Brain TXA(2) and PGI(2) levels after head injury with secondary insults.

OBJECTIVE: The brain TXA(2) and PGI(2) levels in a new rodent model of impact acceleration diffuse brain injury alone and with hypotention and hyperthermia in combination were observed to look into the relationship between TXA(2), PGI(2) levels and different types of head injury. METHODS: Thirty-two SD rats were randomized into sham, head injury alone, secondary insult alone and head injury with secondary insult groups. At 4 hours after injury or experiment, all the rats were decapitated and their brains were sampled for radioimmunoassay (RIA) measurement. RESULTS: Compared with that of sham g

Acute-onset delirium in intensive care COVID patients: association of imperfect brain repair with foodborne micro-pollutants.

BACKGROUND AND PURPOSE: COVID-19 affects the brain in various ways, amongst which delirium is worrying. An assessment was made of whether a specific, long-lasting, COVID-19-related brain injury develops in acute respiratory distress syndrome patients after life-saving re-oxygenation. METHODS: Ten COVID+ patients (COVID+) with unusual delirium associated with neuroimaging suggestive of diffuse brain injury and seven controls with non-COVID encephalopathy were studied. The assessment took place when the intractable delirium started at weaning off ventilation support. Brain magnetic resonance im

Delivering Traumatic Brain Injury to Larval Zebrafish.

We describe a straightforward, scalable method for administering traumatic brain injury (TBI) to zebrafish larvae. The pathological outcomes appear generalizable for all TBI types, but perhaps most closely model closed-skull, diffuse lesion (blast injury) neurotrauma. The injury is delivered by dropping a weight onto the plunger of a fluid-filled syringe containing zebrafish larvae. This model is easy to implement, cost-effective, and provides a high-throughput system that induces brain injury in many larvae at once. Unique to vertebrate TBI models, this method can be used to deliver TBI witho

Diffusion-weighted imaging in neonatal cerebral infarction: clinical utility and follow-up.

We describe the clinical utility of echo-planar diffusion-weighted imaging in neonatal cerebral infarction. Eight full-term neonates aged 1 to 8 days referred for neonatal seizures were studied. Patients were followed for a mean of 17 months with detailed neurologic examinations at regular intervals. Head computed tomography (CT) and conventional magnetic resonance (MRI) and diffusion-weighted images were obtained. Percent lesion contrast was evaluated for 19 lesions on T2-weighted and diffusion-weighted images. Follow-up conventional MRIs were obtained in seven patients. The findings on diffu

Early diffusion restriction of white matter in infants with small subdural hematomas is associated with delayed atrophy.

BACKGROUND: Traumatic brain injury (TBI) is a major cause of infant morbidity and mortality. In these patients, magnetic resonance imaging (MRI) including diffusion-weighted imaging (DWI) is the test of choice to describe the extent of microstructural injury. CASE PRESENTATION AND DISCUSSION: In this case series, we describe novel acute and chronic MRI findings in four infants (6-19 months) with small, unilateral subdural hematomas in whom the etiology of head injury was suspicious for non-accidental trauma (NAT). Acute (<1-week post-injury) DWI revealed extensive areas of restricted diffusio

Head Kinematics and Injury Metrics for Laboratory Hockey-Relevant Head Impact Experiments.

Investigating head responses during hockey-related blunt impacts and hence understanding how to mitigate brain injury risk from such impacts still needs more exploration. This study used the recently developed hockey helmet testing methodology, known as the Hockey Summation of Tests for the Analysis of Risk (Hockey STAR), to collect 672 laboratory helmeted impacts. Brain strains were then calculated from the according 672 simulations using the detailed Global Human Body Models Consortium (GHBMC) finite element head model. Experimentally measured head kinematics and brain strains were used to c

Postural Instability in Patients With Injury of Corticoreticular Pathway Following Mild Traumatic Brain Injury.

OBJECTIVE: We attempted to investigate postural instability in patients with injury of the corticoreticular pathway (CRP) after mild traumatic brain injury. METHODS: The CRP was reconstructed, and the fractional anisotropy value, apparent diffusion coefficient value, and fiber volume of the CRP were measured. For evaluation of postural instability, both the Balance Error Scoring System score and the displacement of center of pressure were measured. RESULT: Significantly lower tract volume of the CRP was observed in the patient group than in the control group with no significant difference in

Association Between Proteomic Blood Biomarkers and DTI/NODDI Metrics in Adolescent Football Players: A Pilot Study.

While neuroimaging and blood biomarker have been two of the most active areas of research in the neurotrauma community, these fields rarely intersect to delineate subconcussive brain injury. The aim of the study was to examine the association between diffusion MRI techniques [diffusion tensor imaging (DTI) and neurite orientation/dispersion density imaging (NODDI)] and brain-injury blood biomarker levels [tau, neurofilament-light (NfL), glial-fibrillary-acidic-protein (GFAP)] in high-school football players at their baseline, aiming to detect cumulative neuronal damage from prior seasons. An image is not always worth a thousand words: an image mimic of transient global amnesia.

Transient global amnesia (TGA) is a neurological syndrome with rather distinctive brain MRI features, namely hyperintense lesion in hippocampus on diffusion-weighted imaging (DWI) and fluid-attenuated inversion recovery (FLAIR) sequences. Post-traumatic amnesia is another amnestic syndrome which can also show hyperintense lesions in brain MRI due to cytotoxic oedema caused by traumatic brain injury. We present a case of a patient with post-traumatic amnesia with a brain MRI image mimic of TGA.

Detection of blast-related traumatic brain injury in U.S. military personnel.

BACKGROUND: Blast-related traumatic brain injuries have been common in the Iraq and Afghanistan wars, but fundamental questions about the nature of these injuries remain unanswered. METHODS: We tested the hypothesis that blast-related traumatic brain injury causes traumatic axonal injury, using diffusion tensor imaging (DTI), an advanced form of magnetic resonance imaging that is sensitive to axonal injury. The subjects were 63 U.S. military personnel who had a clinical diagnosis of mild, uncomplicated traumatic brain injury. They were evacuated from the field to the Landstuhl Regional Medica

Do human brain white matter and brain stem structures show direction-dependent mechanical behavior?

Since the corpus callosum and the brain stem are both vulnerable to diffuse axonal injury during head impacts, there is a high interest in modeling the mechanical behavior of these brain structures. In recent years, different versions of fiber-reinforced material models have been proposed for the corpus callosum and other white matter regions, as well as for the brain stem, even though there is currently no consensus on whether those structures exhibit a significant direction-dependent behavior during mechanical loading. Here, we present the first large-strain, multimodal experimental study on

Decreased uncinate fasciculus integrity in functional seizures following traumatic brain injury.

OBJECTIVE: The uncinate fasciculus (UF) has been implicated previously in contributing to the pathophysiology of functional (nonepileptic) seizures (FS). FS are frequently preceded by adverse life events (ALEs) and present with comorbid psychiatric symptoms, yet neurobiological correlates of these factors remain unclear. To address this gap, using advanced diffusion magnetic resonance imaging (dMRI), UF tracts in a large cohort of patients with FS and pre-existing traumatic brain injury (TBI + FS) were compared to those in patients with TBI without FS (TBI-only). We hypothesized that dMRI meas

Neurite orientation dispersion and density imaging in a rodent model of acute mild traumatic brain injury.

BACKGROUND AND PURPOSE: Identification of changesin brain microstructure following mild traumatic brain injury (mTBI) could be instrumental in understanding the underlying pathophysiology. The purpose of this study was to apply neurite orientation dispersion and density imaging (NODDI) to a rodent model of mTBI to determine whether microstructural changes could be detected immediately following injury. METHODS: Fifteen adult male Wistar rats were scanned on a Bruker 9.4 Tesla small animal MRI using a multi-shell acquisition (30 b = 1000 s/mm2 and 60 b = 2000 s/mm2 ). Nine animals experienced

Diffuse white matter injury and neurologic outcomes of infants born very preterm in the 1990s.

Neurocognitive outcomes of infants born very preterm (less than 32 weeks gestation) remain a major concern in perinatal practice. Very preterm birth rates have increased, with enhanced survival since 1990. As focal brain lesions become less common, diffuse injury to both gray and white matter is now the primary focus for improving neurologic outcomes in survivors. Recent evidence supports preoligodendrocytes as the principal cellular target of diffuse white matter injury due to their susceptibility to hypoxic-ischemic and inflammatory insults. An understanding of their development and vulnerab

Midline (central) fluid percussion model of traumatic brain injury in pediatric and adolescent rats.

OBJECTIVE Experimental traumatic brain injury (TBI) models hold significant validity to the human condition, with each model replicating a subset of clinical features and symptoms. TBI is the leading cause of mortality and morbidity in children and teenagers; thus, it is critical to develop preclinical models of these ages to test emerging treatments. Midline fluid percussion injury (FPI) might best represent mild and diffuse clinical brain injury because of the acute behavioral deficits, the late onset of behavioral morbidities, and the absence of gross histopathology. In this study, the auth

Information processing in severe disorders of consciousness: vegetative state and minimally conscious state.

OBJECTIVE: To study the presence of electrophysiological indicators of remaining cortical functions in patients with persistent vegetative state (PVS) and minimally conscious state (MCS). Previous electrophysiological and PET data indicated that some PVS patients have partially intact cortical processing functions. However, it remains unclear whether the reported patients were representative for PVS population or just some exceptional cases. METHODS: Event-related brain responses to stimuli of different complexity levels, recorded in 98 patients with extremely severe diffuse brain injuries, 5

Subtle Differences in Brain Architecture in Patients with Congenital Anosmia.

People suffering from congenital anosmia show normal brain architecture although they do not have functional sense of smell. Some studies in this regard point to the changes in secondary olfactory cortex, orbitofrontal cortex (OFC), in terms of gray matter volume increase. However, diffusion tensor imaging has not been explored so far. We included 13 congenital anosmia subjects together with 15 controls and looked into various diffusion parameters like FA. Increased FA in bilateral OFC confirms the earlier studies reporting increased gray matter thickness. However, it is quite difficult to int

True hemicranial decompression for severe pediatric cranial trauma: a short series of 4 cases and literature review.

BACKGROUND: Traumatic acute SDH in pediatric patients is a life-threatening situation. There is a severe increase in ICP caused by acute SDH or diffuse brain swelling or secondary to ischemic brain damage. In certain situations, conventional measures may fail to control such a rapid increase in ICP. CASE DESCRIPTION: The cases of 4 pediatric patients with cranial trauma with raised ICP, in whom hemicranial decompression was performed, are described. All patients had acute SDH with diffuse brain injury; in addition, 2 of them had associated massive infarcts. Three of them survived and had a fa
