Anamnestic, clinical and morphological data presented concern 34 children with malformations of excretory bile ducts (15 with atresia of intra- and extrahepatic bile ducts, 12 with atresia of distal regions of the external bile ducts, 7 with cystic dilatation of ductus choledochus). Pancreatitis, commonly encountered in the Emergency Department, possesses a very broad clinical spectrum and may be associated with shock and multiple organ failure. Its diagnosis is based on clinical, laboratory, and radiographic data, but there is no gold standard. Biliary tract disease ranges in severity from cholelithiasis with colic to acute suppurative cholangitis, which may lead to shock. Clinical examination and imaging studies are most useful in these disorders, and definitive treatment is primarily surgical.

A 3-year-old child presented with vague abdominal pain, fever, leucocytosis and elevation of alkaline phosphatase. Ultrasonography revealed a space occupying process within the extrahepatic bile ducts surrounded by fluid. Various densities (between 15-25 Hounsfield units) were measured in this intrabiliary tumor by computed tomography.

The authors provide the results of studying the microcirculation in 85 patients with acute cholecystitis and 56 patients with mechanical jaundice caused by choledocholithiasis. The data were obtained with the aid of bulbar capillaroscopy during conservative and surgical treatment. Considerable alterations were detected in all the components of the microcirculation (perivascular, vascular, intravascular), correlating with the gravity of destructive lesions in the gallbladder, duration and degree of jaundice. The times and degree of the normalization of microcirculatory disorders were defined.

Caroli's disease (congenital dilatation of the intrahepatic bile ducts) is a rare condition which is usually diagnosed postoperatively. Scintigraphic evaluation of this entity is very limited. We report here a patient with Caroli's disease in whom a dichotomy in hepatic uptake of 99mTc-HIDA (hot spots) and 99mTc-colloid (cold spots) was observed, leading to a correct preoperative evaluation of the disease.

The records of Alaskan Native patients with gallbladder and extrahepatic bile duct cancers diagnosed during 1970--79 were reviewed. Of 34 cases identified, the primary cancer was in the gallbladder in 29, resulting in age-adjusted incidence rates for gallbladder cancer of 4.4 for males and 17.6 for females. Biliary-tract disease is not only more common in the geriatric population but carries an increased surgical risk. Described is a series of 10 middle-aged and elderly patients who had stones in the common bile duct associated with significant underlying medical illness or peri-ampullary pathologic changes. They were successfully treated by endoscopic papillotomy. The 80 percent success rate in such critically ill patients suggests that endoscopic papillotomy might be used as the initial treatment modality in aging patients with amenable biliary-tract disease.

From 1974-8, 808 postoperative choledochoscopy procedures, conducted by insertion of choledochofiberscope into the biliary tract through the sinus tract after the T-tube had been removed, were carried out in 292 patients at Teikyo University Hospital, Tokyo, Japan. Opinions of the surgeons on the one hand and the rest of the medical profession on the other hand differ, often markedly, on the indications for operation in the gallbladder and biliary tract area particularly. A case of an elongated, anomalous duodenum with intestinal rotation, annular pancreas and disease of the biliary tract with acute jaundice caused by gallstone perforation is presented. The ERCP (endoscopic retrograde cholangiopancreatography) was of great value in the preoperative investigations. Surgical relief of the obstruction of the biliary duct made the patient free of symptoms.
