapy or neuromuscular electric stimulation for arm paresis after stroke and describe either a systematic review, meta-analysis, randomised controlled trial, or controlled cohort study. The evidence was grouped into three areas of interest: comparison of physiotherapy schools, effects of intensity of training, and efficacy of specific arm rehabilitation techniques. The only physiotherapy school with evidence of superior efficacy was the task-oriented 'motor relearning programme'. H…

Both body and arm position significantly influence blood pressure measurement.

The position of both the body and the arm during indirect blood pressure (BP) measurement is often neglected. The aim of the present study was to test the influence of the position of the patient on BP readings: (1) sitting with the arms supported precisely at the right atrium level and (2) supine: (a) with the arms precisely at the right atrium level and (b) with the arms on the examination bed. In a first group of 57 hypertensive patients, two sessions of BP and heart rate (HR) measurements were performed in two positions: sitting and supine with the arms supported precisely at right atrium …

Impact of neck and arm pain on overall health status.

STUDY DESIGN: A prospective, multicenter, cross-sectional analysis of data from the National Spine Network database. OBJECTIVES: To compare the relative impact of radicular and axial symptoms associated with disease of the cervical spine on general health as measured by the SF-36 Health Survey, and to compare the impact of these symptoms among patients of varying age and symptom duration. BACKGROUND: Degenerative disorders of the cervical spine can cause debilitating symptoms of neck and arm pain. Physicians generally treat radiculopathy more aggressively than axial neck pain alone, although…

Towards epidemiological criteria for soft-tissue disorders of the arm.

BACKGROUND: The lack of universally agreed criteria has hampered population studies of the prevalence and causation of soft-tissue disorders of the upper limb. OBJECTIVES: To establish core variables for classification of the commonest disorders seen in population samples. METHODS: Consecutive new cases seen in clinical practice in five different centres were evaluated with respect to 30 variables shown to have discriminatory value in univariate analysis. Multivariate analysis using logistic regression modelling was carried out with these as the independent variables and with the clinical di

Diagnosis and neuroimaging of acute stroke producing distal arm monoparesis.

Strokes which result in the isolated, pure motor weakness of an upper extremity are unusual and under-recognized cerebrovascular syndromes. Few reports in the literature describe the syndrome adequately or provide substantive clinical or anatomical correlation. Moreover, it may be misdiagnosed as a disorder of the peripheral nervous system because of the lack of pyramidal tract signs or the involvement of speech, the face, or lower limbs. We describe 35 patients who presented with sudden isolated pure motor weakness of an arm or hand caused by stroke, and provide clinical anatomic correlation …

Isometric arm counter-pressure maneuvers to abort impending vasovagal syncope.

OBJECTIVES: We hypothesized that isometric arm exercises were able to increase blood pressure (BP) during the phase of impending vasovagal syncope and allow the patient to avoid losing consciousness. BACKGROUND: Hypotension is always present during the prodromal phase of vasovagal syncope. METHODS: We evaluated the effect of handgrip (HG) and arm-tensing in 19 patients affected by tilt-induced vasovagal syncope. The study consisted of an acute single-blind, placebo-controlled, randomized, cross-over tilt-table efficacy study and a clinical follow-up feasibility study. RESULTS: In the acute …

A descriptive study of women injured by hand-arm vibration.

The aim of this study was to describe the symptoms and the prognosis of vibration injuries in women. The investigation was based on a study of 374 women who had reported an injury due to hand-arm vibration to the Social Insurance Office or had received financial compensation from the Swedish Labor Market Insurance scheme during 1988-1997. Information on, for example, self-rated health symptoms and vibration exposure was collected by means of a questionnaire. On average, the first symptoms started after 7 yr of exposure and the first visit to a doctor took place after 11 yr. Neurological sympto…

Hand-arm vibration syndrome from exposure to high-pressure hoses.

Hand-arm vibration syndrome has been reported in the literature to occur following exposure to vibration from the use of many tools, but to date there have been no case reports of its occurrence in workers who have used high-pressure hoses, alone or with other tools. To remedy this, the case histories of nine subjects (two without mixed exposure) examined in the UK and Canada are presented, together with their severity classified according to the Stockholm scales. Attention is drawn to the need to use multiple diagnostic tests to establish the diagnosis and the need to implement vibration isol…

Model-based development of neuroprostheses for restoring proximal arm function.

Neuroprostheses with the use of functional neuromuscular stimulation (FNS) have the potential to restore elbow and shoulder function lost to paralysis because of spinal cord injury (SCI). The human shoulder is highly flexible and thus provides a large range of motion to the arm and hand, although at the expense of precarious stability of the articulations. The complexity of the shoulder has prevented widespread use of FNS at this joint. However, musculoskeletal modeling of the elbow and shoulder has the potential to significantly speed the development of neuroprostheses by allowing many mechan…

Salvage of ischemic digits using a lateral arm fascial flap.

Four patients underwent microvascular transfer of a lateral arm fascial flap to salvage severely ischemic digits by means of induction of neovascularization. The cause of the digital ischemia was direct trauma (crush injury) in one patient and chronic embolic phenomena (proximal arterial occlusion) in three patients. None of the patients had responded to traditional therapy, including treatment with one or more of the following: anticoagulation, lytic therapy, oral vasodilators, digital sympathectomy, and vein bypass grafting. Each patient underwent noninvasive (Doppler ultrasound, digital pre…

Modulation of human cutaneous reflexes during rhythmic cyclical arm movement.

The organization and pattern of cutaneous reflex modulation is unknown during rhythmic cyclical movements of the human upper limbs. On the assumption that these cyclic arm movements are central pattern generator (CPG) driven as has been suggested for leg movements such as walking, we hypothesized that cutaneous reflex amplitude would be independent of electromyographic (EMG) muscle activation level during rhythmic arm movement (phase-dependent modulation, as is often the case in the lower limb during locomotion). EMG was recorded from eight muscles crossing the human shoulder, elbow, and wrist…
