Interventional cardiology: state of the art.

Since the advent of percutaneous transluminal coronary angioplasty (PCTA) in 1977, a new breed of cardiologists was born "Interventional Car … ions though to represent a high risk/low success rate for balloon angioplasty. Directional coronary atherectomy, transluminal extraction catheters, and Rotacs (a low-speed rotator) have also been developed. Laser angioplasty, which initially created great enthusiasm, probably will be of limited use. Mitral balloon valvuloplasty has emerged as the most common …

Transradial rotablation in a patient with dextrocardia and acute ST-elevation myocardial infarction.

An 82-year-old gentleman with situs inversus and dextrocardia was admitted to our unit following thrombolysis for inferior ST-elevation myocardial infarction (STEMI). Due to ongoing ischemic symptoms, he underwent emergency transradial coronary angiography. His culprit right coronary artery (RCA) was heavily calcified with severe stenosis in the mid and distal segments. Therefore, rotational atherectomy was performed for debulking, and four drug eluting stents were deployed in the RCA. To the best of our knowledge, this is the first reported case of successful transradial rotablation percutane…

Anatomy, histology, and pathology of coronary arteries: a review relevant to new interventional and imaging techniques--Part IV.

In the last 15 years, intense interest has focused on various interventional pharmacologic and mechanical forms of therapy for the treatment of atherosclerosis coronary artery disease. Many techniques and devices (dilating balloons, perfusion catheters, thermal probes and balloons, lasers, atherectomy devices, stents, intravascular ultrasound) have been used or are under study for future use. Many of these techniques and devices require an understanding of histologic and pathologic features of the coronary arteries and diseases which affect them. This article reviews selective areas of anatomy…

Therapeutic angiogenesis for treatment of peripheral vascular disease.

Peripheral vascular disease (PVD) is characterized by compromised circulation to the limbs. Currently, PVD is treated conservatively and the first-line options include therapy with pain medication and "anti-thrombotic" drugs to prevent blood from clotting. Nonsurgical interventions such as angioplasty and thrombolytic therapy provide alternative treatment modalities. However, in the case of severe symptoms and with progressive deterioration, surgical interventions such as bypass surgery, atherectomy and even amputation may be necessary. Despite major advances in these areas, PVD is challenging…

Percutaneous coronary intervention and subsequent endovascular beta-radiation brachytherapy in a 14-year-old with repaired anomalous left coronary artery from pulmonary artery.

Anomalous origin of the left coronary artery from pulmonary artery (ALCAPA) is a rare congenital anomaly. Re-establishment of the dual coronary system is the standard treatment, although the long-term outcome after surgical repair is not well defined. We report a case in which coronary stenting was performed to treat left anterior descending artery lesions eight months after surgical repair of ALCAPA. The patient then developed rapid in-stent restenosis within three months, which was successfully treated by rotational atherectomy, balloon angioplasty, and catheter-based beta-radiation brachyth…

Three-vessel coronary artery disease complicated with congestive heart failure in a highly aged patient with tetralogy of Fallot having undergone palliative surgeries.

An increasing number of patients with tetralogy of Fallot (TOF) are reaching older age. We encountered a 75-year-old woman with uncorrected TOF and concomitant severe coronary artery disease (CAD) with congestive heart failure. Her CAD risk factor was hyperlipidemia, which had been untreated. Successful percutaneous coronary interventions have improved her clinical condition and provided long-term survival. Although CAD is considered to be a rare complication in adults with TOF, both strict modification of CAD risk factors and early detection of CAD would be also required in this population, g

Management of Calcified Coronary Lesions.

With an aging population, coronary calcification is increasingly encountered in modern day interventional practice. Unfortunately, it is ass … larization. A variety of treatment options for these lesions exist, including specialized balloons, atherectomy, and intravascular lithotripsy. While there is currently no universally accepted algorithm for choosing between these treatment strategies, several different algorithms exist, and the optimization of these treatment regimens will continue to evolve …

A collaborative approach to using new technologies for the management of heart disease at the University of Ottawa Heart Institute.

Nursing in the Cardiac Operating Rooms and Catheterization Laboratories at the University of Ottawa Heart Institute has played a collaborative role in advancing and promoting the use of new technologies for treating heart disease. This has been demonstrated in the Catheterization Laboratories with the introduction of such procedures as angioplasties, atherectomies, stents and rotablators and in the Cardiac Operating Rooms with heart transplants and total artificial heart implants. Currently, nurses in the Operating Rooms are actively involved in research on the electrohydraulic totally implant

Rotablation in Patients with Advanced Renal Insufficiency through End-Stage Renal Disease: Short- and Intermediate-Term Results.

OBJECTIVE: Patients with advanced renal insufficiency are at high risk of coronary artery disease (CAD) and complex lesions. Treating complex calcified lesion with rotational atherectomy (RA) in these patients might be associated with higher risks and poorer outcomes. This study was set to evaluate features and outcomes of RA in these patients. METHOD: Consecutive patients who received coronary RA from April 2010 to April 2018 were queried from the Cath Lab database. The procedural details, angiography, and clinical information were reviewed in detail. RESULTS: A total of 411 patients were e…

Bailout Intravascular Lithotripsy for the Treatment of Acutely Underexpanded Stents in Heavily Calcified Coronary Lesions: A Case Series.

Stent underexpansion is a common problem in heavily calcified coronary lesions treated with percutaneous coronary intervention, and has been associated with in-stent restenosis, stent thrombosis and, subsequently, poor clinical outcomes. Adequate preparation of heavily calcified coronary lesions (e.g. using non-compliant balloons, cutting/scoring balloons, rotational/orbital atherectomy or intravascular lithotripsy) prior to stent implantation is essential in preventing stent underexpansion. However, in certain cases the deployed stent may remain underexpanded despite extensive lesion preparat…

In-Stent Restenosis: Pathophysiology and Treatment.

Management of in-stent restenosis (ISR) remains a clinical challenge after both bare metal stent and drug-eluting stent placement. Autopsy studies and intravascular imaging have augmented our understanding of the pathophysiology of ISR. The clinical presentation and symptoms vary considerably among patients from stable angina to unstable angina and acute myocardial infarction. Potential treatment strategies include plain old balloon angioplasty (POBA), rotational atherectomy, vascular brachytherapy (VBT), bare metal stents (BMS), drug-coated balloons (DCBs), and drug-eluting stent (DES) placem…

Outcomes of Endovascular Management of Isolated Profunda Femoris Artery Occlusive Disease.

BACKGROUND: Endovascular management of isolated profunda femoris artery occlusive disease has not been well studied. Our aim is to analyze the outcomes of endovascular management of profunda artery occlusive disease. METHODS: This is a retrospective analysis using data from the Vascular Quality Initiative. All patients from 2013 to 2018 treated percutaneously for isolated profunda artery occlusive disease were included. Endovascular treatment included plain balloon alone, stent, stent graft, atherectomy, and drug-coated balloon without any concomitant endovascular or surgical treatment. Demog…

Antithrombotic Therapy After Peripheral Vascular Intervention.

Cardioprotective medications and risk-factor modification are the hallmarks of treatment for all patients with peripheral artery disease (PAD). If symptoms are life-limiting and/or do not respond to conservative treatment, endovascular or surgical revascularization can be considered especially for patients with critical limb ischemia or acute limb ischemia. The rates of peripheral vascular intervention (PVI) have risen dramatically over the past few decades and much of this care have shifted from inpatient hospital settings to outpatient settings and office-based clinics. While PVI rates have

Combined percutaneous treatment for pulmonary valve stenosis and atrial septal defect in an adult patient.

Pulmonary valve stenosis and atrial septal defect are common forms of congenital heart disease; however, their association is relatively rare. When the two conditions are present simultaneously, significant left-to-right shunt is often prevented by the outflow obstruction, which protects the pulmonary bed until adulthood. This work describes a case of simultaneous percutaneous treatment of both congenital malformations. Although these procedures have been applied in isolation as methods of treatment, this case demonstrates the feasibility and effectiveness of a combined percutaneous treatment.
