Energy intake and body composition in anorexia and bulimia nervosa.

Energy intake during the weight gain phase and the weight maintenance phase was examined in three groups of inpatients: 64 anorectic restrictors (AN-R), 37 anorectic bulimics (AN-B), and 74 normal-weight bulimics (BN). The influence of body composition and other variables such as weight, exercise, and bingeing and purging frequencies on energy intake was analyzed. Eating disorder subgroups were found to differ in energy intakes to gain weight and to maintain weight within a target weight range. Anorectic restrictors consumed significantly more energy to gain weight than did the anorectic bulim

Milnacipran in the treatment of bulimia nervosa: a report of 16 cases.

Controlled trials in patients with bulimia nervosa have demonstrated efficacy of antidepressant medications with serotonergic function (e.g. fluoxetine) as well as noradrenergic function (e.g. desipramine). Sixteen out-patients with bulimia nervosa according to DSM-IV criteria were treated in a drug surveillance with 100 mg of milnacipran, a specific serotonin and noradrenaline reuptake inhibitor (SNRI). Ten patients completed the 8-week observation period. The reasons for premature attrition were improvement in one patient (no. 12), a generalized exanthema in one patient (no. 7), severe nause…

[Suicide and attempted suicide in anorexia nervosa and bulimia nervosa].

Suicide in anorexia nervosa and bulimia nervosa is a major cause of death. Risk factors for suicide and attempted suicide (which in many cases results in successful suicide) in anorexia nervosa include: purging type, chronic disease, and during treatment, obsessive symptoms and drug abuse, major depression, and for anorexia nervosa low body mass index (BMI) at presentation. In anorexia nervosa suicide has been considered the first cause of death and attempted suicide is a serious threat to these individuals. Data concerning suicide in bulimia nervosa has still scarce whereas attempted suicides…

A qualitative investigation of the relapse experiences of women with bulimia nervosa.

This study presents a qualitative analysis of relapse experiences of 26 women with bulimia nervosa who were recruited from Overeaters Anonymous. Using focus groups and individual interviews, participants reported conditions that activated binge eating and/or purging as triggers and described their attempts to manage triggers as an intense effort to arbitrate between high-risk circumstances and their capacity to avoid binging and purging. Responses revealed two broad categories of triggers: (1) internal emotional states and (2) interpersonal relationships. Sixty-one percent of the triggers were…

Binge and purge processes in bulimia nervosa: a qualitative investigation.

This study explored the nature and functions of the binge and purge processes of those who suffer from bulimia nervosa. Using a qualitative research design, the researchers conducted semi-structured interviews with eight affected women, asking them to discuss their bingeing and purging experiences. Use of an emergent data analysis method revealed participants behaviors as attempts to cope and control, to improve self-regard and social status, to regulate emotion, and to provide physiological reinforcement. Implications of the findings for clinical practice are discussed.

The cognitive-behavioral model of bulimia nervosa: a direct evaluation.

OBJECTIVE: This study represented the first attempt to directly evaluate Fairburn et al's (1986) cognitive-behavioral model of bulimia nervosa--the model on which the most widely used treatment for bulimia nervosa is based. METHOD: The major predictions of the model were tested using structural equation modeling. Data were collected from the responses of 526 subjects to a number of self-report measures. RESULTS: The factors of self-esteem, overconcern with weight and shape, and dietary restraint accounted for a large proportion of the variance in binge eating and purging. The key pathway in …

Randomized controlled trial of a treatment for anorexia and bulimia nervosa.

Evidence for the effectiveness of existing treatments of patients with eating disorders is weak. Here we describe and evaluate a method of treatment in a randomized controlled trial. Sixteen patients, randomly selected out of a group composed of 19 patients with anorexia nervosa and 13 with bulimia nervosa, were trained to eat and recognize satiety by using computer support. They rested in a warm room after eating, and their physical activity was restricted. The patients in the control group (n = 16) received no treatment. Remission was defined by normal body weight (anorexia), cessation of bi…

Self-harm and suicide attempts in individuals with bulimia nervosa.

Self-harm has been identified as a clinical feature in at least a subgroup of women with bulimia nervosa. We explored how women with bulimia who engage in self-harming behaviors differ from women with bulimia without self-harming behaviors and from women with bulimia who attempt suicide with lethal intent. We compared features of the eating disorder, Axis I and II comorbidity, and Eating Disorder Inventory and Temperament and Character Inventory (TCI) scores across 19 women with bulimia who engaged in self-harming behaviors, 28 women with bulimia who reported suicide attempts with lethal inten…

[Endocrine and reproductive disturbances in anorexia nervosa and bulimia nervosa].

Numerous endocrine abnormalities are associated with anorexia nervosa and bulimia nervosa. The principal complication is amenorrhoea. Hypothyroidism and hypercortisolism have been described as a protective mechanism to conserve energy. Growth hormone concentrations are often increased as a result of starvation. Insulin and blood sugar concentrations are decreased, but prolactin concentrations are remain normal. Considerable evidence exists of hypothalamic dysfunction in patients with eating disorders. This dysfunction is reflected in disturbances of endocrine function. Endocrine disturbances m…

Stage of change as a predictor of response to psychotherapy for bulimia nervosa.

OBJECTIVE: This study was designed to test the hypotheses that stage of change is a useful predictor of dropout and that it is related to treatment outcome in patients receiving brief psychotherapy for bulimia nervosa. METHOD: One hundred ten patients with bulimia nervosa were randomly assigned treatment with cognitive-behavioral therapy (CBT) or interpersonal therapy (IPT). On initial screening, patients were classified as being in the stage of precontemplation, contemplation, or preparation. RESULTS: Initial stage of change was not related to dropout either before or following randomizatio…

An integration of feminist and self-psychological approaches to bulimia nervosa.

Theoretical frameworks have guided approaches to treatment for the eating disorders. While unitary models were characteristic of earlier eras, more recent formulations have attempted to improve understanding by integrating various schools of thought (1). Several of these have placed cultural factors in the context of individual and familial predispositions. A weakness of these models has been the relative lack of understanding of the means by which these cultural values create mechanisms for expression of these disorders in specific women. In this manuscript I will explore the theoretical fram

Antidepressants versus psychological treatments and their combination for bulimia nervosa.

BACKGROUND: Psychotherapeutic approaches, mainly cognitive behavior therapy, and antidepressant medication are the two treatment modalities that have received most support in controlled outcome studies of bulimia nervosa. OBJECTIVES: The primary objective was to conduct a systematic review of all RCTs comparing antidepressants with psychological approaches or comparing their combination with each single approach for the treatment of bulimia nervosa. SEARCH STRATEGY: (1) electronic searches of MEDLINE (1966 to December 2000), EMBASE (1980-December 2000), PsycLIT (to December 2000), LILACS & S…

Validity of questionnaires for assessing dysfunctional cognitions in bulimia nervosa.

OBJECTIVE: This research investigated the validity of the Bulimic Cognitive Distortions Scale and the Mizes Anorectic Cognitions Questionnaire. The ability of these questionnaires to discriminate among individuals with bulimia, restrained eaters, and normals was also investigated. METHOD: Scores on these retrospective self-report questionnaires were compared to cognitions assessed via in vivo thought-sampling. Cognitions of 15 women with bulimia nervosa, 15 restrained eaters, and 15 non-eating disordered women were assessed. RESULTS: Results indicated that both the BCDS and the MAC successfu…

Impact of definitions on the description and prediction of bulimia nervosa outcome.

OBJECTIVE: The authors sought to compare definitions of eating disorder outcome found in the bulimia nervosa literature and to determine the impact of definitions on the description and prediction of outcome. METHOD: Definitions of outcome were identified from studies involving a follow-up duration of at least 5 years. Definitions were applied to a sample of women (N = 173) assessed more than 10 years following presentation with bulimia nervosa. RESULTS: Across definitions, the percentage of women considered fully recovered ranged from 38% to 47% in the follow-up sample. Associations between…

Assessment of ego state in anorexia nervosa and bulimia nervosa.

To clarify the effects of ego states on anorexia nervosa (AN) and bulimia nervosa (BN), the questionnaire of the Tokyo University Egogram (TEG) with critical parent, nurturing parent, adult, free child and adapted child scales was administered to 32 female AN patients aged 14 to 32 years (mean age 20 years) and 49 female BN patients aged 14 to 33 years (mean age 23 years). The scores on the critical parent and adapted child scales for AN were significantly lower than those for BN. A multiple logistic regression analysis including all of the TEG scales, age, duration of illness and education sh…

Life events and the onset of bulimia nervosa: a controlled study.

BACKGROUND: Life events may play a role in precipitating the onset of bulimia nervosa. This study aimed to determine the rates of occurrence of certain life events during the year before onset of disordered eating in women with bulimia nervosa, and to compare them with the rates among age-matched normal controls. METHODS: The subjects were 102 women with bulimia nervosa and 204 women without an eating disorder, all recruited from the same community sample of young adult women. Interview measures were used for diagnosis and for assessment of life events. RESULTS: The bulimia nervosa cases rep…

Factors influencing the outcome of group psychotherapy for bulimia nervosa.

OBJECTIVE: Group psychotherapy is a widely used form of treatment for bulimia nervosa, and many open and controlled studies have demonstrated that it is relatively effective. However, it is not clear what factors enhance or reduce that effectiveness. This review considers the potential role of a number of such factors. METHOD: The literature on the effectiveness of group psychotherapy was selected on the basis of its publication inclusion in peer-reviewed journals. Unpublished material was not included. Factors considered included theoretical orientation, therapy dose, concurrent treatment, c…
