Cardiac metabolism in diabetes mellitus.

Diabetes mellitus is one of the most common chronic illnesses throughout the world. Diabetic cardiomyopathy is a specific syndrome, consisting of cardiomegaly, left ventricular dysfunction, electrical remodeling of the ventricle, and symptoms of congestive heart failure, that is seen in diabetic patients in the absence of other predisposing factors. Many researchers have suggested that inhibition of the renin-angiotensin-aldosterone system and the sympathetic nervous system may exert a therapeutic effect in individuals with diabetic cardiomyopathy. Indeed, angiotensin II and aldosterone blocka…

High-output heart failure caused by arteriovenous fistula long after nephrectomy.

A 70-year-old Japanese woman was admitted to our hospital because of anasarca. At 32 years of age, she had undergone nephrectomy for renal tuberculosis. A continuous abdominal bruit was heard. The chest X-ray showed cardiomegaly and dilatation of the pulmonary artery. Abdominal three-dimensional computed tomography scanning clearly revealed an arteriovenous fistula. Cardiac catheterization disclosed cardiac output of 9.2 l/min and a step-up of oxygen saturation at the renal vein level of the inferior vena cava. Surgical closure of the fistula promptly decreased her cardiac output and improved …

Direct pericardial involvement of non-small cell lung cancer rapidly developing pericardial constriction.

A 71-year-old male, who had been followed up after being treated with chemo-radiotherapy for non-small cell lung cancer (adenocarcinoma), rapidly developed dyspnea and mild fever. Radiographs showed left pleural effusion and cardiomegaly, and echocardiographic examination revealed echo-free space, suggesting a pericardial effusion. The patient was treated conservatively without any surgical procedures such as pericardiocentesis. Disappearance of the echo-free space was followed by development of pericardial constriction within two months. At post-mortem examination, a direct extension to the p…

How to do a mitral valvuloplasty.

Since the first publication on Percutaneous Mitral Commissurotomy (PMC) in 1984 there has been a considerable evolution in the technique and a large number of patients have now been treated. PMC is an effective treatment in a wide range of patients with mitral stenosis. Its risk is low in a high volume center. It provides good mid term results. Age, previous surgery, anatomy and procedural factors are predictors of immediate results. Anatomy, NYHA class, degree of cardiomegaly, and the quality of initial results are predictors of the mid-term results. Consequently the selection of the candidat…

The role of the adrenals in copper deficiency.

The present study was undertaken in order to establish whether (1) a decrease in catecholamines will prevent the heart hypertrophy of copper … ction in catecholamine concentration did not protect the copper-deficient rats fed fructose against cardiomegaly and mortality. The data support the contention that the severity of copper deficiency in rats fed fructose is not solely dependent on hepatic copper concentration and/or levels of catecholamines.

Cardiac adaptation to exercise training in health and disease.

The heart is the primary pump that circulates blood through the entire cardiovascular system, serving many important functions in the body. Exercise training provides favorable anatomical and physiological changes that reduce the risk of heart disease and failure. Compared with pathological cardiac hypertrophy, exercise-induced physiological cardiac hypertrophy leads to an improvement in heart function. Exercise-induced cardiac remodeling is associated with gene regulatory mechanisms and cellular signaling pathways underlying cellular, molecular, and metabolic adaptations. Exercise training al

Infantile hypertrophic cardiomyopathy masquerading as cardiac tamponade: A case report.

Hypertrophic cardiomyopathy in infancy often follows a rapidly progressive clinical course, with many cases becoming fulminant and fatal. We reported a case of a 4-month-old infant with worsening breathlessness. While admitted, she was in severe respiratory distress and appeared drowsy. We also noted indiscernible heart sounds from physical examination and cardiomegaly with a globular cardiac silhouette from chest X-ray. Considering her distressing clinical presentation, we suspected cardiac tamponade and performed an emergent transthoracic echocardiography to confirm our suspicion. Echocardio…

Respiratory Viral Infection Unmasking Congenital Heart Disease.

Congenital heart disease affects nearly 1% of all live births or 40,000 infants each year. Anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) is a rare congenital heart defect that constitutes 0.25% to 0.50% of all congenital cardiac diseases. If left untreated, infant mortality is as high as 90%. The following is a case report of a 4-month-old infant presenting with respiratory viral illness and a chest radiograph demonstrating cardiomegaly. Echocardiography revealed possible ALCAPA with severe left ventricular (LV) dysfunction. The diagnosis was confirmed using g…

Fetal brain small vessel disease 1 caused by a novel mutation in the COL4A1 gene.

A singleton fetus was referred to fetal magnetic resonance imaging (MRI) at 25 weeks due to mild ventriculomegaly and an abnormal fetal echocardiogram showing cardiomegaly, right ventricular hypertrophy and tricuspid insufficiency. Patchy areas of ischemic infarction, extensive subacute and chronic hemorrhage not respecting vascular territories, encephaloclastic cysts and closed lip schizencephaly were identified. Cataract was detected postnatally. The anomalies were caused by a pathogenic mutation (c.353 G>A; p.G118D) in the COL4A1 gene. The phenotype seen in this case, i.e. small vessel cere…

Noonan syndrome associated with anomalous left coronary artery from the pulmonary artery in a patient with the rare RAF1 mutation: A case report and review of literature.

We present the case of a 7-week-old male infant diagnosed with anomalous left coronary artery from the pulmonary artery (ALCAPA) who underwent repair by left coronary artery reimplantation, followed by an eventful postoperative period including need for venous arterial extracorporeal membrane oxygenation and mitral valve replacement due to mitral calcification and severe insufficiency. He also required heart transplant due to severe rapidly progressive biventricular hypertrophy. The pathology examination of the explanted heart showed massive cardiomegaly. Subsequently, the infant's cardiomyopa…

Mutation of KCNJ8 in a patient with Cantú syndrome with unique vascular abnormalities - support for the role of K(ATP) channels in this condition.

KCNJ8 (NM_004982) encodes the pore forming subunit of one of the ATP-sensitive inwardly rectifying potassium (KATP) channels. KCNJ8 sequence variations are traditionally associated with J-wave syndromes, involving ventricular fibrillation and sudden cardiac death. Recently, the KATP gene ABCC9 (SUR2, NM_020297) has been associated with the multi-organ disorder Cantú syndrome or hypertrichotic osteochondrodysplasia (MIM 239850) (hypertrichosis, macrosomia, osteochondrodysplasia, and cardiomegaly). Here, we report on a patient with a de novo nonsynonymous KCNJ8 SNV (p.V65M) and Cantú syndrome, w…

[The importance of determination of Nt-proBNP and big endothelin in diagnosing chronic heart failure in patients on regular haemodialysis treatment].

We have followed 99 patients with end stage renal failure, treated by regular haemodialysis. Chronic renal failure is frequently accompanied by chronic heart failure (over 50%), especially by heart failure with preserved ejection fraction. Patients treated by regular haemodialysis had a tendency to cardiomegaly (51%), mild systolic dysfunction of the left ventricle (mean LVEF 53%) and diastolic dysfunction (88%) of the hypertrophic left ventricle. They had also activated endothelin and neurohumoral system. Only 3% of the patients had normal values of Nt-proBNP and big endothelin. The plasma le…

Primary pulmonary arterial hypertension presenting as diffuse micronodules on CT.

Primary pulmonary arterial hypertension is a rare lethal disease that typically presents radiographically with enlarged central pulmonary arteries, pruning of the peripheral vasculature, and cardiomegaly but clear lung fields. Although it is a disease of unknown etiology, primary PAH has been associated with anorexigen use. We present a case of pulmonary arterial hypertension in a woman with a history of fenfluramine and phentermine use who presented with diffuse micronodules on computed tomography scan. Lung biopsy confirmed the micronodules were radiographic manifestations of extensive diffu…

[Successful repair of right atrial rupture due to nonpenetrating trauma of the chest].

A 24-year-old male was brought to our hospital after being injured in a traffic accident. On arrival, his blood pressure was 70/44 mmHg and his pulse rate was 135/min and regular. Chest X-p revealed cardiomegaly but there was no pleural effusion or bone fracture. Echocardiogram revealed cardiac tamponade and he was diagnosed as cardiac rupture due to non penetrating trauma. Under midline sternotomy, right atrial rupture was repaired. The patient developed cardiac arrest lasting approximately 8 minutes during anesthetic induction, so he needed to be ventilated for 8 days. However his postoperat…
