Blepharospasm, psychotic symptoms, and obsessive-compulsive symptoms can result from brain infarction. However, a presentation of these 3 symptoms simultaneously is rare. This report describes a 65-year-old woman who had old brain infarction and presented with these 3 symptoms for 2 years. Tardive dystonia is a movement disorder dominated by involuntary muscle contractions that may be tonic, spasmodic, patterned or repetitive, associated with the use of dopamine-receptor blocking agents. Most of the patients with tardive dystonia present initially with blepharospasm. Treatment of dystonia is generally disappointing. A patient with chronic paranoid schizophrenia who developed blepharospasm is described here. Blepharospasm remitted after a course of electroconvulsive therapy. Remission was sustained until 3 months after stopping maintenance electroconvulsive therapy.

We analyzed the influence of clinical and psychological factors on the long-term efficacy of botulinum toxin A (BTX) injections in 45 patients with blepharospasm and 66 patients with hemifacial spasm. Injections efficacy (respectively 94.3% and 95.7%) and duration of relief (respectively 14.8 and 18.7 weeks) remained stable over seven successive injections. In the neuro-ophthalmology clinic at St. Vincent's Hospital, Melbourne, 57 patients with blepharospasm and 50 patients with hemifacial spasm were treated with botulinum toxin.
