Saccharomyces cerevisiae biofilm tolerance towards systemic antifungals depends on growth phase.

BACKGROUND: Biofilm-forming Candida species cause infections that can be difficult to eradicate, possibly because of antifungal drug toleran … y reduced by treatment of mature biofilms with amphotericin B but not voriconazole, flucytosine, or caspofungin. We showed that metabolic activity in yeast biofilm cells decreased with time, as visualized by FUN-1 staining, and mature, 48-hour biofilms contained cells with slow metabolism and limited growth. Time-kill studies showed that in exponentially grow …

Paradoxical antifungal activity and structural observations in biofilms formed by echinocandin-resistant Candida albicans clinical isolates.

Echinocandin-resistant clinical isolates of Candida albicans have been reported, and key-hot spot mutations in the FKS1 gene, which encodes a major glucan synthase subunit, have been identified in these (caspofungin-resistant [CAS-R]) strains. Although these mutations result in phenotypic resistance to echinocandins in planktonic cells, there is little data on antifungal susceptibilities of CAS-R C. albicans strains within biofilms. Thus, we analyzed biofilms formed by 12 C. albicans CAS-R clinical strains in which we previously identified FKS1 hot-spot mutations and compared the sessile antif…

METHODS: Voriconazole-treated patients with … ole was used in combination (response 69 vs. 64 % alone) with topical [amphotericin B 10/24 (42 %), caspofungin 5 (21 %), natamycin 1 (4 %)] and systemic agents [caspofungin 3 (13 %), amphotericin 2 (8 %)]. Aspergillus nidulans galactofuranose biosynthesis affects antifungal drug sensitivity.

The cell wall is essential for fungal survival in natural environments. Many fungal wall carbohydrates are absent from humans, so they are a … heir repression by alcA(p) or niiA(p), OR, ugmA overexpression by alcA(p), increased sensitivity to Caspofungin. Strains with alcA(p)-mediated overexpression of ugeA and ugtA had lower caspofungin sensitivity. Galf appears to play an important role in A. nidulans growth and vigor.

A Rare Case of Fungal Burn Wound Infection Caused by Fusarium solani in Vietnam.

A patient with extensive burn injuries was admitted to the National Hospital of Burns in Hanoi, Vietnam, and diagnosed with fungal wound infection by histological examination of skin biopsy samples. Fusarium solani was isolated and identified by analysis of its morphological features and the sequence of the internal transcribed spacer region. The isolation showed in vitro resistant to fluconazole, voriconazole, itraconazole, amphotericin B, and caspofungin. Invasive fusariosis is difficult to treat due to its angioinvasive property and its lacking amenability to treatment with antifungal drugs…

[Invasive pulmonary aspergillosis. Occurrence in a non-neutropenic female patient with abdominal sepsis].

Invasive pulmonary aspergillosis (IPA) is a life-threatening infection predominantly affecting immunocompromised patients, e.g. with acute leukemia. This case report demonstrates that IPA can also occur in non-neutropenic critically ill surgical patients. The case of a 63-year-old woman is reported, who developed IPA of the respiratory tract in the course of diffuse purulent peritonitis. First-line therapy with voriconazol failed to be effective. However, application of caspofungin, intensive kinetic therapy (including prone position) and airway management by interventional bronchoscopy enable…

In vitro activity of nine antifungal agents against a global collection of Hortaea werneckii isolates, the agent of tinea nigra.

The in vitro susceptibility of molecularly identified Hortaea werneckii isolates (n = 37), the causative agent of tinea nigra, originating from clinical and environmental sources was determined for nine antifungal agents. Posaconazole had the lowest geometric mean minimum inhibitory concentration (GM MIC) (0.07 µg/mL), followed by voriconazole (0.13 µg/mL), isavuconazole (0.14 µg/mL), itraconazole (0.16 µg/mL), terbinafine (0.19 µg/mL) and amphotericin B (0.92 µg/mL). In contrast, fluconazole (14.56 µg/mL), caspofungin (2.41 µg/mL) and anidulafungin (1.42 µg/mL) demonstrated the highest GM MIC…

Candida krusei pericarditis: A case report and review of the literature.

A 66-year-old female, a known case of endometrial serous carcinoma, presented with a history of poor oral intake. Elevated C-reactive protein was identified, and an echocardiogram showed evidence of large circumferential pericardial fluid with maximum diameter measuring 4.4 cm. Further analysis of the pericardial fluid revealed growth of Candida krusei. Treatment was initiated with caspofungin 150mg intravenously once daily for 28 days, followed by oral maintenance therapy with voriconazole for 10 days. No relapse or re-infection was observed during 10 days of follow-up. A literature search wa…

Fatal mucormycosis and aspergillosis coinfection associated with haemophagocytic lymphohistiocytosis: A case report and literature review.

Invasive mould infections are life-threatening and mainly occur in immunocompromised patients. Whereas aspergillosis is described during haemophagocytic lymphohistiocytosis (HLH), only a few cases of concomitant mucormycosis with HLH have been reported. Here, we present an uncommon coinfection of mucormycosis and aspergillosis associated with HLH probably due to a varicella zoster virus (VZV) viraemia which was unresponsive to triple antifungal therapy (liposomal amphotericin B combined with isavuconazole and caspofungin). A review of the cases of mucormycosis with HLH showed that this uncommo…

Antifungal-Associated Drug-Induced Cardiac Disease.

The etiology of cardiomyopathies are classified into 4 main groupings (dilated, hypertrophic, restrictive, and idiopathic) and can be mechanistically caused by myocarditis, conduction abnormalities, focal direct injury, or nutritional deficiency. Based on our review of this topic, evidence suggests that echinocandin-related cardiac dysfunction is a mitochondrial drug-induced disease caused by focal direct myocyte injury. With caspofungin or anidulafungin administration into the heart via central line, exposure is likely extreme enough to induce the acute toxicity. Chronic or low-dose exposure …

Fatal bronchial invasion of Scopulariopsis brevicaulis in an acute monocytic leukemia patient.

A case of fatal invasion of Scopulariopsis brevicaulis to the bronchus in an acute monocytic leukemia (M(5)) patient is described. This infection leads to mediastinal emphysema, bronchial bleeding, and bronchial obstruction before finally spreading to the entire lung. The patient was initially diagnosed with pulmonary aspergillosis based on clinical signs and morphological examination. However, S. brevicaulis was finally identified by 18S rDNA sequencing. The patient failed lipid amphotericin B therapy and voriconazole plus caspofungin combination therapy. To the best of our knowledge, this is…

Candida utilis candidaemia in neonatal patients.

In recent years, an evident rise in the frequency of candidaemia caused by non-albicans Candida species has been reported. In this paper we present three cases of clinically manifested candidaemia caused by Candida utilis in neonatal patients hospitalized in the same neonatal intensive care unit within a 6 month period. To the authors' knowledge, only two cases of C. utilis candidaemia have been reported in the literature to date, but neither of these involved newborns. Clinical resolution and elimination of C. utilis from the blood were achieved using liposomal amphotericin B or caspofungin i…
