Effects of spaceflight on cartilage: implications on spinal physiology.

Spaceflight alters normal physiology of cells and tissues observed on Earth. The effects of spaceflight on the musculoskeletal system have been thoroughly studied, however, the effects on cartilage have not. This area is gaining more relevance as long duration missions, such as Mars, are planned. The impact on intervertebral discs and articular cartilage are of particular interest to astronauts and their physicians. This review surveys the literature and reports on the current body of knowledge regarding the effects of spaceflight on cartilage, and specifically changes to the spine and interve…

[Progress on diagnosis and treatment of cartilage injury of the talus].

The diagnosis of osteochondral lesions of the talus can be based on the patients' symptoms, medical history, MRI, detailed physical examination and arthroscopy. Its treatment includes conservative treatment and surgical treatment. Conservative treatment includes rest, partial-weight bearing, plaster immobilization, wear the orthopedic support, take the non-steroidal anti-inflammatory drugs and so on. Conservative treatment is less risky and effective for mild injuries. Surgical treatment includes debridement, marrow stimulation, internal fixation and cartilage transplantation, autologous chond…

Integrating Morphology and Genetics in the Diagnosis of Cartilage Tumors.

Cartilage-forming tumors of bone are a heterogeneous group of tumors with different molecular mechanisms involved. Enchondromas are benign hyaline cartilage-forming tumors of medullary bone caused by mutations in IDH1 or IDH2. Osteochondromas are benign cartilage-capped bony projections at the surface of bone. IDH mutations are also found in dedifferentiated and periosteal chondrosarcoma. A recurrent HEY1-NCOA2 fusion characterizes mesenchymal chondrosarcoma. Molecular changes are increasingly used to improve diagnostic accuracy in chondrosarcomas. Detection of IDH mutations or HEY1-NCOA2 fusi…

Impact of cartilage graft size on success of tympanoplasty.

INTRODUCTION: In the last decade, there has been an increasing use of cartilage grafts in the primary repair of tympanic membrane perforations. The major advantages of cartilage are its stiffness and its very low metabolic requirements, which make it particularly suitable for difficult conditions, such as subtotal perforations, adhesive otitis and reoperation. OBJECTIVE: To analyze the impact of different perforation sizes requiring different sizes of cartilage on the anatomical and functional outcome after tympanoplasty. METHODS: Through this prospective non-controlled, non-randomized study…

[Update on the treatment of articular cartilage lesions in the knee].

Despite articular cartilage lesions being common, there is no evidence-based treatment protocol for them. The preoperative assessment must diagnose all of the concurrent pathologies that need to be treated at the same time. Several types of treatments exist, and each has its advantages and disadvantages. First-line therapy consists of well-conducted conservative treatment. Surgical treatment is the second choice. It is chosen according to the size of the lesion, its depth, damage to the subchondral bone, and the condition of the neighboring cartilage. The few studies that have compared the dif…

The Application of Sheet Technology in Cartilage Tissue Engineering.

Cartilage tissue engineering started to act as a promising, even essential alternative method in the process of cartilage repair and regeneration, considering adult avascular structure has very limited self-renewal capacity of cartilage tissue in adults and a bottle-neck existed in conventional surgical treatment methods. Recent progressions in tissue engineering realized the development of more feasible strategies to treat cartilage disorders. Of these strategies, cell sheet technology has shown great clinical potentials in the regenerative areas such as cornea and esophagus and is increasing…

Extramedullary relapse of multiple myeloma in the thyroid cartilage.

Multiple myeloma involving the thyroid cartilage is a very uncommon disease entity. Only 10 cases have been reported in the international literature to date and as such it constitutes a rare and challenging diagnosis to make. We report the case of a 63-year-old man with a background of malignant melanoma and multiple myeloma both of which were in remission at the time of presentation. The patient presented with hoarseness and a large neck mass that turned out to be an extramedullary deposit of his previously treated multiple myeloma indicating disease relapse. The differential diagnosis of a m…

Is the tragal cartilage necessary for type 1 tympanoplasties?

The use of cartilage grafts in tympanoplasties is recommended in cases with a higher probability of failure. Many grafting techniques have been reported, including palisade, cartilage island, and cartilage shield grafts. Cartilage minimizes the inflammatory tissue reaction, ensures resistance against infection during the recovery process, and provides good support against the retraction by virtue of its rigidity. This study compared the anatomic and functional results of conchal cartilage, tragal cartilage, and temporal muscle fascia grafts in primary type 1 tympanoplasty. Between January 2007 …

Viscoelasticity of the articular cartilage surface in early osteoarthritis.

OBJECTIVE: Structural and biochemical changes in articular cartilage occur throughout the pathogenesis of osteoarthritis (OA). Early changes include proteoglycan loss and collagen network disorganization at or near the articular surface. These changes accompany reductions in mechanical properties of cartilage, yet the relationships between mechanics and structure in early OA are poorly defined. Thus, the overall goal of this work was to measure changes in the microscale mechanics and structure of the articular surface in an in vivo model of OA to better understand the early pathogenesis of car…

A bare area of the glenoid misdiagnosed as a cartilage ulceration.

We report on a 17-year-old girl who developed shoulder pain after a fall on an outstretched arm. CT arthrography and MR imaging demonstrated a cartilage defect centrally located in the glenoid. This was accompanied by an eroded appearance of the underlying bone. Since symptoms persisted over several months it was initially thought this represented a cartilage injury. Subsequently arthroscopy was performed and the abnormality was identified as a bare area of the glenoid. A tear of the superior glenohumeral ligament was depicted and was repaired. The bare area is an oval area denuded of cartilag…

An investigation of the fixation materials for cartilage frames in microtia.

UNLABELLED: When performing auriculoplasty for microtia surgery, wires are typically used to fix the costal cartilage frames. However, cases in which such wires become exposed during a long-term follow-up were frequently observed at our facility. Hence, using various materials, we conducted an investigation of the materials most suitable for fixation. METHOD: The subjects consisted of 122 cases in which auriculoplasty by costal cartilage graft surgery was performed and the postoperative course was traceable, within approximately 24 years from January 1984 to March 2007. Regarding the fixation…

[Repair of local cartilage defects in the patellofemoral joint].

Cartilage defects of the patellofemoral joint represent different entities. Results of patellofemoral cartilage repair are often variable. Concomitant pathologies, such as malalignment and instability are frequent. Currently no common treatment concept exists. Improvement of patellofemoral biomechanics, e.g. tubercle transfer, results in better outcome independent of the cartilage repair technique used. In patellofemoral instability reconstruction of the medial patellofemoral ligament (MPFL) has recently become a focus of interest. Microfracturing and osteochondral cylinder transfer/mosaicplas…

Surgical management of articular cartilage defects in the knee.

The treatment of isolated cartilage lesions of the knee is based on several underlying principles, including a predictable reduction in the patient's symptoms, improvements in function and joint congruence, and prevention of progressive damage. Surgical options for cartilage restoration are described as palliative treatments, such as débridement and lavage; reparative, such as marrow stimulation techniques; or restorative, such as osteochondral grafting and autologous chondrocyte implantation. The choice of an appropriate treatment should be made on an individual basis, with consideration for …
