Cholecystopleural fistula as a cause of recurrent infected biliothorax.

A cholecystopleural fistula is an extremely rare condition characterized by abnormal communication between the gallbladder and pleural cavity. This abnormal connection can lead to the leakage of bile or infectious material into the pleural space, potentially causing infected biliothorax. Here, we report a case of recurrent infected biliothorax caused by a cholecystopleural fistula in a 77-year-old man. The patient had a history of acute cholecystitis and choledocholithiasis for which endoscopic gallbladder stenting was performed one year earlier. The patient was admitted for fever and right pl

[Bipolar approach to biliobronchial fistulas].

On the basis of 4 cases (three bilio-bronchial fistulae of hydatic origin, and one post-traumatic fistula), the authors review the problems of etiology and of pathological physiology. After an interpretation of the results obtained they recommend the bi-polar approach (pulmonary and biliary) in the same surgical intervention. In relation with the condition of the patient the second stage of the intervention will be performed after 14 to 21 days (the abdominal stage being carried out after the thoracic stage).

Common bile duct complications of pancreatitis evaluation and treatment.

Five cases that illustrate the spectrum of biliary complications of pancreatitis and pancreatic pseudocyst are discussed. Obstructive jaundice, hemobilia, and bilious ascites were the major problems in these five patients. Sonography, transhepatic cholangiogram, endoscopic retrograde cholangiopancreatography, operative cholangiography, and arteriography are important in establishing the diagnosis and planning the treatment. Three patients had biliary obstruction caused by chronic pancreatitis, a pancreatic pseudocyst, or both. Two patients had a fistula between the common duct and the pseudocy

Histological changes at an endosonography-guided biliary drainage site: a case report.

Endosonography-guided biliary drainage (ESBD) is a new method enabling internal drainage of an obstructed bile duct. However, the histological conditions associated with fistula development via the duodenum to the bile duct have not been reported. We performed ESBD 14 d preoperatively in a patient with an ampullary carcinoma and histologically confirmed changes in and around the fistula. The female patient developed no complications relevant to ESBD. Levels of serum bilirubin and hepatobiliary enzymes declined quickly, and pancreatoduodenectomy was carried out uneventfully. The resected specim

Cholecystocutaneous Fistula Associated with Edwardsiella tarda as a Result of Aggressive Lymphoma Development.

A cholecystocutaneous fistula is a type of external biliary fistula that occurs when there is an abnormal connection between the gallbladder and skin. We report the first case of a cholecystocutaneous fistula that occurred in association with the development of lymphoma in the gallbladder. A 76-year-old woman who was under observation for follicular lymphoma with a low tumor burden presented with fatigue and abdominal pain. Imaging studies revealed cholecystitis associated with an abdominal subcutaneous abscess, and lymphoma transformation was confirmed by a lymph node biopsy. Edwardsiella tar…

Is live problem better than dead cert?

Cholecystoduodenal fistula is a type of biliary enteric fistula that can occur as a result of chronic cholecystitis. The majority of cases are diagnosed and their subsequent management planned intraoperatively. Previous reports have focused on successful laparoscopic management. We report three cases of elective cholecystectomy that were diagnosed intraoperatively with a cholecystoduodenal fistula (CDF). The first two cases were managed laparoscopically. In case 3 further continuation of surgery would have subjected the patient to significant risk of further harm. Instead the patient underwent

Incidentally Found Cholecystoduodenal Fistula and an Unusual Case of Gallstone Ileus After Laparoscopic Cholecystectomy.

Gallstone ileus, a rare and potentially fatal complication of cholelithiasis, occurs when gallstones breach the gastrointestinal tract through a fistula, causing an obstruction and potentially leading to severe complications. This case report details the experience of a 44-year-old woman with gallstone ileus stemming from an unnoticed cholecystoduodenal fistula following a routine cholecystectomy. The fistula was only discovered during surgery despite advanced imaging, revealing extensive adhesions. The discovery led to a subtotal cholecystectomy and fistula repair. Postoperatively, complicati

Gallbladder-duodenal fistula presenting with liver abscess and upper gastrointestinal bleeding.

BACKGROUND: Bilio-enteric communications leading to liver abscess formation are encountered rarely and are therefore not easily suspected by the attending physician. CASE OUTLINE: A bilio-enteric communication involving the gallbladder and the duodenum presented as a septic event with upper gastrointestinal bleeding in a 71-year-old man who was wrongly thought to have undergone a previous cholecystectomy. A pyogenic bacterial liver abscess developed from the fistula in the absence of biliary obstruction. The patient was treated surgically with disconnection of the fistula and drainage of the

Beyond Rigler's Triad: Gallstone Ileus in the Absence of a Bilioenteric Fistula.

Gallstone ileus is an uncommon cause of mechanical intestinal obstruction, classically associated with bilioenteric fistula formation that enables gallstone migration into the gastrointestinal tract. Its incidence increases among elderly patients and is associated with significant morbidity due to delayed diagnosis. Obstruction typically occurs at the terminal ileum, and diagnosis traditionally relies on Rigler's triad: intestinal obstruction, pneumobilia, and an ectopic gallstone. Gallstone ileus without a bilioenteric fistula is an exceptionally rare presentation that challenges conventional

Atypical Proximal Gallstone Ileus Near the Ligament of Treitz Managed With Staged Surgical Intervention: A Case Report.

Gallstone ileus is a rare mechanical bowel obstruction caused by the migration of a gallstone through a biliary-enteric fistula, most commonly impacting at the ileocecal valve. Proximal obstruction is uncommon and is typically classified as Bouveret syndrome when involving the stomach or proximal duodenum. Obstruction near the ligament of Treitz represents an exceedingly rare variant. A 76-year-old female presented with nausea, vomiting, and abdominal distention. Computed tomography demonstrated gastric and proximal small bowel dilation secondary to an obstructing gallstone in the distal duode

Etiological relationships between choledochal cyst and anomalous junction of the pancreaticobiliary ductal system.

The etiology of choledochal cyst has been investigated by clinicians and pathologists primarily of Germany, Austria, United States, United Kingdom, and Japan from the end of the last century, but Japanese investigators contributed greatly to the study of the relationship between choledochal cyst and anomalous junction of the pancreaticobiliary ductal system. Heid (1893) is considered to be the first to document choledochal cyst associated with anomalous pancreaticobiliary junction, but it was probably Kozumi et al. (1916) that first evaluated anomalous junction in relation to the etiology of c

[Digestive complications of biliary gallstone lost during laparoscopic cholecystectomy].

A patient was admitted because of an intestinal obstruction. Eight years before, he underwent a laparoscopic cholecystectomy. Abdominal ultrasonography and small bowel series showed a gallstone in the small bowel that computed tomography scan failed to identify. Laparotomy showed a Meckel's diverticula and a biliary stone in the terminal ileum. In the literature, it has been shown that gallstone lost during laparoscopic cholecystectomy may be responsible for intraperitoneal abscess. In the contact of intestine, the stone may induce an obstructive abscess, a communicating abscess, a digestive f

[Cholecystocolic fistula: from symptoms to diagnosis].

We report 2 patients with a cholecystocolic fistula in whom diametrically opposite symptoms (isolated steator-rhea, jaundice and fever) were responsible for important clinical difficulties. These fistulae are far less common than cholecysto-duodenal fistulae. Endoscopic retrograde cholangiography was performed because of abnormal liver tests and led to diagnosis in both cases. Classical aerobilia was absent in both of our cases. Barium enema was disappointing (case n. 1), whereas technetium 99m scintiscan visualized the fistula (case n. 2). Two therapeutic approaches are possible in the case o

[Gastrojejunocolic fistula--a rare complication of stomach surgery].

The gastrojejunocolic fistula represents a clinical entity that occurs very rarely following gastro-jejunal anastomoses and manifests itself clinically and paraclinically by a severe malabsorption syndrome. The results of the physiopathological approach may be summed up as follows: reduced level of seric proteins, fluid and electrolytic depletion, deficiencies in the absorption of the vitamins soluble in fats and water, which may all vary from mildness to severeness, depending on the flow rate of the fistula. Most often, the diagnosis is set by performing barium enema, which is positive for al

Interventional radiology in the treatment of early postoperative biliary complications.

PURPOSE: We sought to evaluate the feasibility and efficacy of percutaneous treatment of early postoperative biliary complications. The primary aims were to evaluate clinical and technical success and complications and perioperative mortality, and secondary aims were to evaluate treatment duration and recurrence rate. MATERIALS AND METHODS: Between March 2007 and March 2010, 75 patients (42 men and 33 women; age range, 17-88 years; mean age, 60.8 years) underwent interventional radiology procedures to treat early postoperative biliary complications of biliary and pancreatic-duodenal surgery w

Endoscopic Retrograde Cholangiopancreatography (ERCP): Used to Diagnose and Treat Cholecystoduodenal Fistula, a Rare Clinical Entity.

Biliary enteric fistula is a rare diagnosis. Common etiologies include chronic cholecystitis with cholelithiasis and peptic ulcer disease. Of these, the number one cause is chronic cholecystitis with cholelithiasis. Adhesion of a chronically inflamed gallbladder to the duodenum followed by erosion of the gallbladder wall by gallstones leads to the establishment of an abnormal communication between the gallbladder and duodenum. This abnormal communication, namely, cholecystoduodenal fistula, has a high mortality rate and therefore must be managed in a timely manner. The case presented in this r

Mirizzi syndrome and biliobiliary fistulas: roentgenologic appearance.

Five cases of extrahepatic cholestasis caused by impaction of a voluminous gallstone in the neck of the gallbladder or the cystic duct are reported. The formation of a biliobiliary fistula is the rule. Percutaneous transhepatic cholangiography visualizes the fistulas and permits their classification into 2 groups: gallbladder to hepatic duct, and cystic to hepatic duct. Preoperative diagnosis permits the best resolutions of 2 problems encountered during surgery: (1) the tumor-like feature often produced by the cholecystitis can be ascribed to benign lithiasic etiology if the biliobiliary fistu…

The relationship of Mirizzi syndrome and cholecystoenteric fistula: validation of a modified classification.

BACKGROUND: Mirizzi syndrome and cholecystoenteric fistula with or without gallstone ileus are late complications of gallstone disease. We previously suggested that the natural history of Mirizzi syndrome may not end with just a cholecystobiliary fistula and that the continuous inflammation in the triangle of Calot area may result in a complex fistula involving the biliary tract and the adjacent viscera. The purpose of this study was to establish the relationship of Mirizzi syndrome with cholecystoenteric fistulas. METHODS: We retrospectively reviewed the records of all patients older than ag…
