From Pathogenesis to Therapeutics: A Review of 150 Years of Huntington's Disease Research.

Huntington's disease (HD) is a debilitating neurodegenerative genetic disorder caused by an expanded polyglutamine-coding (CAG) trinucleotide repeat in the huntingtin (HTT) gene. HD behaves as a highly penetrant dominant disorder likely acting through a toxic gain of function by the mutant huntingtin protein. Widespread cellular degeneration of the medium spiny neurons of the caudate nucleus and putamen are responsible for the onset of symptomology that encompasses motor, cognitive, and behavioural abnormalities. Over the past 150 years of HD research since George Huntington published his desc…

Acute necrotizing encephalopathy causing human bocavirus.

Acute necrotizing encephalopathy is characterized by multiple, symmetrical lesions involving the thalamus, brainstem, cerebellum, and white matter and develops secondarily to viral infections. Influenza viruses are the most common etiological agents. Here, we present the first case of acute necrotizing encephalopathy to develop secondarily to human bocavirus. A 3-year-old girl presented with fever and altered mental status. She had had a fever, cough, and rhinorrhea for five days. The patient was admitted to the intensive care unit with an initial diagnosis of encephalitis when vomiting, convu

Clinicopathological aspects of motor parkinsonism.

Motor parkinsonism is characterised by bradykinesia, muscle rigidity, postural instability and resting tremor that occur most often in Parkinson's disease, multiple system atrophy and progressive supranuclear palsy. In all three disorders the main regulators of the caudate nucleus and putamen (dopaminergic substantia nigra and glutaminergic caudal intralaminar nuclei), as well as the cortical projection from the presupplementary motor area, degenerate. Degeneration of the major basal ganglia circuit neurons also occurs in multiple system atrophy, while degeneration of the subthalamus and a wid…

Pineal modulation of the rat caudate-putamen spontaneous neuronal activity: roles of melatonin and vasotocin.

The effects of microiontophoretic application of melatonin and melatonin plus vasotocin on spontaneously active neurons of caudate-putamen in sham-operated and pinealectomized rats were studied. Extracellular unit recordings showed that in sham-pinealectomized rats, melatonin ejection primarily produced inhibition of the responsive neurons (74.1%), whereas only 24.9% of the neurons were excited. Iontophoretic ejection of vasotocin or melatonin+vasotocin produced, in both cases, an inhibition of 100% of the responsive neurons. In pinealectomized rats, iontophoretic melatonin ejection produced a

Interhemispheric approach for the surgical removal of thalamocaudate arteriovenous malformations.

A series of 250 surgically treated cerebral arteriovenous malformations (AVM's) is presented, in which 22 lesions were located primarily in the thalamus and caudate nucleus. A standardized interhemispheric approach through the posterior corpus callosum and into the atrium of the lateral ventricle was utilized for the surgical removal of these AVM's. Total removal was confirmed by angiography in 18 patients; removal was subtotal in four cases. There were no deaths in this group of patients. Disturbances of recent memory pre- and postoperatively were seen in half of the patients, but most of the…

Head turning induced by unilateral intracaudate thyrotropin-releasing hormone (TRH) injection in the cat.

Unilateral intracaudate injection of TRH in cats (10 microgram) induced head turning responses similar to the responses produced by dopamine (DA) (10 microgram). Contralateral head turning responses were observed after injections in the central part of the caudate body, whereas, injections placed more laterally induced ipsilateral responses. While haloperidol (3 mg/kg i.m.) suppressed the DA-induced response, it appeared to potentiate the effects of TRH. Cyproheptadine (10 mg/kg i.p.) did not alter the TRH-induced responses. These results suggest that, although TRH induces DA-like effects on p

Meningeal carcinomatosis in patient with esophagus adenocarcinoma.

Meningeal carcinomatosis (MC) is an uncommon from of metastasis of solid tumors. We reported a clinical case of a woman with esophagus adenocarcinoma that a few months after surgical operation, presented episodes of vomiting, nausea, hypertension, mental change, unconsiousness; A contrast-enhanced CT of the brain revealed an "area of enhancement in the caudate nucleus" and cerebrospinal fluid cytologic studies evidenced the presence of carcinomatous cells. The clinical state of the patient deteriorated and she died a few days later. Autopsy confirmed meningeal carcinomatosis without parenchyma…

Alterations in cerebellar glutamic acid decarboxylase (GAD) activity in a genetic model of torsion dystonia (rat).

Glutamic acid decarboxylase (GAD) activity was studied in specific brain regions of a newly identified genetic (rat) model of human torsion dystonia. GAD activity was found to be significantly increased in the deep cerebellar nuclei of dystonic rats at 16, 20, and 24 days of age. GAD activity in the other regions examined (vermis, cerebellar hemispheres, caudate nucleus, and globus pallidus) did not differ from that of age-matched normal littermate controls. Diazepam treatment significantly reduced the frequency of dystonic movements in the mutant.

Percutaneous choroid plexus coagulation in hydranencephaly.

Percutaneous intracranial endoscopy, performed on 3 infants with hydranencephaly and signs of intracranial hypertension, showed normal choroid plexus and choroid arteries in 2 patients. Transendoscopic coagulation of the arteries and plexus alleviated intracranial hypertension postoperatively. Minimal choroid plexus and no choroid arteries were present in the 3rd patient and coagulation did not alter the clinical course. Computerized tomography scans of the infants with normal choroid plexus demonstrated cerebral tissue in the region of the caudate nucleus; the child with minimal choroid plexu…

[Neuroradiological changes by suppression of tics].

Tourette's syndrome is characterized by involuntary tics. First choice of treatment has been pharmacological, but recently, behavioural therapy teaching patients to suppress their tics has been introduced. Neuroimaging studies have shown an increased activity in the prefrontal cortex, temporal lobes and caudate nucleus, and a decreased activity in globus pallidus and putamen during inhibition of tics. The activity in the frontal lobes changes with age, probably caused by a lack of compensatory hypertrophy. In order to fully understand the mechanism behind behavioural therapy further studies ar…

Epidermal nevus syndrome: megalencephaly with bihemispheric and cerebellar involvement: imaging and neuropathologic correlation.

Epidermal nevus syndrome (ENS) is a sporadic neurocutaneous disorder characterized by the combination of epidermal nevi with significant central nervous system, eye, and skeletal abnormalities. Hemimegalencephaly and related brain abnormalities are the dominant neuropathologic abnormalities in ENS. Magnetic resonance imaging features of the case reported here revealed enlargement of both cerebral hemispheres and malformed basal ganglia, with incomplete separation of the caudate nucleus and putamen. In addition, the right cerebellar hemisphere was enlarged, with disorganized folia. None of thes…

[The mechanism of the pathogenesis of drug-induced parkinsonism (experimental biochemical and bioelectrical research on the action of haloperidol)].

Haloperidol was administered to rabbits in a dose 500 micrograms/kg during 60 min or 30 days. Its effects were studied in enzymes controlling neurotransmitters utilization--monoamine oxidase (MAO) A and B, acetylcholine esterase (ACE) in synaptosomes subfractions and mitochondria of the cells--and on the content of biogenic amines in the sensomotor cortex and caudate nucleus tissues. It was found that this neuroleptic induces reciprocal changes in MAO specific activity (a decrease in MAO B activity and an increase in MAO A), a fall in the levels of dopamine and noradrenaline in unchanged level…

Ornithine aminotransferase in Huntington's disease.

Ornithine aminotransferase (Orn-T) activities in Huntington's disease (HD) brains were found to be reduced, when compared to age-matched control brains, by 34-49% in the frontal cortex, parietal cortex, caudate nucleus and putamen. Such changes were not observed in senile dementia of Alzheimer type or schizophrenia. Alterations in choline acetyltransferase activities were consistent with previous findings for these disorders. If Orn-T is involved in the synthesis of neurotransmitter glutamate, the reported losses of Orn-T activity may reflect deterioration of the corticostriatal glutamatergic …

Previous glucocorticoid treatment in childhood and adolescence is associated with long-term differences in subcortical grey matter volume and microstructure.

BACKGROUND: Glucocorticoids are widely used in the treatment of several pediatric diseases with undisputed disease-related benefits. Perinat … ume and mean diffusivity (MD) measures were extracted for hippocampus, amygdala, nucleus accumbens, caudate nucleus and putamen. Multiple linear regression analyses were used to assess differences between patients and controls, and to evaluate possible dose-response relationships. A priori, we expected patients to display lower hippocampal and amygdala volumes. …

Effect of subthalamic nucleus stimulation during exercise on the mesolimbocortical dopaminergic region in Parkinson's disease: a positron emission tomography study.

To elucidate the dynamic effects of deep brain stimulation (DBS) in the subthalamic nucleus (STN) during activity on the dopaminergic system, 12 PD patients who had STN-DBS operations at least 1 month prior, underwent two positron emission tomography scans during right-foot movement in DBS-off and DBS-on conditions. To quantify motor performance changes, the motion speed and mobility angle of the foot at the ankle were measured twice. Estimations of the binding potential of [(11)C]raclopride (BP(ND)) were based on the Logan plot method. Significant motor recovery was found in the DBS-on condit
