Associations between social cognition and inflammatory markers in eating disorders and borderline personality disorder.

INTRODUCTION/OBJECTIVES: Eating disorders (EDs) and borderline personality disorder (BPD) are severe psychiatric conditions characterized by emotional dysregulation and impairments in social cognition. Growing evidence suggests that inflammatory and oxidative stress pathways may contribute to their pathophysiology. The present study aimed to explore the association between social cognition performance and immune-inflammatory and oxidative stress biomarkers in individuals with BPD, restrictive ED (EDR), and purging ED (EDP) vs healthy controls (HC), using a pathway-based psychoneuroimmunologica…

Is the Prejudice Towards People With Borderline Personality Disorder Model Reflective of Lived Experience?

The high levels of stigma faced by people with borderline personality disorder (BPD) is well recognized but inconsistently operationalized within the literature through the use of multiple instruments that measure different prejudicial attitudes. The Prejudice towards People with Borderline Personality Disorder (PPBPD) framework was developed to provide a comprehensive model and psychometrically sound scale for measuring prejudice. The four PPBPD subscales encompass the different prejudicial attitudes measured across other existing instruments, namely Fear/Avoidance, Malevolence, Authoritarian…

Childhood Mental Health Characteristics of Adults Diagnosed With Borderline Personality Disorder: A Descriptive Study.

INTRODUCTION: Borderline personality disorder (BPD) is a burdensome and diagnostically challenging condition. There is a need to identify its risk factors, yet prior studies are scarce and small-scale. We aimed to describe the prevalence of psychiatric comorbidities and the level of mental health resource utilization during adolescence in patients with BPD. METHODS: We conducted a retrospective descriptive study of 938 participants (aged 18-26 years) with BPD at a large, multi-site, integrated health maintenance organization (HMO) system in Southern California. Data were collected from Januar…

Follow-up effects of a brief Good Psychiatric Management treatment for borderline personality disorder.

BACKGROUND: Borderline personality disorder (BPD) is among the most frequent, debilitating, and lethal mental conditions. Treatment for patients with BPD involves structured psychotherapy, but such treatment is often not readily available due to imbalances in supply and demand for care. Brief treatments that are available as first-line interventions may address this delay in care. No controlled study has assessed the effectiveness of brief psychiatric interventions, such as 10 sessions of Good Psychiatric Management (GPM) over 12 months follow-up, in comparison with treatment as usual. Whereas…

Treatment of patients with borderline personality disorder in the emergency room? A scoping review.

BACKGROUND: Borderline personality disorder (BPD) is frequently encountered in emergency departments. Acute emotional crises, suicidal behaviors, and severe interpersonal distress often precipitate care. Despite the high clinical burden and elevated suicide risk associated with this disorder, guidance for emergency decision-making remains fragmented and sometimes contradictory. OBJECTIVE: This scoping review aimed to identify key elements informing emergency psychiatric assessment, disposition decisions, and acute management strategies for adults diagnosed with BPD. Methods: Following Preferr…

Promoting self-compassion meditation with intranasal oxytocin in borderline personality disorder: a pilot study.

BACKGROUND: Borderline personality disorder (BPD) is characterized by emotion dysregulation, interpersonal difficulties, and high levels of self-criticism, which may hinder engagement with compassion-based therapeutic practices. Oxytocin has been proposed as a modulator of social-affective processes and may facilitate specific mechanisms involved in psychotherapy. METHODS: In this pilot study, 18 individuals with BPD received intranasal oxytocin (24 IU) or placebo prior to a guided compassion-based meditation practice over five weekly sessions. Compassion practice quality was assessed at base…

Survey on Nursing Education and the Perceived Burden of Borderline Personality Disorder in France.

Borderline personality disorder (BPD) is a complex psychiatric condition often linked with self-harm, comorbidities, and high healthcare use. Nurses, frequently on the front line of care, report relational difficulties that may stem from insufficient training. A cross-sectional survey was conducted among nurses in three psychiatric hospitals in Northern France. An anonymous online questionnaire assessed professional identity, nursing school training, transition into practice, perceptions of BPD, and comfort in care. 210 nurses were included. Most judged their psychiatric training incomplete, c…

Inflammatory biomarkers and white matter microstructure in borderline personality disorder: A cross-sectional study.

BACKGROUND: Borderline personality disorder (BPD) is a severe psychiatric condition with unclear neurobiology. Inflammation and white matter (WM) abnormalities have been implicated in BPD. We aimed to compare WM microstructure and inflammatory biomarkers between Individuals with BPD and healthy controls (HC), and to test their relationship. METHODS: In a cross-sectional study, female Individuals with BPD (n = 40) and age-matched female HCs (n = 37) were assessed. We measured serum levels of pro-inflammatory cytokines (e.g., interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), and C-reactive…

Why Is It Still So Difficult to Acknowledge the Reality of Borderline Personality Disorder?

Borderline personality disorder (BPD) has emerged as a major clinical and societal challenge since its mid-twentieth-century conceptualization. This paper addresses four persistent obstacles to effective care: diagnostic complexity, interpersonal dysfunction and risk behaviors, high societal burden, and pervasive stigma. We argue that despite recent critiques, many of which propose replacing the diagnosis with trauma-related formulations, BPD remains a valid, coherent, and clinically indispensable construct. Misdiagnosis contributes to iatrogenic harm, while accurate diagnosis enables targeted…

Assessment of neurotransmitter-related structural brain changes in borderline personality disorder: an MRI study.

Borderline personality disorder (BPD) is characterized by grey matter (GM) alterations in areas related to emotion regulation, impulsivity and metacognitive abilities. Dysfunction of specific neurotransmitters, including serotoninergic and dopaminergic systems, is thought to be involved, but evidence is currently limited. In this exploratory study, we tested for associations between GM alterations in BPD, the spatial distribution of neurotransmitter systems, and clinical features in a sample of 58 patients with BPD and 28 healthy controls (HC). GM volume alterations were assessed on 3T MRI usi…

Structural white matter alterations in borderline personality disorder: A narrative review and methodological considerations.

Borderline personality disorder (BPD) is a severe and heterogeneous psychiatric condition characterized by emotional dysregulation, impulsivity, and interpersonal instability. Increasing evidence suggests that disruptions in white matter (WM) microstructure may contribute to its neurobiological underpinnings. Clarifying these findings is important because diffusion MRI studies in BPD remain heterogeneous in terms of age, symptom severity, comorbidity, medication status, trauma exposure, and analytic approach. This narrative review synthesizes current findings from diffusion tensor imaging (DTI…

Structural alterations in early auditory cortex in borderline personality disorder with auditory verbal hallucinations.

Auditory Verbal Hallucinations (AVH) are not exclusive to schizophrenia but also occur in other psychiatric disorders, including Borderline Personality Disorder (BPD). While AVH in SZ has been extensively studied, their neurobiological underpinnings in BPD remain poorly understood. Preliminary evidence suggests structural anomalies in auditory regions among BPD patients experiencing AVH, indicating potential neuroanatomical vulnerabilities. This study examined structural brain differences in female BPD patients with (BPD-AVH n = 20) and without (BPD-nAVH n = 27) a lifetime history of AVH, as w…

Emotion regulation in young children of mothers with borderline personality disorder: A transgenerational perspective.

Emotion regulation (ER) difficulties in early childhood are considered potential predictors of later psychopathology, particularly in children of mothers with Borderline Personality Disorder (BPD). This multicenter study examined ER in German-speaking children of mothers with BPD (C-BPD), children of mothers with anxiety/ depressive disorders, and children of mentally healthy mothers (N = 318; Mage = 39.41 months, SD = 22.16; range: 6-84 months, 53.5% female). We hypothesized that C-BPD would display more unfavorable (maladaptive) ER strategies than the other groups. ER was assessed via behavi…

Functional brain connectivity changes following Masterson Psychotherapy in borderline personality disorder: a pilot study.

The Masterson Psychotherapy (MP) is a therapeutic method used to manage symptoms of Borderline Personality Disorder (BPD). This pilot study aimed to investigate the effects of MP on brain connectivity in individuals with BPD using resting-state fMRI. We collected resting-state fMRI data from 10 individuals with BPD before and after 20 MP sessions to examine changes in brain connectivity. Pre-therapy data from these individuals were also compared to data from 10 healthy controls. Results showed reduced hippocampal connectivity in individuals with BPD compared to controls, which significantly im…

A pilot and feasibility study of Sage: A couple therapy for borderline personality disorder.

Objective: Borderline personality disorder (BPD) is characterized by emotion dysregulation and relationship dysfunction. However, the majority of BPD treatments provide individually-focused treatment and do not target relationship functioning or include significant others. The current paper presents the primary and secondary outcomes from an uncontrolled trial of Sage, a manualized conjoint treatment for people with BPD and their intimate partners that targets BPD, relationship conflict, and partner mental health simultaneously. Method: Sixteen couples wherein one member has BPD and elevated s…

Sleep and affect in individuals with borderline personality disorder features: A daily diary study.

Given that people with borderline personality disorder (BPD) features are vulnerable to emotion dysregulation, they might be particularly sensitive to the emotional impacts of poor sleep quality and low sleep quantity. In the present study, we evaluated whether BPD features moderate the relationship between daily sleep, measured objectively (total sleep time [TST]) and subjectively (perceived sleep quality), and self-reports of positive and negative affect (NA). A sample of 83 cohabitating couples (n = 166) were recruited for a 3-week daily diary study. Participants completed a BPD symptom mea…
