Cardiac structural and functional changes in OSAHS patients with heart failure with preserved ejection fraction and atrial fibrillation.

OBJECTIVE: To explore the cardiac structural and functional changes in obstructive sleep apnea-hypopnea syndrome (OSAHS) patients with heart failure with preserved ejection fraction (HFpEF) and atrial fibrillation (AF). METHODS: This retrospective study included 336 OSAHS patients with HFpEF. They were divided into Groups A (without an AF history and no AF episodes during cardiac color ultrasound examination), B (an AF history but no AF episodes), and C (an AF history and AF episodes). They all received cardiac color ultrasound examinations. Cardiac structural and functional changes in ultras

Deciphering the roles of triiodothyronine (T3) and thyroid-stimulating hormone (TSH) on cardiac electrical remodeling in clinical and experimental hypothyroidism.

Hypothyroidism is the most frequent endocrine pathology. Although clinical or overt hypothyroidism has been traditionally associated to low T3 / T4 and high thyrotropin (TSH) circulating levels, other forms exist such as subclinical hypothyroidism, characterized by normal blood T3 / T4 and high TSH. In its different forms is estimated to affect approximately 10% of the population, especially women, in a 5:1 ratio with respect to men. Among its consequences are alterations in cardiac electrical activity, especially in the repolarization phase, which is accompanied by an increased susceptibility

Cardiac function, structural, and electrical remodeling by microgravity exposure.

Space medicine is key to the human exploration of outer space and pushes the boundaries of science, technology, and medicine. Because of harsh environmental conditions related to microgravity and other factors and hazards in outer space, astronauts and spaceflight participants face unique health and medical challenges, including those related to the heart. In this review, we summarize the literature regarding the effects of spaceflight on cardiac structure and function. We also provide an in-depth review of the literature regarding the effects of microgravity on cardiac calcium handling. Our r

Mapping the Progression of Atrial Fibrillation Substrate: Insights from Left Atrial Wall Thickness and High-Density Voltage Mapping.

Background/Objective: Advances in cardiac computed tomography (CT) now allow high-resolution, three-dimensional reconstruction of the left atrium, enabling precise quantification of left atrial wall thickness (LAWT). However, the relationship between LAWT and electrophysiological properties, particularly as assessed by high-density bipolar voltage mapping, remains poorly defined. The objective was to evaluate left atrial structural remodeling in atrial fibrillation (AF) by integrating three-dimensional CT-derived LAWT maps with high-density bipolar voltage mapping, and to assess the relationsh

Reduced FNDC5-AMPK signaling in diabetic atrium increases the susceptibility of atrial fibrillation by impairing mitochondrial dynamics and activating NLRP3 inflammasome.

Fibronectin type III domain-containing protein 5 (FNDC5) exerts potential anti-arrhythmic effects. However, the function and mechanism of FNDC5 in diabetes-associated atrial fibrillation (AF) remain unknown. In this study, bioinformatics analysis, in vivo and in vitro experiments were conducted to explore the alteration and role of FNDC5 in diabetes-related atrial remodeling and AF susceptibility. RNA sequencing data from atrial samples of permanent AF patients and diabetic mice exhibited significantly decreased FNDC5 at the transcriptional level, which was in line with the protein expression …

Atrial fibrillation promotion in a rat model of heart failure induced by left ventricle radiofrequency ablation.

BACKGROUND: Atrial fibrillation (AF) frequently coexists with congestive heart failure (CHF). The increased susceptibility to AF in CHF has been attributed to a variety of structural and electrophysiological changes in the atria, particularly dilation and interstitial fibrosis. We evaluated atrial remodeling and AF vulnerability in a rat model of CHF induced by left ventricle (LV) radiofrequency (RF) ablation. METHODS: Wistar rats were divided into 3 groups: RF-induced CHF (Ab, n = 36), CHF animals treated with spironolactone (AbSpi, n = 20) and sham controls (Sham, n = 29). After 12 weeks, a…

Novel Mechanistic Insights Into Atrial Functional Mitral Regurgitation - 3-Dimensional Echocardiographic Study.

BACKGROUND: Left atrial remodeling caused by persistent atrial fibrillation (AF) causes atrial functional mitral regurgitation (MR), even though left ventricular (LV) remodeling and organic changes of the mitral leaflets are lacking. The detailed mechanism of atrial functional MR has not been fully investigated. METHODS AND RESULTS: Of 1,167 patients with AF who underwent 3D transesophageal echocardiography, 75 patients were retrospectively selected who developed no, mild, or moderate-to-severe atrial functional MR (n=25 in each group) despite an LV ejection fraction ≥50% and LV volumes withi…

Cardiac α2δ1 C-terminal contributes to left atrial hypertrophy in chronic ischemic heart failure, in association with changes in membrane GluN1 and p-CAMKII/p-HDAC4 signaling.

Left atrial remodeling (LAR) critically contributes to the progression of heart failure (HF) and the development of atrial fibrillation (AF) following myocardial infarction (MI). The protein α2δ1, primarily known for its role in neuropathic pain, is abundantly expressed in atrial tissue, but its involvement in post-MI LAR remains unclear. Here, LAR models were established in rats post-MI, and atrial hypertrophy was induced in HL-1 cells using angiotensin II (AngII). The role of α2δ1 in atrial hypertrophy was examined through treatment with either the α2δ1 inhibitor gabapentin or a C-terminal i…

Transient changes in P-wave terminal force after electrical cardioversion of persistent atrial fibrillation.

BACKGROUND: P-wave terminal force in lead V1 (PWTFV1) reflects left atrial remodeling and is associated with atrial fibrillation (AF) outcomes, but its time course after electrical cardioversion of persistent AF is unclear. METHODS: We retrospectively analyzed 108 patients with persistent AF after successful electrical cardioversion, with 12‑lead electrocardiograms at 30 min, 1 month, and 3 months. Terminal P-wave duration (PNTV1) and amplitude (PNAV1) in lead V1 were measured and PWTFV1 calculated as their product; changes are expressed in absolute magnitude. Correlations with left atrial di…

Increased thin filament activation enhances alternans in human chronic atrial fibrillation.

Action potential duration (APD) alternans (APD-ALT), defined as beat-to-beat oscillations in APD, has been proposed as an important clinical marker for chronic atrial fibrillation (cAF) risk when it occurs at pacing rates of 120-200 beats/min. Although the ionic mechanisms for occurrence of APD-ALT in human cAF at these clinically relevant rates have been investigated, little is known about the effects of myofilament protein kinetics on APD-ALT. Therefore, we used computer simulations of single cell function to explore whether remodeling in myofilament protein kinetics in human cAF alters the

Ventricular hypertrophy and left atrial dilatation persist and are associated with reduced survival after valve replacement for aortic stenosis.

OBJECTIVES: We sought to understand the factors modulating left heart reverse remodeling after aortic valve replacement, the relationship between the preoperative symptoms and modulators of left heart remodeling, and their influence on long-term survival. METHODS: From October 1991 to January 2008, 4264 patients underwent primary aortic valve replacement for aortic stenosis. Changes in the time course of left ventricular reverse remodeling were assessed using 5740 postoperative transthoracic echocardiograms from 3841 patients. RESULTS: Left ventricular hypertrophy rapidly declined after surg

Atrial cardiomyopathy in endurance athletes.

Atrial cardiomyopathy is characterized by electrical and structural remodeling of the atria, which can predispose to arrhythmias and thromboembolic stroke. Changes in atrial size and function are frequently observed in athletes engaged in endurance sports, a phenomenon known as "athlete's heart." Common left atrial observations in athletes may include larger left atrial volumes but lower left atrioventricular volume ratios, mildly reduced left atrial strain, possible mild left atrial fibrosis, longer P-wave duration, and greater atrial ectopic activity. However, it remains unclear whether thes

Admission eosinophil count is associated with favorable cardiac remodeling after ST-segment elevation myocardial infarction.

Experimental studies suggest that eosinophils promote myocardial repair after myocardial infarction; however, their clinical significance in post-infarction cardiac remodeling remains unclear. We investigated the association between admission peripheral eosinophil counts and left ventricular (LV) and left atrial (LA) remodeling after ST-segment elevation myocardial infarction (STEMI). This retrospective single-center study included 452 consecutive patients with first-time STEMI who underwent successful primary percutaneous coronary intervention between 2016 and 2024. Patients were divided into

Interatrial Shunt Devices.

Elevated left atrial pressure during exercise is a hallmark of heart failure (HF) and is associated with adverse left atrial remodeling and poor outcomes. To decompress the pressure-overloaded left atrium in patients with HF, several device-based approaches have been developed to create a permanent, pressure-dependent, left-to-right interatrial shunt. Such approaches are currently in various stages of investigations in both HF with reduced ejection fraction (EF) and HF with preserved EF. This review discusses the evolution of the concept of left atrial decompression and summarizes the current …
