Fractures of the atlas vertebra in the pediatric population are very rare, yet must be considered when evaluating a child with neck pain and torticollis. Plain radiographs and computerized tomography should be used in conjunction to confirm the diagnosis.

The position of the 1st cervical vertebra is investigated in relation to special anomalies of the craniofacial morphology. Two extremely different types like prognathism and progenia are chosen to demonstrate an interdependence. The latter is probably caused by a different head posture which in itself is influenced by the maxillo-facial anomalies. With the aid of the literature the findings can be explained on a functional basis.

Trigeminal involvement in syringomyelia or in a malformation of the occipito-vertebral junction may rarely be the presenting feature of the disorder. Two cases are reported: in the first, recurrent pruritus of the side of the nose due to sensory involvement of CN V was the presenting feature of syringomyelia; in the second, a malformation of the occipito-vertebral junction presented with refractory and painful trismus by irritation of the motor part of the trigeminal nerve.

OBJECTIVE: To explore radiographic results and clinical effects of posterior atlanto axial vertebra internal fixation in treating instability of occipitocervical.
METHODS: The clinical data of 155 patients with instability of occipitocervical treated by posterior atlanto axial vertebra internal fixation were respectively analyzed from September 2005 to January 2011. There were 68 males and 87 females, ranging in age from 6 to 75 years old with an average of 45.6 years old. Of them, 53 cases were fresh odontoid fractures(Aderson type II C), 30 cases were os odontoideum, 20 cases were old odont

BACKGROUND: Synostosis or fusion of atlas with occipital bone is known as occipitocervical synostosis, occipitalization of the atlas, or atlanto-occipital fusion. This is a rare congenital malformation at craniovertebral junction. Its incidence ranges from 0.08%-3% in general population. Occipitocervical synostosis result in narrowing of foramen magnum which may compress the brain stem, vertebral artery and cranial nerves. Knowledge of occipitocervical synostosis is important for the surgeons during the surgeries in the craniovertebral region. Hence, the present study was undertaken to determi

Understanding brain pathology along with the underlying neuroanatomy and the resulting neurological deficits is of vital importance in medical education and clinical practice. To facilitate and expedite this understanding, we created a three-dimensional (3D) interactive atlas of neurological disorders providing the correspondence between a brain lesion and the resulting disorder(s). The atlas contains a 3D highly parcellated atlas of normal neuroanatomy along with a brain pathology database. Normal neuroanatomy is divided into about 2,300 components, including the cerebrum, cerebellum, brainst

BACKGROUND: Airway compromise secondary to isolated injury at the atlas (C1) and axis (C2) without an associated spinal cord injury is a rare, but recognized phenomenon that results in significant morbidity and mortality. No previous study in the literature has reported the incidence of this potentially lethal complication of these relatively common fractures.
METHODS: The medical records for 625 consecutive patients who presented to a Level I trauma center with C1 and C2 fractures during the years from 1996 to 2005 were reviewed retrospectively. Strict inclusion and exclusion criteria were a

OBJECT: Rigid fixation of the upper cervical spine has become an established method of durable stabilization for a variety of craniocervical pathological entities in children. In children, specifically, the use of C1-2 transarticular screws has been proposed in recent literature to be the gold standard configuration for pathology involving these levels. The authors reviewed the use of rigid fixation techniques alternative to C1-2 transarticular screws in children. Factors evaluated included ease of placement, complications, and postoperative stability.
METHODS: Seventeen patients, ranging in

The unique anatomy of the upper cervical spine and the typical mechanisms of injury yield a predictable variety of injury patterns. Traumatic ligamentous injuries of the atlanto-occipital joint and transverse atlantal ligament are relatively uncommon, have a poor prognosis for healing, and often respond best to surgical stabilization. Bony injuries, including occipital condyle fractures, atlas fractures, most odontoid fractures, and traumatic spondylolisthesis of the axis, generally respond well to nonsurgical management. Controversy in management remains, however, especially with type II odon

PURPOSE: Our study highlights the morphometry of the partial and complete atlas occipitalization (AOZ), its coexistence with fusions of the 2nd and 3rd cervical vertebrae and morphological and morphometric abnormalities of the posterior cranial fossa that are of paramount neurological importance.
METHODS: One hundred and eighty adult dry skulls, the atlas and axis vertebrae were examined.
RESULTS: Four skulls (2.2 %) showed AOZ. Two of them (1.1 %) presented a partial AOZ, one male skull (0.6 %) a complete AOZ and a female skull (0.6 %) had a fused left hemiatlas with the occipital bone and

Posttraumatic chronic atlantoaxial rotatory subluxation and congenital absence of the posterior arch of the atlas are rare upper cervical spine abnormalities. The present case is that of a 4-year-old girl who had these two spinal disorders as well as spastic cerebral palsy. The interrelationship, if any, between these three conditions is unclear but presented an unusual diagnostic triad. A posterior spinal fusion between the occiput and third cervical vertebra was performed because of concern for upper cervical spine instability. The patient was immobilized in a halo vest, and a solid fusion o

Objective: To determine the craniocervical posture in 120 subjects, 90 patients with temporo- mandibular joint disorders (TMD) of varying degrees were selected and compared with those of 30 healthy individuals. Methods: True lateral radiographs in the natural head position were obtained from TMD patients and healthy individuals. Linear measurements from the opisthion to the intersecting point of the craniovertebral angle, the opisthion to the tip of the spinous process of the seventh cervical vertebrae, and the body of Atlas to the lower border of the seventh cervical vertebrae were obtained. Two 8-year-old boys presented with complaints of torticollis and pain on neck turning. Both patients had a history of throat infection. Radiography and computed tomography demonstrated atlanto-axial subluxation. The patients were treated under diagnoses of Grisel's syndrome, unilateral or bilateral subluxation of the atlas on the axis associated with infection in the head or neck. Both patients were treated conservatively with bed rest, muscle relaxants, non-steroid anti-inflammatory agents, and immobilization with a soft cervical collar and cervical halter traction. The torticollis had resolv

Injuries to the upper cervical spine (C0-C2) play a major role in surgical treatment of traumatic sequelae in the entire cervical spine. Even though the number of such operations has increased in recent years, there are no clear treatment recommendations for most types of cervical spine injuries. In view of the wide range of injury types and the correspondingly large number of treatment options, this review focuses mainly on the following types of injuries: C0 fractures, occipital condyle fractures (OCF), atlanto-occipital dislocation (AOD), atlas fractures, atlantoaxial dislocation (AAD), and

A case of bilateral fenestration of the vertebral artery at the level of the atlas in a patient who had occipital neuralgia and cervical myelopathy is presented. MRI and vertebral angiogram demonstrated the fenestrated vertebral artery compressing the upper cervical cord. Surgical decompression for the C-1 and C-2 sensory roots and the upper cervical cord was performed. Fenestration of the vertebral artery is mostly of no clinical significance. However, considering the pathway of the fenestrated vertebral artery, it is quite possible that the fenestrated vertebral artery might compress the neu
