Reliability exercise for the polymyalgia rheumatica classification criteria study: the oranjewoud ultrasound substudy.

Objective. A study supported by the EULAR and the ACR being conducted to establish classification criteria for polymyalgia rheumatica (PMR) will include ultrasound examination of the shoulders and hips. Ultrasound (US) depicts glenohumeral joint effusion, biceps tenosynovitis, subdeltoid bursitis, hip joint synovitis, and trochanteric bursitis in PMR. These findings may aid in distinguishing PMR from other diseases. The purpose of this study was to assess standards and US interreader agreement of participants in the PMR classification criteria study. Methods. Sixteen physicians in four groups …

Prepatellar and olecranon arthroscopic bursectomy.

Arthroscopic resection of prepatellar and olecranon bursae is a technically feasible operation. It is not more difficult than removing synovium from the suprapatellar pouch of the knee. Although there have been complications, this procedure appears to avoid the problems about the wound described with the open excisional operations for chronic olecranon and prepatellar bursitis.

Disseminated protothecosis in a Mexican child.

Protothecosis is a rare human infection. There are 3 clinical forms: cutaneous, olecranon bursitis and systemic. Here we present the first case of disseminated protothecosis in Mexico in an immunocompetent girl whose lesions were mostly dermatologic. The patient was successfully treated with amphotericin B colloidal dispersion and gentamicin, followed by itraconazole.

Chronic Brucella infection of the supra-patellar bursa with sinus formation.

A case of supra-patellar bursitis with the formation of discharging sinus is described. B abortus was isolated from resected bursal tissue. While osteoarticular complications of brucellosis are common and a number of different clinical syndromes are now recognised, disease of the bursae is rare and as far as is known sinus formation has not been described before in this setting.

Greater trochanteric pain syndrome, another cause of hip or thigh pain in multiple sclerosis.

This case report describes greater trochanteric pain syndrome (often referred to as trochanteric bursitis) in a patient with multiple sclerosis (MS). It is a relatively common and treatable cause of hip pain in patients with several underlying conditions. It is probably underdiagnosed in MS where other causes of pain, particularly neurogenic, are so often assumed.

The peritrochanteric space of the hip.

The field of hip arthroscopy continues to grow as surgeons become more familiar with disorders of the intra-articular and extra-articular compartments of the hip. Recent studies have described the endoscopic treatment of injuries to the lateral peritrochanteric compartment of the hip, including recalcitrant trochanteric bursitis, external coxa saltans, and tears of the gluteus medius and minimus tendons.

Rhabdomyolysis in DOMS.

A 26-year-old man with a previous history of external twin bursitis was remitted to our Department for a bone scintigraphy. Before the study, the patient performed an elevated number of intense sprints. Bone scintigraphy showed a bilaterally increased activity in both anterior rectum muscles suggesting rhabdomyolysis. Biochemical studies and MRT confirmed the diagnosis.

[Radiotherapy in degenerative-dystrophic and inflammatory diseases of the osteoarticular system].

Analysis of a 3-6-year follow-up of 565 patients with deforming arthrosis, spondylosis, osteochondrosis, spondylarthrosis and calcareous bursitis indicates the efficacy of the proposed standardized complex of topometric and roentgenotherapeutic methods. Medical and social rehabilitation was achieved in most of the patients. The quantitative assessment of the therapeutic results makes it possible to interpret them objectively and unambiguously.

Fat suppression magnetic resonance imaging in shoulders of patients with polymyalgia rheumatica.

OBJECTIVE: To evaluate the sites of inflammatory process in the shoulders of patients with polymyalgia rheumatica (PMR) using fat suppressed magnetic resonance imaging (MRI). METHODS: Six consecutive, untreated new patients with PMR were investigated. Five patients with early rheumatoid arthritis (RA) and 4 patients with early psoriatic arthritis (PsA) with bilateral shoulder symptoms served as a control group. Bilateral shoulder fat-suppressed MRI sequences were performed in all patients and controls. We evaluated the presence of joint synovitis, bursitis, tenosynovitis, and bone and soft ti…

Tuberculous Distal Biceps Tendon Rupture: Case Report and Review of the Literature.

Tuberculous distal biceps tendon rupture is a rare condition in orthopedics. Musculoskeletal tuberculosis usually presents with bursitis, synovitis, myositis, and osteomyelitis, conditions which demonstrate an excellent response to antituberculosis chemotherapy. Tendon rupture is often associated with delayed diagnosis and treatment. We report a rare manifestation of musculoskeletal tuberculosis in the distal biceps tendon with delayed diagnosis.

Calcific tendinitis of the rectus femoris.

We report 3 patients with acute calcific tendinitis and bursitis of the rectus femoris muscle masquerading hip joint pain. Typically, there is tenderness and warmth lateral to the hip just below the anterior inferior iliac spine. Calcification can be seen on x-rays and may resolve as pain resolves with local injections and antiinflammatory drugs.

Adhesive capsulitis: a review.

Adhesive capsulitis is a common, yet poorly understood, condition causing pain and loss of range of motion in the shoulder. It can occur in isolation or concomitantly with other shoulder conditions (e.g., rotator cuff tendinopathy, bursitis) or diabetes mellitus. It is often self-limited, but can persist for years and may never fully resolve. The diagnosis is usually clinical, although imaging can help rule out other conditions. The differential diagnosis includes acromioclavicular arthropathy, autoimmune disease (e.g., systemic lupus erythematosus, rheumatoid arthritis), biceps tendinopathy, …

Ultrasound of the elbow.

Ultrasound examination of the elbow should be performed in a comparative way, exploring joint spaces and fossas. Ultrasound is the method of choice for the detection of joint fluid and loose bodies. Tendinitis and microfissures can be demonstrated in case of tennis and golfer elbow. On the posterior aspect of the elbow, ultrasound shows the different types of bursitis, and allows evaluation of the ulnar nerve in cases of impingement.

Trochanter fixation with the Dutchman's hook.

For nearly 30 years we have been using a special hook for reattachment of the trochanter. Its use has been evaluated in 80 hip replacements. Non-union was seen in one case. Loss of trochanteric position occurred in 9% of the hips. Trochanteric bursitis was a frequent complication (10%) and six patients had the hooks removed on that account.

Iliotibial band bursitis and Achilles paratenonitis were however aggravated, and mild semitendinosus bursitis was a novel finding during the examination after running 8000 km in 100 days. These results are impressive because preparation for this 8000 km running was only ~5 …

Intramuscular myxoid lipoma in the proximal forearm presenting as an olecranon mass with superficial radial nerve palsy: a case report.

BACKGROUND: Extremity lipomas may occur in any location, including the proximal forearm. We describe a case of a patient with an intramuscular lipoma presenting as an unusual posterior elbow mass. CASE PRESENTATION: We discuss the case of a 57-year-old Caucasian man who presented with a tender, posterior elbow mass initially diagnosed as chronic olecranon bursitis. A minor sensory disturbance in the distribution of the superficial radial nerve was initially thought to be unrelated, but was likely caused by mass effect from the lipoma. No pre-operative advanced imaging was obtained because the…

[Technique and value of arthrosonography in rheumatologic diagnosis. 2: Ultrasound diagnosis of the hip area].

The clinical investigation of the hips in patients with rheumatic diseases is often equivocal. Thus, ultrasonography of this region is very relevant for rheumatologists. We suggest following standard scans: 1) anterior longitudinal scan to detect synovitis of the hip joint, iliopectineal bursitis, irregularities of the bone surface in osteoarthritis, Perthes' disease, and erosions due to inflammatory disease, 2) anterior transverse scan to evaluate these structures in an additional dimension, 3) lateral longitudinal scan of the hip joint with the same objective as the above mentioned scans; 4)…

Ultrasound evaluation of bursae: anatomy and pathological appearances.

A bursa is an extra-articular sac that may communicate with a joint and functions to decrease friction between tendons and either bone or skin. Bursae can be classified as native and non-native (adventitious) bursae. The native bursae are lined with a synovial membrane and occur at predictable anatomical sites; knowledge of these normal structures can help distinguish them from other pathological entities. An adventitious bursa can form at sites of friction rather than at predictable anatomical sites, but otherwise have imaging features similar to native bursae. Bursal distention can occur fro
