Neuroprotective effects of acetyl-L-carnitine on neuropathic pain and apoptosis: a role for the nicotinic receptor.

Several pathologies related to nervous tissue alterations are characterized by a chronic pain syndrome defined by persistent or paroxysmal pain independent or dependent on a stimulus. Pathophysiological mechanisms related to neuropathic disease are associated with mitochondrial dysfunctions that lead to an activation of the apoptotic cascade. In a model of peripheral neuropathy obtained by the loose ligation of the rat sciatic nerve, acetyl-L-Carnitine (ALCAR; 100 mg/kg intraperitoneally [i.p.] twice daily for 14 days) was able to reduce hyperalgesia and apoptosis. In the present study, differ…

Response to therapy in carnitine/acylcarnitine translocase (CACT) deficiency due to a novel missense mutation.

Deficiency of carnitine/acylcarnitine translocase (CACT) is an autosomal recessive disorder of the carnitine cycle resulting in the inability to transfer fatty acids across the inner mitochondrial membrane. Only a limited number of affected patients have been reported and the effect of therapy on this condition is still not well defined. Here, we report a new patient with this disorder and follow the response to therapy. Our patient was the product of a consanguineous marriage. He presented shortly after birth with cardiac myopathy and arrhythmia coupled with severe non-ketotic hypoglycemia. I…

Carnitine levels in patients with skeletal myopathy due to anorexia nervosa before and after refeeding.

OBJECTIVE: To assess the role of carnitine in the skeletal myopathy present in anorexia nervosa. METHOD: Serum levels of free and total carnitine were measured in a group of severely underweight women with anorexia nervosa and skeletal myopathy before and after an inpatient refeeding program. RESULTS: Carnitine levels were within the reference range before refeeding and remained unchanged despite significant weight gain in all the subjects. CONCLUSION: These findings suggest that carnitine plays no part in the muscle weakness seen in severe anorexia nervosa.

Vitamin C depletion is associated with alterations in blood histamine and plasma free carnitine in adults.

OBJECTIVE: The purpose of this study was to determine whether carnitine metabolism or histamine degradation would be useful parameters for investigating the optimal requirement for vitamin C. METHODS: Twenty-two non-scorbutic subjects with subnormal vitamin C status (plasma vitamin C < 28 mumol/L) were placed on a metabolic diet low in vitamin C for 3 weeks and repleted with graded doses of vitamin C: 10, 30 and 60 mg vitamin C daily (group 1) or 10,125 and 250 mg vitamin C daily (group 2) for weeks 1, 2 and 3, respectively. Fasting blood samples were collected weekly and analyzed for plasma …

Identification of a common mutation in the carnitine palmitoyltransferase II gene in familial recurrent myoglobinuria patients.

Carnitine palmitoyltransferase (CPT) II deficiency is the most common inherited disorder of lipid metabolism affecting skeletal muscle. We have identified a missense mutation (Ser113Leu) in one patient with the classical muscular symptomatology. Transfection experiments in COS cells demonstrate that the mutation drastically depresses the catalytic activity of CPT II. The mutation results in normal synthesis but a markedly reduced steady-state level of the protein, indicating decreased stability of mutant CPT II. The Ser113Leu mutation is the most frequent cause of CPT II deficiency. The mutati…

Effects of ammonium chloride, salicylate, and carnitine on palmitic acid oxidation in rat liver slices.

To explore the possible association of hyperlipidemia with hyperammonemia and aspirin ingestion, the effects of NH4+, salicylate, and carnitine on the oxidation of [1-14C]palmitic acid to acid-soluble products (ASP) and to CO2 were investigated in rat liver slices. DL-carnitine (5 mM) increased total oxidation (ASP + CO2) more than oxidation to CO2. KCN (1.5 mM) inhibited more than 90% of the oxidation. NH4Cl inhibited the oxidation that reached a maximum at about 40 mM, but the inhibition of oxidation to CO2 (63%) was larger than that of total oxidation (30%). Carnitine did not influence NH4+…

[Recurrent rhabdomyolysis and deficiency of carnitine palmityl transferase. Evidence of the responsibility of a mutant in 2 brothers].

In two brothers who had been presenting since childhood with intermittent muscular attacks and myoglobinuria after sustained efforts, muscle carnitine palmityl transferase (CPT) activity was undetectable by the backward hydroxamate colorimetric method. Such a total deficiency could not easily fit in with the clinical features (normal muscular activity outside sustained efforts), with the normal results obtained at electromyography and with the moderate fatty overload detected at muscle biopsy. In order to elucidate these apparently discordant findings, another CPT measurement method, the forwa…

N-acetylcysteine, Acetyl-L-carnitine, and Citicoline: A Potential Synergism in Neurological and Psychiatric Disorders.

The cellular oxidative balance is finely regulated by glutathione (GSH) levels and its reduced form. N-acetylcysteine (NAC) is an antioxidant agent that reduces disulphide bonds, scavenges reactive oxygen species (ROS), and serves as a precursor for GSH biosynthesis. Moreover, NAC can modulate glutamatergic transmission in the central nervous system (CNS), stimulating the system xc- activity and thus enhancing the endogenous activation of metabotropic glutamate receptors type 2 and 3 presinaptically, which restrains the excessive release of glutamate. Acetyl-Lcarnitine (ALC) is an acetylated f…

Dose-dependent modulation of mitochondrial physiology and nutrient secretion in Sertoli cells by l-carnitine.

l-Carnitine is a ubiquitous quaternary amine, essential for the mitochondrial β-oxidation of long-chain fatty acids, and a dietary supplement widely consumed by athletes due to its purported ergogenic and recovery-enhancing properties. Beyond its metabolic role in muscle tissue, l-Carnitine is highly concentrated in the male reproductive tract, suggesting a critical function in spermatogenesis. In the testis, Sertoli cells (SCs) act as essential "nurse cells," providing structural, nutritional, and immunomodulatory support to developing germ cells. This study elucidates the effects of l-Carnit…

Effects of L-carnitine over-supplementation on spermatogenesis and sperm function in healthy NMRI mice.

L-carnitine (LC) plays a key role in lipid metabolism by transporting fatty acids from the cytosol into mitochondria for energy production. Beyond its metabolic function, LC exhibits significant antioxidant, anti-cytokine, and anti-apoptotic properties, contributing to cellular protection. This study investigates the potential adverse effects of four weeks of LC supplementation (20, 100, and 500 mg/kg body weight, n = 10) on spermatogenesis and sperm function in healthy NMRI male mice. Sperm parameters-motility, concentration, and morphology-were assessed alongside sperm and testicular oxidati…

Heart failure treatment and serum carnitines in HFpEF and HFrEF with or without hypertension - a pilot study.

The ratio of serum acylcarnitines to free carnitines (AC/FC) reflects impaired cardiomyocyte β-oxidization. The effect of heart failure (HF) treatment on AC/FC remained unclear. This pilot study retrospectively examined treatment-induced AC/FC changes in patients with HF with preserved ejection fraction (HFpEF) and HF with reduced ejection fraction (HFrEF) in 148 consecutive inpatients undergoing echocardiography and carnitine measurement at admission and discharge. Symptomatic ACCF/AHA HF stage C/D patients (HFpEF, n = 40; HErEF, n = 24) received standard medical therapy and comprehensive car…

An Amino Acid and Carnitine Metabolite Profile for the Early Detection and Differential Diagnosis of Alzheimer's Disease.

As the most common type of dementia, Alzheimer's disease (AD) often presents challenges in terms of early identification. In particular, there is a notable lack of reliable and accessible biomarkers. To measure the levels of amino acids and carnitine metabolites in the peripheral blood of AD patients to identify cognitive impairment-associated metabolites, two cohorts were recruited in this cross-sectional study from September 2018 to October 2023. Serum amino acid and carnitine levels were measured using liquid chromatography-mass spectrometry. The test cohort (normal cognition (NC), n = 70; …

Modulation of Carnitine Palmitoyl Transferase 1b Expression and Activity in Muscle Pathophysiology in Osteoarthritis and Osteoporosis.

In the pathophysiology of osteoarthritis and osteoporosis, articular cartilage and bone represent the target tissues, respectively, but muscle is also involved. Since many changes in energy metabolism occur in muscle with aging, the aim of the present work was to investigate the involvement of carnitine palmitoyl transferase 1b (Cpt1b) in the muscle pathophysiology of the two diseases. Healthy subjects (CTR, n = 5), osteoarthritic (OA, n = 10), and osteoporotic (OP, n = 10) patients were enrolled. Gene expression analysis conducted on muscle and myoblasts showed up-regulation of CPT1B in OA pa…
