The effects of gracilis muscle transplantation on speech in children with Moebius syndrome.

Speech and communication are major problems for children with Moebius syndrome, a congenital anomaly that includes facial and abducens nerve paralysis and, often, other cranial nerve deficits. In addition, these children frequently have severe functional problems such as drooling as well as poor self-esteem. The purpose of this study was to investigate the outcome of speech with bilateral gracilis muscle transplants innervated by the masseteric nerve in children with Moebius syndrome. The outcome of this two-stage procedure was investigated in 12 patients. Assessments were done before and afte

The effect of target-selection strategy on phonological learning.

In this study, 48 children with moderate or severe delays in phonological ability received treatment for four phonemes, selected in accordance with either traditional or nontraditional target-selection criteria. Children who received treatment for phonemes that are early developing and associated with greater productive phonological knowledge showed greater progress toward acquisition of the target sounds than did children who received treatment for late-developing phonemes that were associated with little or no productive phonological knowledge. Between-group differences in generalization lea

A multidimensional view of gradient change in velar acquisition in three-year-olds receiving phonological treatment.

The acquisition of phonemes does not occur in an "all or nothing" manner; instead, children gradually acquire dimensions of phonological knowledge. This gradual acquisition of phonemes is explored in the present study by comparing three types of measures taken from speech samples of three preschool-aged girls with a Speech Sound Disorder. The process of acquisition of velar stops was measured during 16 weeks of Cycles based speech treatment. Three types of measures were used to study the gradual acquisition of velar stops: acoustic analyses using voice onset time (VOT) for initial consonants a

Investigating the use of traditional and spectral biofeedback approaches to intervention for /r/ misarticulation.

PURPOSE: Misarticulation of /r/ is among the most challenging developmental speech errors to remediate. Case studies suggest that visual biofeedback treatment can establish perceptually accurate /r/ in clients who have not responded to traditional treatments. This investigation studied the response of children with persistent /r/ misarticulation to a course of traditional treatment and a course of biofeedback treatment. METHOD: Eleven children with /r/ misarticulation completed 10 weeks of individual treatment consisting of 4-6 weeks of traditional treatment followed by 4-6 weeks of biofeedba

Intra-word inconsistency in apraxic Hebrew-speaking children.

Intra-word inconsistency in a child is perceived as an indicator of speech impairment. Because the speech of typically developing children is highly variable, the extent and nature of the inconsistency must be defined when used as a diagnostic marker of speech impairment (McLeod, S., & Hewett, S. R. (2008). Variability in the production of words containing consonant clusters by typical 2- and 3-year-old children. Folia Phoniatrica et Logopaedica, 60(4), 163-172). In this paper, we study inconsistency with reference to the prosodic hierarchy (McCarthy, J. J., & Prince, A. S. (1996). Prosodic mo

The influence of psycholinguistic variables on articulatory errors in naming in progressive motor speech degeneration.

We describe an analysis of speech errors on a confrontation naming task in a man with progressive speech degeneration of 10-year duration from Pick's disease. C.S. had a progressive non-fluent aphasia together with a motor speech impairment and early assessment indicated some naming impairments. There was also an absence of significant phonological or semantic impairment. In order to examine naming difficulties and the factors that influence his speech production errors, we selected 210 words varying in frequency, age of acquisition (AoA), imageability, phonemic length and syllable length and

Enhancement of electrolarynx speech based on auditory masking.

Electrolarynx (EL) speech provides a valuable means of verbal communication for the laryngectomees. Yet EL speech tends to be less intelligible speech due to the presence of background noise. This paper addresses the issue of EL speech enhancement. The proposed approach takes into account the frequency-domain masking properties of the human auditory system for a subtractive-type enhancement process. Subtractive-type algorithms can efficiently reduce the radiated noise of EL speech but not to reduce the additive noise from the environment due to the use of fixed subtraction parameters. Consider

Treatment decisions for children with speech-sound disorders.

PURPOSE: In this article, I consider how research, clinical expertise, client values, a clinician's theoretical perspective, and service delivery considerations affect the decisions that clinicians make to treat children with speech-sound disorders (SSD). METHOD: After reviewing the research on phonological treatment, I discuss how a clinician's theoretical perspective influences goal selection. Five perspectives are considered: (a) normative; (b) bottom-up, discrete skill; (c) language-based; (d) broad-based; and (e) complexity-based. The literature on treatment efficiency is then considered

Deviant neurophysiological responses to phonological regularities in speech in dyslexic children.

Developmental dyslexia is strongly associated with a phonological deficit. Yet, implicit phonological processing (in)capacities in dyslexia remain relatively unexplored. Here we use a neurophysiological response sensitive to experience-dependent auditory memory traces, the mismatch negativity (MMN), to investigate implicit phonological processing of natural speech in dyslexic and normally reading children. In a modified passive oddball design that minimizes the contribution of acoustic processes, we presented non-words that differed by the degree of phonotactic probability, i.e. the distributi

Spectrographic measures of the speech of young children with cleft lip and cleft palate.

Twenty-two consecutive children with repaired cleft lip and/or palate [isolated cleft lip (CL) 6, isolated cleft palate (CP) 7, unilateral cleft lip and palate (UCLP) 7, and bilateral cleft lip and palate 2] with a mean age of 27 months underwent spectrographic measures of tape-recorded speech (DSP Sona-Graph digital unit). Controls were 22 age- and sex-matched noncleft children. Data analyzed included (1) the Spanish vocalic variables [a, i, u, e, o]: first formant, second formant, duration, and context; (2) obstruent variables [p, t, k]: burst, voice onset time, and duration, and (3) nasal v

Occurrence of dental consonant misarticulations in different cleft types.

To study the occurrence and type of misarticulations in dental consonants /r/, /s/ and /l/ 280 (115 girls, 165 boys) 6-year-old cleft children were examined by 1 of the 2 experienced speech pathologists of the cleft team. The patients included 82 children with isolated cleft palate (CP), 82 with cleft lip with (34) or without (48) cleft alveolus [CL(A)], 85 with unilateral cleft lip and palate (UCLP) and 31 with bilateral cleft lip and palate (BCLP). CP children were first divided into subgroups; there were 17 children with soft palate cleft, 49 with partial and 16 with complete hard palate cl

Genetic basis of developmental dysphasia. Report of eleven familial cases in six families.

The genetic basis of Developmental Dysphasia is discussed with precise neuropsychological descriptions of 11 cases in six families. In these cases the coexistence in the same family of completely normal and severely impaired siblings suggests a genetic rather than a familial socio-linguistic cause. The possibility of father-son transmission eliminates X-linked inheritance. The results are discussed with a reappraisal of the literature and seem to emphasize the genetic basis, possibly, with autosomal dominant transmission in some cases of Developmental Dysphasia.

[Stammering: a new approach to its treatment].

Conventional approaches to the treatment of stammering were judged ineffective in terms of both the percentage of patients cured and the relapses rate. Application of the new devices, e.g., biofeedback ones, combined with the conventional techniques only fairly improved the situation. A new combined technique is offered. It derives from a neurophysiological approach allowing one to destabilize the pathological speech pattern and create prerequisites for a quick learning (10 to 20 days) of the logopedical skills. The technique proved highly effective both in adults and in pre-school children. I

Parents' perceptual preferences between compensatory articulation and nasal escape of air in children with cleft palate.

Two groups of parents (27 parents of children with cleft palate and 25 parents of normal children) listened to 14 pairs of tape-recorded words produced by two children with cleft palate. In each pair, one word was produced with compensatory articulation and the other with audible nasal escape of air. The parents indicated their preference by selecting from each pair the word that sounded better. Parents considered single words produced with compensatory articulation to be better than single words produced with nasal escape of air. Results are interpreted in terms of existing hypotheses and inf

Children's perception of sibilants: the relation between articulation and perceptual development.

When synthetic fricative noises from an [s] continuum are followed by [a] and [u], adult listeners perceive fewer instances of in the context of the rounded vowel [u] (Mann & Repp, 1980). This perceptual context effect presumably reflects adjustment for certain coarticulatory effects and implies tacit knowledge of coarticulation and its consequences. To clarify the role of articulatory experience in the ontogeny of such knowledge and the consequent perceptual adjustment, the present study examined the effect of rounded and unrounded vowels on the perception of [s] and by adults, 5-, and 7-year

A trial speech screening test for school entrants.

An objective, low-cost means of screening school entrant children for speech disorders was designed, and speech therapists evaluated the doctor's screening results. In a field trial comprising 438 children, an accuracy rate of 92% was reported, with a referral rate by doctors of 14%, and speech therapists subsequently observing or treating 10%. The trial showed also the effects on accuracy rates of setting different pass levels.

Novel three-dimensional MRI technique for study of cartilaginous hip surfaces in Legg-Calvé-Perthes disease.

Treatment of Legg-Calvé-Perthes disease (LCPD) may improve if new knowledge can be obtained regarding how articular cartilage changes shape during the course of this disorder. A new technique is presented showing how analyses of magnetic resonance images can be used to quantify the three-dimensional changes in the femoral and acetabular articulating cartilage surfaces of children with LCPD. Ten male subjects (8 +/- 1 years) with unilateral LCPD were enrolled in this IRB approved study. Sets of magnetic resonance images of both hips were obtained at three different times.
