Rhinosinusitis is a common medical problem. Complications involving the orbits or brain are not frequently met. Concomitant involvement of both orbits and the brain is very rare. We here describe a 6-year-old girl who suffered from acute rhinosinusitis and initially, subperiosteal abscesses in both orbits. Bilateral functional endoscopic sinus surgery and drainage of the subperiosteal abscesses through the upper eyelids were done. Pus cultures showed methicillin-resistant Staphylococcus aureus. The brain abscesses were found under magnetic resonance imaging (MRI) and were treated conservativel…

Three cases of bacterial brain abscesses, in immunocompetent patients, are reported. In all these cases, the diffusion-weighted magnetic resonance (MRI) with apparent diffusion coefficient (ADC) map has permitted an early diagnosis and a rapid treatment. This emergency MRI showed in the three cases a low signal on TI-weighted images, a high signal on T2-weighted and echo-planar images, and a decrease of ADC (0.36- 0.49 x 10(-3) mm2/s). So, this new MRI technique provides an available and rapid element in the brain abscess diagnosis which often remains a complex clinical and radiological diagno…

We report the case of a young man who apparently suffered successive episodes of meningitis and cerebral abscess over a 1-month period, both of which were diagnosed by two different molecular approaches; PCR for Neisseria meningitidis IS1106 from CSF and 16S rRNA gene sequencing on a specimen of brain pus. In each case, cultures were negative due to prior antibiotic therapy.

The frequency of tuberculous cerebral involvement associated with miliary tuberculosis is not well known; some authors have evaluated it at around 22%. We report the case of a 43-year-old man hospitalised in the pulmonology department with bacillary positive miliary tuberculosis. On the 26th day of antituberculosis treatment he complained of frontal headaches, with impairment in visual acuity. Examination of the optic fundus showed bilateral papillary oedema. Cerebral computed tomography showed diffuse rounded lesions with significant oedema. Antituberculosis chemotherapy was supplemented with

Brain abscesses are classical complications of congenital heart disease (CHD) in children and adolescents. This association is rarely observed in adults. We report a 46-year-old man presenting a fronto-parietal abscess associated with an asymptomatic atrial septal defect. Positron emission tomography (PET) study revealed high uptake of L-[methyl-11C]methionine ([11C]methionine) and 2-[18F]fluoro-2-deoxy-D-glucose (FDG) around the brain abscess. We suggest (1) to exclude a silent cardiac malformation in the presence of a cerebral abscess of unknown source occurring in adults; (2) to consider th…

We report a premature infant with Staphylococcus aureus sepsis, most probably originating from an infected peripheral iv site, and complicated by multiple brain abscesses. Diagnosis was made by cranial ultrasonography and computed tomography. Since systemic antibiotic treatment failed, surgical drainage was performed. The same organism that caused the initial sepsis grew from the pyogenic material obtained. The child is currently severely handicapped.

This paper deals with nocardiosis with brain abscess in a cynomolgus monkey imported from Indonesia. At necropsy, typical abscesses were found in the mandible, lung, kidney, heart, liver, and brain. Nocardia asteroides was isolated as the causative agent. The lesions in the kidney, heart, and brain were considered to be secondary metastases.

We present a case of multiple brain lesions in which the preoperative computerized tomographic (CT) scan suggested the presence of brain abscesses rather than metastatic tumors. The patient presented with symptoms of left frontal and right cerebellar mass lesions, confirmed by carotid and vertebral arteriography and a radionuclide brain scan. A CT scan revealed multiple contiguous ring-enhancing lesions of various sizes at both sites. With the exception of a prior mild pulmonary infection, no findings suggested underlying malignancy, immunosuppression, or medical illness. We based our decision…

BACKGROUND: Neurosurgical intervention is essential in the management of brain abscesses, both for optimising patient outcomes and obtaining samples. Aspiration of pus is increasingly favoured over excision, but with limited and diverging data on differences in outcome. Surgery facilitates microbiological diagnosis through cultures and molecular-based diagnostics such as polymerase chain reaction (PCR), expanding the microbiological spectrum, but with an uncertain impact on patient care.
OBJECTIVES: To provide data on outcomes after aspiration and excision of brain abscesses and to provide da…

Brain abscess is a focal intraparenchymal infection. Brain abscess breaking into ventricles is a potentially fatal complication of brain abscess, which can lead to sudden deterioration of neurological function. Its incidence rate is 0.3%-35.0%, and the mortality rate is 84.0%-100.0%. This paper reports a case of a 50-year-old male patient who had previously undergone intracranial hematoma evacuation and skull fixation for traumatic brain injury. He was admitted to the hospital with dizziness for 20 days and bradyphrenia for 4 days. After admission, enhanced computerized tomography(CT) and magn…

A 56-year-old man with a history of brain abscess undergoing a 68 Ga-labeled prostate-specific membrane antigen (PSMA) ligand PET/CT scan due to rising prostate-specific antigen levels. Besides the uptake of PSMA in the prostate lesion, the 68 Ga-PSMA PET/CT images showed significant PSMA uptake in the abscesses in the left parietal and left temporal lobes. This highlights the necessity of cautious interpretation of PSMA PET/CT results, considering alternative diagnoses like infections.

The European Society for Clinical Microbiology and Infectious Diseases recommends 3rd generation cephalosporins and metronidazole for empirical treatment of community-acquired brain abscesses. In 53 retrospectively analyzed pediatric patients with community-acquired brain abscesses at a German University Hospital Staphylococcus aureus was identified as a relevant pathogen (21%). Therefore, it may be reasonable to cover S. aureus when selecting empirical therapy.

INTRODUCTION: The annual incidence of brain abscesses is 1-2% in developed countries and up to 8% in developing countries. Our aim was to describe the profile and etiological agents of patients with surgical brain infections according to their nosological diagnosis on admission, and to analyze whether the initial diagnosis influenced the neurological deficit at discharge.
METHODOLOGY: This was an observational study with convenience sampling. All surgical cases operated between January 2017 and February 2022 with a final diagnosis of an infectious process were included. Three groups were anal…

Brain abscesses are localized infections in the brain's parenchyma, characterized by inflammation, pus formation, and the development of a surrounding capsule. These lesions typically occur due to underlying factors such as immunosuppression, ear and sinus infections, and contamination during neurosurgery. While brain abscesses are a life-threatening complication of cyanotic heart defects, they are rarely reported, with only sporadic cases previously documented. This article presents the case of an eight-year-old male patient with an uncorrected transposition of the great arteries, who was eva…
