Perforated double appendicitis: Horseshoe type.

Appendix vermiformis duplex is an infrequent malformation. However if it is missed out, there might be some complications and medicolegal troubles. A surgeon must be aware of any other appendix during appendectomy. Therefore, the possible locations and shapes described in the Cave-Wallbridge classification should be considered by the surgeon. In this case report, we present a patient with a horseshoe-type dupplication of appendix in a perforated appendicitis diagnosed during an emergency laparotomy.

[Cerebral gas embolism after a cough fit].

An 81-year-old man, with a history of pulmonary tuberculosis presented with coma after coughing associated to haemoptysis. The CT-scan revealed several scattered gas bubbles, while the thoracic scan disclosed an old tubercular parenchymatous cave. In spite of several sessions of hyperbaric oxygen treatment, the patient died with multivisceral organ failure. This case report reviews the pathophysiology of gas embolism due to coughing.

Occupational risk of Histoplasma capsulatum exposure among field biologists in Ecuador: Evidence from skin testing and serology.

Histoplasma capsulatum, a dimorphic fungus, is the causative agent of histoplasmosis, a respiratory disease with a wide clinical spectrum ranging from asymptomatic infection to severe pulmonary or disseminated disease. Although Ecuador is recognized as an endemic country, histoplasmosis remains underreportesd. This study presents the first investigation in Ecuador assessing occupational exposure to H. capsulatum among field biologists and environmental scientists, a group presumed to be at increased risk due to frequent contact with wildlife, caves, and rural environments. A total of 46 worker…

Neurosarcoidosis involving cervical nerve root with unusual MRI findings: a case report and systematic literature review.

BACKGROUND: Neurosarcoidosis is rare, and among its manifestations, nerve root involvement has been reported in only a few cases. Therefore, magnetic resonance imaging (MRI) findings of neurosarcoidosis, particularly those involving nerve roots, are scarce in the literature. METHODS: We presented the case of neurosarcoidosis involving cervical nerve roots and cranial nerves, alongside a systematic literature review. RESULTS: A 28-year-old female suddenly developed right facial numbness as well as left upper extremity and left hand pain. Initial brain and spine MRI showed a bulging mass of T2

The skeleton of Congruus kitcheneri, a semiarboreal kangaroo from the Pleistocene of southern Australia.

The macropodine kangaroo, Wallabia kitcheneri, was first described in 1989 from a Pleistocene deposit within Mammoth Cave, southwestern Australia, on the basis of a few partial dentaries and maxilla fragments. Here, we recognize W. kitcheneri within the Pleistocene assemblages of the Thylacoleo Caves, south-central Australia, where it is represented by several cranial specimens and two near-complete skeletons, a probable male and female. We reallocate this species to the hitherto monotypic genus Congruus. Congruus kitcheneri differs from all other macropodid species by having a highly unusual …

Capsule endoscopy as a diagnostic test for Meckel's diverticulum.

OBJECTIVE: We described a case series of patients diagnosed with Meckel's diverticulum using capsule endoscopy (CE), reviewed the literature, and aimed to reveal the common CE characteristics of Meckel's diverticulum. MATERIALS AND METHODS: We reviewed a case series of Meckel's diverticulum in the Department of Gastroenterology, Beijing Shijitan Hospital Affliated to the Capital Medical University. In each case, primary upper endoscopy and colonoscopy failed to reveal a reason to explain the complaints (hematochezia in three cases and abdominal pain in one). CE (Pillcam SB2, Given Imaging, Yo

Renal Cell Carcinoma with Primary Presentation via Metastasis to the Trigeminal Ganglion.

BACKGROUND: Renal cell carcinoma (RCC) accounts for ∼8% of all brain metastatic disease; however, spread to the cranial nerves and their ganglia is uncommon. To the best of our knowledge, we report the first case of RCC metastatic to Meckel's cave, which was diagnosed secondary to new trigeminal sensory loss. METHODS: A 45-year-old man had presented with acute-onset right V3 numbness. Magnetic resonance imaging demonstrated contrast enhancement of the ipsilateral V3, extending from its root at the Gasserian ganglion to the foramen ovale. RESULTS: He elected for observation, and his symptoms

Some usual and unusual poisonings due to carbon monoxide.

Carbon monoxide poisoning is a frequent occurrence in both developed and under developed countries of the world. Carbon monoxide can be produced in fires, automobile engine exhausts and the incomplete combustion of organic matter. It is a "silent killer" that initially produces a mild progressive frontal headache, drowsiness and sleep that is usually ignored as common place. Continued low-level CO exposure for a long period of time in a confined space is cumulative and these accidental deaths are frequent but should be avoidable. Several usual and unusual poisonings are reported to illustrate

Perineural extension of cutaneous desmoplastic melanoma mimicking an intracranial malignant peripheral nerve sheath tumor. Case report.

The authors present a case illustrating the importance of obtaining a biopsy of any facial skin lesions in a patient presenting with an intracranial tumor involving the facial or trigeminal nerve. Conventional malignant melanoma metastasizes to the brain frequently and does not usually pose diagnostic difficulties. Direct intracranial spread of cutaneous melanoma is rare. In our patient, desmoplastic melanoma with perineural spread to the Meckel cave mimicked a malignant peripheral nerve sheath tumor clinically, radiographically, and histologically.

[Surgical treatment of chronic constrictive pericarditis].

From April, 1969 to December, 1988, partial pericardiectomy was performed in 78 patients with chronic constrictive pericarditis. The long-term follow-up study in 60 patients showed a satisfactory functional result. The authors considers that the median sternotomy is a better approach, the effectiveness of operation should be gauzed by dropping the vena cave pressure below 140 mmH2O, resection of pericardium from left to right is logical, and rational use of cardiotonic agents is important.

Transoval trigeminal cisternography.

A new water-soluble contrast medium, metrizamide (Amipaque) was used to visualize the subarachnoid space within Meckel's cave. By a needle introduced through the foramen ovale, successful filling of the trigeminal cistern was accomplished in 48(80%) of 60 patients. Transoval trigeminal cisternography offers a possibility to control the position of the tip of the needle in various blocking procedures of the trigeminal ganglion, and may also be used for target localization in stereotactic radiosurgery in the treatment of trigeminal neuralgia.

Double dehiscence (Superior semicircular canal and tegmen tympani) in the epicampaniform period (Arboli type).

PURPOSE: Although the superior semicircular canal dehiscence syndrome was described at the end of the 20th century, we want to check if it is a pathology that has existed since ancient times, through the anthropological study of bone remains. METHODS: We have carried out an anthropological and radiological study (CT scan) of 8 skulls found in caves, as secondary burials of the Arbolí type epicampaniform culture (1800 - 1700 BC) on the Iberian Peninsula. RESULTS: The 8 skulls (16 temporal bones) show a grade 4 degree of pneumatisation or hyperpneumatization. One of these skulls, belonging to …

Pathogenicity and virulence of Marburg virus.

Marburg virus (MARV) has been a major concern since 1967, with two major outbreaks occurring in 1998 and 2004. Infection from MARV results in severe hemorrhagic fever, causing organ dysfunction and death. Exposure to fruit bats in caves and mines, and human-to-human transmission had major roles in the amplification of MARV outbreaks in African countries. The high fatality rate of up to 90% demands the broad study of MARV diseases (MVD) that correspond with MARV infection. Since large outbreaks are rare for MARV, clinical investigations are often inadequate for providing the substantial data ne…

Observations of ozone, acyl peroxy nitrates, and their precursors during summer 2019 at Carlsbad Caverns National Park, New Mexico.

Carlsbad Caverns National Park (CAVE) is located in southeastern New Mexico and is adjacent to the Permian Basin, one of the most productive oil and natural gas (O&G) production regions in the United States. Since 2018, ozone (O3) at CAVE has frequently exceeded the 70 ppbv 8-hour National Ambient Air Quality Standard. We examine the influence of regional emissions on O3 formation using observations of O3, nitrogen oxides (NOx = NO + NO2), a suite of volatile organic compounds (VOCs), peroxyacetyl nitrate (PAN), and peroxypropionyl nitrate (PPN). Elevated O3 and its precursors are observed whe

Correction of the pinched nasal tip with alar spreader grafts.

A pinched nasal tip is caused by collapsed alar rims secondary to weak lateral crura. The resulting deformity can be corrected with alar spreader grafts--autogenous grafts of septal or auricular cartilage that are inserted between and deep to the remaining lateral crura to force them apart, propping up the caved-in segment. We describe the surgical technique, indications, and variations in design of alar spreader grafts and present representative results from our series of 38 patients.

Pulmonary histoplasmosis diagnosed in a Japanese woman after traveling to central and South America: A case report.

Histoplasmosis is a fungal infection caused by Histoplasma capsulatum, and Japan is considered a non-endemic area for histoplasmosis. Most patients diagnosed with histoplasmosis in the past usually have exposure to caves and bat guano with travel history to endemic areas. Therefore, travel history and risk activities should be comprehensively assessed when suspecting histoplasmosis because this important information may be overlooked. Although few, possibilities of indigenous cases have also been suggested. Moreover, it is assumed that the number of travelers and endemic mycoses has decreased …
